CELL CHRONO BLOCKERS AND NUTRITIONAL SUPPORT TO GERIATRIC PATIENTS IN ORDER TO PREVENT AGE-ASSOCIATED DISEASES AND SYNDROMES
Abstract
The article examines the role of cell chronological and nutritional support to geriatric patients in order to prevent age-associated diseases and syndromes. The published scientific materials on this problem are analyzed. Malnutrition syndrome covers 50% or more of the elderly and senile age and is one of the most common geriatric syndromes, especially in hospitalized patients. In multimodal programs of treatment, prevention and rehabilitation of geriatric patients, cell chronoblockers (succinic acid, taurine, beta-alanine, methylsulfonylmethane, alpha-lipoic acid, coenzyme Q10, L-carnitine, L-arginine, Selenomethionine) showed their effectiveness.

EXPERIENCE WITH QUALIFIED TOBACCO SMOKING CESSATION TELE-ASSISTANCE VIA THE INTERNET
Abstract
One of the main areas of action in the fight against the tobacco smoking (TS) epidemic is to organize and introduce various forms of qualified TS cessation assistance in practical public health service. The paper reports the experience in organizing and introducing qualified TS cessation assistance in practice through the Internet. It describes a system to inform the population about this type of assistance and a scheme of its implementation. Demographic characteristics, the features of a smoking history, and the effectiveness of tele- assistance to 1256 smokers who have used the Internet for smoking quitting have been analyzed. The advantage of assistance delivered via the Internet is its wide availability for people living in the country's most different regions and the low cost of its organization and implementation. The disadvantage of Internet assistance is limited abilities to monitor patients and to assess the results of care provided. The analysis of the authors’ own experience and the data available in the literature has led to the conclusion that there is an absolute prospect for the development and dissemination of qualified TS cessation assistance through the Internet.

COMPLICATIONS OF DIABETES MELLITUS IN ITS CONCURRENCE WITH PULMONARY TUBERCULOSIS
Abstract
Complications of diabetes mellitus (DM) are observed in 80.7% of patients in whom DM is concurrent with tuberculosis. The complications of DM were more commonly observed in patients aged older than 50 years. The incidence of complications increased in parallel with the longer duration of DM.

HIV-ASSOCIATED TUBERCULOUS MENINGOENCEPHALITIS: RARITY OR REGULARITY?
Abstract
The medical records of 244 tuberculosis (TB) patients and 125 HIV/TB ones followed up by the Regional TB Dispensary and the Avtozavodsky TB Dispensary in Nizhny Novgorod were analyzed. The patients with TB/HIV tuberculosis were found to have a high mortality rate from tuberculous brain lesion: 34% in 2016 and 29% in 2017. The causes of tuberculosis spread in the penitentiaries were analyzed. An example of HIV-associated meningoencephalitis in a representative of a special contingent of the Nizhny Novgorod region is given. The paper gives an example of HIV-associated meningoencephalitis in a representative of the prison population in the Nizhny Novgorod Region.

THE FEATURES OF NEUROIMMUNE ENDOCRINE AND GERIATRIC STATUS IN PATIENTS WITH CORONARY ARTERY DISEASE SCHEDULED CORONARY ARTERY BYPASS SURGERY
Abstract
The article presents an overview of the features of neuroimmunoendocrine and geriatric status in patients with coronary heart disease, which are scheduled on a planned coronary artery bypass grafting. The possible effect of the dynamics of proinflammatory immune response on the development of adverse events in such patients is described. It is shown that the frailty syndrome and the main geriatric syndromes can be predictors of adverse events after a planned coronary artery bypass grafting. The data of recent studies on biomarkers of fatal and nonfatal complications, as well as innovative developments of ways to identify the risks of adverse events in such patients are presented.

INFLAMMATORY PSEUDOTUMORS OF THE ILEOCECAL ANGLE
Abstract
The differential diagnosis between inflammatory and neoplastic lesions of the ileocecal angle is a difficult task of abdominal surgery. At the same time, rather informative diagnostic techniques, such as ultrasound, computed tomography, and colonoscopy, do not always help a physician make the right diagnosis before complications dictating the need for surgery develop. These observations clearly demonstrate complexities in the timely diagnosis of inflammatory pseudotumors of the ileocecal region, which have required surgical treatment.

ASSESSMENT OF MICROVASCULAR CHANGES BY CAPILLAROSCOPY IN PATIENTS WITH FOOT MYCOSES AND RECURRENT LOWER EXTREMITY ERYSIPELAS
Abstract
The mortality rates were estimated in patients with mycosis and recurrent erysipelas of the lower extremities. Capillaroscopy was used for the first time, which could identify predominantly spastic-atonic microcirculatory disorders in 83.9% of these patients. The efficiency of therapy was also evaluated. It was concluded that combination treatment for foot mycosis, by applying magnetic therapy, could reduce the mean diameters of the venous portion of the microvasculature by 1.5 times, bring the mean diameters of its arterial and transitional portions to the normal limits, and also increase the rates of clinical cure by 14.3% and mycological cure by 17.8% compared with those of standard therapy. The findings may suggest that it is feasible to use capillaroscopy in patients with foot mycoses associated with recurrent lower extremity erysipelas for identification of the type of microcirculation, which is important for the choice of therapy, as well as to evaluate the efficiency of magnetic therapy in these patients.

IMPACT OF STRESS ON THE DEVELOPMENT OF CARDIOVASCULAR DISEASES
Abstract
The paper describes the impact of stress on the cardiovascular system and the development of cardiovascular diseases and considers the possibilities of their correction using the combined drug Stressovit. It also depicts numerous positive effects of Stressovit, the components of which contribute to relieving emotional stress, nervousness, anxiety, and irritability, increasing stress resistance, improving mood, enhancing mental and physical performance, and eliminating the autonomic manifestations of stress. Stressovit has a good safety profile and can be recommended for cardiovascular diseases.

THE SOCIOMEDICAL AND LEGAL ASPECTS OF PROVIDING MEDICAL CARE TO TUBERCULOSIS PATIENTS AT THE PRESENT STAGE OF DEVELOPMENT OF TB CARE
Abstract
The analysis of the existing provisions of the regulatory and legal framework governing the rights of citizens with tuberculosis was checked. The legal framework in phthisiology is developing, but, due to the lack of an implementation mechanism, a number of regulatory documents are difficult to apply in practical work. A certain part of citizens do not fulfill their obligations not to spread tuberculosis infection without any consequences for their asocial lifestyle. Inconsistency in the need to comply with medical secrecy with an epidemiological investigation and the implementation of anti-epidemic measures in some cases causes moral damage to the patient, which can serve as a reason for commencement of an action against the dispensary. It is necessary to inform physicians about the social problems of patients with tuberculosis, which will enhance patient compliance. Effective protection of the interests of the individual and society requires further improvement of legal regulation in the fight against the spread of tuberculosis in the Russian Federation.
