СОВРЕМЕННЫЕ АНТИКОАГУЛЯНТЫ В ПРОФИЛАКТИКЕ РАЗВИТИЯ ИНСУЛЬТА У ПАЦИЕНТОВ С ФИБРИЛЛЯЦИЕЙ ПРЕДСЕРДИЙ

Аннотация

Кардиоэмболические инсульты (КЭИ), на долю которых приходится около 20% всех ишемических поражений головного мозга, представляют собой один из важных аспектов проблемы инсульта. Адекватная профилактика КЭИ у пациентов с фибрилляцией предсердий (ФП) возможна только при назначении пероральных антикоагулянтов, из которых наиболее широко применяется варфарин. Однако его применение сопряжено с рядом трудностей: нежелательные взаимодействия с лекарственными препаратами и пищевыми продуктами, узкое терапевтическое окно и т.д. Альтернативой ему является дабигатран, который не уступает варфарину в снижении риска инсульта у лиц с ФП, однако в дозе 150 мг/сут он чаще варфарина вызывает массивные желудочно-кишечные кровотечения. Прямой ингибитор фактора Ха ривароксабан в исследовании ROCKET продемонстрировал большую, чем у варфарина, эффективность в снижении риска острых нарушений мозгового кровообращения (ОНМК) и внемозговой эмболии у пациентов с ФП, а по показателям безопасности он не отличался от варфарина. Таким образом, ривароксабан может стать перспективной альтернативой варфарину в профилактике ОНМК у лиц с ФП.
