Abstract

The paper analyzes an update on the etiology, mechanisms, and symptoms of the common clinical and laboratory syndrome associated with a reduction in blood potassium level to <3.5 mmol/l. The etiological and pathogenetic classification of hypokalemia has been developed, which reflects a wide range of physiological and pathological conditions accompanied by this dangerous homeostatic disorder. The author details the basic mechanisms of hypokalemia, shows difficulties in clinical and laboratory diagnosis, and specifies ways, means, and methods to overcome the diagnostic difficulties. A scheme/algorithm is proposed to diagnose the main varieties of hypokalemia. Safe approaches to its elimination are discussed.

Abstract

Atopic dermatitis (AtD) is a chronic inflammatory skin disease, the basis for the pathogenesis of which is an incompetent skin barrier with an immune system imbalance. The skin forms mechanical and immune barriers that protect the body from aggressive environmental factors. Innate immunity imbalance leads to skin infection with opportunistic bacteria that maintain the local inflammatory process. Dry skin is now considered as an auxiliary criterion for AtD. Some experts are ready to include this factor for the progression of AtD in a set of its main criteria. The paper considers the experience with the new cream, the main component of which is the ectoin molecule at a concentration of 7%. There has been recently a noticeable increase in the number of works dealing with different therapeutic and prophylactic properties of ectoines, which are useful in medicine and cosmetology. Ectoin used as part of the agent to relieve inflammatory reactions in allergies and AtD has a certain positive effect. Skin hydration control along with anti-inflammatory therapy can contribute to reducing the risk of more severe forms of the disease, by maintaining the achieved remission of AtD.

Comparative analysis of skin functional parameters in persons of different age groups after retinoic acid peeling

Abstract

Retinoids are widely used to smooth skin texture and to correct wrinkles during photoaging. Fifty-nine patients of different age groups, from 32 to 59 years old, who were treated with chemical peeling with retinoic acid (RA), were examined for skin functional parameters after its peeling. RA peeling produces a mild whitening effect in all age groups. After peeling, the skin erythema is increased by 10%; the skin around the eyes may be more sensitive, its sensitivity does not depend on the age of a patient. The water content in the epidermal stratum corneum increases in persons of all ages after RA peeling. The detected changes may suggest the intensity of skin metabolic processes throughout the follow-up period; therefore, a study of the longterm results of peeling is necessary for evaluation of its efficiency.

Costa acrokeratoelastoidosis: a rare case

Abstract

The paper considers the etiopathogenetic factors of Costa acrokeratoelastoidosis that is a type of focal palmoplantar keratoderma with predominantly autosomal dominant inheritance and is accompanied by an elastic tissue lesion. It gives clinical and morphological criteria for verifying this pathology: childhood or adolescence diseases; focal palmoplantar keratoderma that is manifested by thickening of the stratum corneum of the palms and soles, by multiple ochroleucous horny papules, hyperhidrosis, as well as by histological changes characteristic of Costa acrokeratoelastoidosis (hyperkeratosis, acanthosis, granulosis of the epidermis and elastorrhexis) in the reticular dermis. The paper describes a rare sporadic case of non-hereditary Costa acrokeratoelastodosis. Its differential diagnosis is performed with focal acral hyperkeratosis and Devergie’s disease. The authors show the important diagnostic role of a histopathological examination of papular elements with the mandatory use of histochemical staining of elastic fibers

The paper gives the results of identifying risk factors (RFs) and target organ damages in obese hypertensive patients, as well as data on evaluating the efficiency of 6-month non-drug and triple combination antihypertensive medication therapy. Active non-drug and triple combination drug therapy in obese hypertensive patients had a positive impact on the modifiable RFs: waist circumference, epicardial fat thickness, and atherogenic index significantly decreased; there was a tendency to lower fasting blood glucose levels. Combination therapy was established to have significant antihypertensive and organ protective efficacies.

Hyaluronic acid (HA) preparations have been known for a rather long time. More than 20 years ago, the Food and Drug Administration (FDA) in the USA permitted their clinical use. Vast experience reflected in numerous high-level studies and meta-analyses has been accumulated during this time. The paper presents data on the pathogenetic substantiation of the clinical use of HA preparations, their results, including those in comparison with other drugs for the therapy of osteoarthrosis (OA). Indications for their use, efficacy, and safety during intra-articular injection are separately considered. The role of HA preparations in the treatment of OA is defined on the basis on the above data.
