Фоновые и предраковые заболевания вульвы и влагалища: современные представления, диагностика, лечение и реабилитация

Аннотация

Статья посвящена современным аспектам классификации, эпидемиологии, патогенеза, диагностики, лечения и реабилитации фоновых и предраковых заболеваний вульвы и влагалища. Особое внимание уделено стратификации интраэпителиальных неоплазий по ВПЧ-статусу, роли иммуногистохимических маркеров (p16, p53) и молекулярно-генетических характеристик в прогностической оценке. Представлены актуальные данные по клиническим проявлениям и диагностическим подходам, включая неинвазивные методы, рефлективную конфокальную микроскопию. Обобщены современные терапевтические стратегии с анализом эффективности имиквимода, фотодинамической терапии, CO2-лазера и хирургических вмешательств. Описаны подходы к мультидисциплинарной реабилитации, включая использование вагинальных расширителей, физиотерапии тазового дна, психосексуальной поддержки и телемедицинских технологий. Акцент сделан на необходимости междисциплинарного и персонализированного подхода к ведению данной категории пациенток с целью повышения качества жизни и профилактики малигнизации.
