Anomalous origin of the right coronary artery from the pulmonary trunk concurrent with aortopulmonary septal defect is an extremely rare abnormality. The paper describes a clinical case of this abnormality in a 2-month-old infant who underwent one-stage radical surgery for the defect: plasty of aortopulmonary septal defect with a xenopericardial flap; creation of an intrapulmonary tunnel with a xenopericardial flap (Takeuchi procedure); ligation of the patent ductus arteriosus; suture of the patent fenestra vestibuli under extracorporeal circulation and hypothermia.
