Abstract

Abstract

The relevance of diabetes as a problem of non-communicable diseases has not decreased, but due to the growth of its prevalence has increased significantly. Diabetes, along with obesity and tobacco Smoking, is a leading risk factor for noncommunicable diseases. The article provides data reflecting the epidemiological, pathophysiological, and diagnostic aspects of diabetes mellitus, and describes in detail the recommendations that should be given to patients by doctors of the primary health care network. Abstract

The literature review provides information about the principles of radical pathogenetic treatment for hemorrhoid disease (HD), the emergence and further development of donor techniques that have served as the basis for developing the hemorrhoidal artery ligation and rectoanal (HAL-RAR) technology; and analyzes the reasons for its effectiveness due to the combination of two different approaches that have evolved independently of one another until a certain moment. The combination of these techniques has been incentive to a technological breakthrough in the treatment of HD and to the better quality of life in patients after surgical treatment. HAL-RAR has proven to be a highly effective and safe technique for the surgical treatment of HD. The relatively rare failures that occur after the HAL-RAR technology and lead to the need for resection procedures in the future do not reduce its popularity. One of the main advantages of this technology is the simplicity of its technical execution and minimal requirements for the skill and experience of the operator. Another important feature may be considered to be the ability to adapt the HAL-RAR technology for use in both outpatient settings and day hospitals without the need for long-term medical observation of patients.

Main groups of proprotein convertase subtilisin/kexin type 9 inhibitors: mechanisms of action and clinical efficacy. Part 1

Abstract

Since establishing the important role of cholesterol and low-density lipoproteins (LDL) in the pathogenesis of atherosclerosis and cardiovascular diseases, many researchers have focused on developing drugs that lower the levels of atherogenic lipoproteins. Thanks to the work of Japanese researcher Akira Endo, the first effective anti-atherosclerotic drugs - statins-were created in the 1970s and 80s. The mechanism of hypocholesterolemic action of statins is based on competitive inhibition of the enzyme 3-hydroxy-3-methylglutaryl-coenzyme A (HMGCoA) - reductase, which is necessary for cholesterol biosynthesis. In 2003, a new enzyme, proprotein convertase subtilisin - Kexin type 9 (PCSK9), was discovered and a new mechanism for regulating LDL levels in blood serum. As a result of research, the PCSK9 enzyme has been considered as a target for therapeutic effects to reduce cholesterol and LDL levels. To date, several groups of drugs that inhibit PCSK9 have been developed. In this article, we will review the mechanisms of action and clinical efficacy of the main groups of PCSK9 inhibitors. The first part of the article is devoted to preparations of the group of monoclonal antibodies against PCSK9, antisense oligonucleotides, and small interfering ribonucleic acids.

Abstract

The high prevalence of cardiovascular diseases and their pathophysiological association with various comorbid conditions contributes to the progressive increase in the number of patients with a concurrence of several diseases. Combined pathology creates a new clinical situation that requires the consideration of its features when choosing therapeutic, diagnostic, and preventive strategies. A comorbid patient is a serious problem for any clinician, since in this case it is necessary to take into account several components of therapy, which makes it difficult to control treatment efficiency, contributes to an increase in polypharmacy, the risk of local and systemic side effects of drugs, and reduces adherence to therapy. Treatment for comorbid disorders requires careful compliance with the requirements of rational pharmacotherapy, high clinical professionalism, and a wide range of knowledge of related specialties. The paper describes and discusses the clinical case of a comorbid patient in the practice of a general practitioner. It gives the steps in the performed diagnostic search, by verifying the clinical diagnosis and prescribed pharmacotherapy strategies. The presented data show that it is difficult for a physician to avoid polypharmacy in case of comorbidities. Such difficulties are daily encountered by any clinician regardless of his/her clinical experience and awareness of medical science.

Treatment options for overactive bladder and urinary incontinence in patients with comorbid conditions

Overactive bladder (OAB) and urinary incontinence (UI) have high prevalence in both genders after the age of 40 years. Urgency is the leading OAB symptom negatively impacted on patient quality of life. Antimuscarinic therapy is the primary of OAB and UI treatment. Solifenacin due to high-selectivity relieves OAB symptoms while good tolerability including patients with cardio-vascular comorbidity and seniors and improves quality of life. Management of women in postmenopause and men with benign prostatic hyperplasia needs complex treatment approach. Generics improves therapy affordable and treatment adherence.

Abstract

The article presents the clinical, anamnesis and prognostic significance of a complex perinatal assessment in a comparative analysis of the tactics of respiratory therapy in children with extremely low body weight (ELBW) in the intensive care unit of newborns (NICU). Perinatal (maternal and neonatal) risk factors and the degree of progression of intraventricular hemorrhage (IVN) in the early neonatal period were studied based on the study of modern domestic and foreign scientific studies and our own clinical observation data in 2 children with EBMT. In a comparative analysis of cardiorespiratory predictors, clinical picture of varying severity of respiratory failure and gestational morphometry, a differentiated approach to the choice of the optimal tactics of initial respiratory therapy in children with EBMT in an ICU in the early neonatal period was considered.

