Влияние медицинской телекоммуникации на индивидуальную гигиену полости рта у детей с терминальной стадией хронической болезни почек

Аннотация
Цель. Оценить влияние медицинской телекоммуникации между детскими врачами-стоматологами, нефрологами и пациентами с терминальной стадией хронической болезни почек (тХБП) на качество индивидуальной гигиены полости рта в условиях стационара.
Материал и методы. В исследовании приняли участие 20 детей с тХБП. Участники исследования были разделены на 2 группы: 1-я (n=10) – пациенты с диагнозом по МКБ-10 N18.0 «Терминальная стадия хронической болезни почек», проходящие заместительную почечную терапию в объеме гемо- или перитонеального диализа; 2-я (n=10) – пациенты с диагнозом по МКБ-10 T86.1 «Дисфункция трансплантата почки», находящиеся на иммуносупрессивной терапии. Детям были проведены онлайн-уроки гигиены полости рта. Для оценки эффективности гигиены полости рта был рассчитан индекс Грина–Вермиллиона (индекс гигиены полости рта упрощенный – ИГР-У) до и после обучения. Кроме того, для всех участников исследования был скорректирован набор средств индивидуальной гигиены полости рта, учитывая особенности основного заболевания и терапии.
Результаты. Все пациенты продемонстрировали статистически значимое снижение показателей ИГР-У после прохождения дистанционного обучения (до обучения – 2,29±0,37; после обучения – 1,32±0,35; t=9,720; df=19; p<0,001). Сравнение исследуемых групп показало, что после обучения ИГР-У статистически значимо уменьшился в 1-й группе почти в 2 раза (от 2,48±0,32 до 1,31±0,38; t=7,967; df=19; p<0,001), во 2-й группе – в 1,5 раза (от 2,09±0,31 до 1,33±0,35; t=7,343; df=19; p<0,001).
Заключение. Медицинская телекоммуникация между детскими стоматологами, нефрологами и пациентами с тХБП значительно улучшает качество индивидуальной гигиены полости рта в условиях стационара.