Analysis of the pattern and course of juvenile idiopathic arthritis in children of the Samara Region

Abstract

Juvenile idiopathic arthritis (JIA) is the most common chronic rheumatic disease in children and one of the most frequent childhood chronic diseases. Patients with continuous disease progression (33 to 75%) and insufficient efficiency of various types of basic therapy are currently prevalent in the pattern of JIA. It is important for the physician to predict the outcomes of the disease and to determine the clinical predictors of a long-term adverse JIA outcome. Objective. To study the pattern and course of JIA in children living in the Samara Region to optimize approaches to therapy. Subjects and methods. The gender and age characteristics of patients with JIA were studied. The diagnosis was verified according to the ILAR criteria. The pattern of JIA in the Samara Region was presented. The authors analyzed risk factors for, the mean age at onset of JIA in various types of its course, the involvement of different joints in the pathological process, the activity of JIA, X-ray characteristics and functional class in the most common variants, as well as concomitant diseases. Biological therapy, the mean duration of biological agent (BA) use and the mean age of patients receiving the treatment were analyzed. Results. The presented information on the pattern and course of JIA in the Samara Region, the time of diagnosis and the timing of prescribed therapy, including that with BAs, and the reasons for their discontinuation allow us to apply a differentiated approach to therapy and to evaluate its efficiency and safety.

Silent myocardial ischemia in young women with cardialgia, dyshormonosis, and various comorbidities (cardiac and extracardiac ones)

Abstract

The paper presents the results of Holter ECG monitoring in 126 women with Stages I-II hypertension and noncoronarogenous cardialgia-type cardiac pain syndrome in the late fertile period with different levels of estradiol and follicle-stimulating hormone (FSH) according to the cardiac and extracardiac comorbidities in an outpatient setting. The risk of silent myocardial ischemia (SMI) is higher in hypertensive women with a history of chronic ovarian diseases, medical abortions, bad habits (smoking), and a genetic predisposition to hypertension. SMI occurs in 18.8% of the women with hypertension and cardialgia in the late fertile period with chronic reproductive system diseases, a family gynecologic history, premenstrual syndrome, and, possibly, as a consequence, altered estradiol and FSH levels.

Abstract

Objective of the Review: to study and systematize the clinical and experimental research data about menopause and disorders associated with sex hormone deficiency, to assess the safety and efficiency of correction of disorders in menopause, using menopausal hormone therapy (MHT) or alternative methods. Key Points: Menopause is often accompanied by variable symptoms and a decrease in the quality of life. MHT is recognized as the most effective way to treat the symptoms of sex hormone deficiency and with an individual approach to the appointment of therapy, the risk of complications is minimal. The effectiveness of alternative methods for correcting the symptoms of menopausal syndrome (MS) was been confirmed. Conclusion: MHT is indicated for all patients with MS, but there is low awareness of the effectiveness and high safety profile of such treatment. Alternative therapy for MS is the main choice for patients with a high risk of complications or if the patient refuses to use MHT.

The use of combined recombinant interferon alpha-2b medications in the treatment of allergic rhinitis

Abstract

Objective: to present the experience with combined recombinant interferon alpha-2b medications for the topical treatment of seasonal allergic rhinitis (AR) exacerbations. Materials and Methods: The study design was a simple prospective one. This study was conducted on the basis of the Department of Allergology and Immunology of the Omsk City Polyclinic No. 12. The study involved 100 patients who had consulted an allergist about allergic rhinitis (AR) exacerbations - men and women aged 18 to 55 years, mean age (Me 25;75) - 33.0 (28;47) years. There were two consecutive groups of patients: those with acute non-severe seasonal AR and those with acute moderate-to-severe seasonal AR. A group of patients with non-severe AR (group 1; n=50) used the combined interferon-based medication Allergoferon (gel) to treat the disease in the acute phase. A group of patients with moderate-to-severe AR (group 2; n=50) used Allergoferon beta (nasal and eye drops) for the disease treatment in the acute phase. The patients’ conditions were assessed during follow-up examinations on the 7th (visit 2) and 14h (visit 3) days of treatment. Subjective symptoms were evaluated by the questionnaire-based method. This helped to determine the treatment effectiveness as well as the presence and severity of side effects. The effectiveness was assessed based on the reduction in the severity of local and general reactions. Tolerability of the medications was determined according to the patients subjective assessment. Results: Control examinations of patients in both groups were conducted on the 7th and 14th days of treatment. On the 7th day of treatment the patients in both groups reported improvement: in the group 1 (patients with non-severe AR) symptoms resolution was noted by 50% of patients, in the group 2 (patients with moderate-to-severe AR) - by 80%. In the group 1 some patients had nasal congestion (50%), eyelids itching (50%), nasal discharge (40%) and sneezing (50%), but the severity of these symptoms (in points) significantly decreased. In the group 2 some patients had the following symptoms: nasal congestion (38%), nasal discharge (31%), sneezing (20%) and eyelids itching (25%), but they reported much less severe symptoms. Patients in both groups had a significant reduction in the severity of all symptoms (nasal congestion, eyelids itching, nasal discharge, sneezing). No side effects were reported. Conclusion: During our study it was noted that both examined interferon-based medications (Allergoferon and Allergoferon Beta) allowed us to achieve a relief of subjective and objective symptoms of AR. In both groups a good tolerability of these medications was noted, no side effects and no cases of withdrawal or refusal were observed.

Abstract

In the article, the authors describe the experience of using in clinical practice the oral fibrinolytic in the conservative treatment of acute combined thrombosis of the saphenous and deep veins of the lower extremities against the background of the use of anticoagulants, flavonoid drugs, elastic compression of the lower extremities. The results of using the proposed treatment regimen in relation to the therapy of this pathology without prescribing a thrombolytic agent after 1, 3 and 6 months of treatment at the outpatient stage are presented. The dynamics of recanalization of the venous bed and the dynamics of reduction of clinical manifestations of the disease are analyzed. The safety of the treatment is also assessed.
