ISTORIYa I REZUL'TATY LEChENIYa ZhELChNO-KAMENNOY BOLEZNI V STATsIONARE KRATKOVREMENNOGO PREBYVANIYa

Abstract

An outpatient surgery center was set up at Krasnodar City Clinical Hospital Three in 1998, which introduced into practice all types of endoscopic operations for cholelithiasis, such as papillotomy, transpapillary interventions, and laparoscopic cholecystectomy (LCE). An algorithm and principles were developed for the treatment of cholelithiasis at a short-stay hospital (SSH). In 1998-2004, a total of 1361 patients were operated on and 1465 operations: 1305 laparoscopic cholecystectomies, 251 endoscopic retrograde cholangiopancreatographies, and 160 endoscopic papillotomies were performed. Failures of LCE were its duration of more than 2 hours (18%), conversion (0.3%), and complications (0.5%), among which there were damages to the hepaticocholedochus and two hollow organs. Transpapillary operations resulted in failures (2%) and complications (6%). Thus, there were complications (3%), conversions (0.6%), and no deaths after endosurgical treatment for cholelithiasis at the SSH. The mean bed/days were in the range of 1.7-4.5.

Abstract

The purpose of the study was to evaluate brain perfusion and cognitive function (CF) in patients with metabolic syndrome (MS) during combination antihypertensive therapy. Twenty-four patients with MS underwent brain single-photon emission computed tomography using 99mTc-hexamethylpropylenamine oxime (HMPAO) and their CF were determined before and 6 months after antihypertensive therapy with enalapril in combination of long-acting nifedipine or moxonidine. Brain tomoscintigraphy showed that before treatment the patients with MS exhibited both areas of lower accumulation of 99mTc-HMPAO (hypoperfusion) and those with higher accumulation of this indicator (hyperperfusion) than did the controls. Six-month antihypertensive therapy with enalapril in combination with long-acting nifedipine or moxonidine leads to optimization of brain blood supply and CF improvement in patients with MS.

Abstract

Twenty-two vermiform appendages removed during delayed appendectomy in patients with a 1-6-month history of appendicular infiltrate (AI) or periappenducular abscess (PAA) were morphologically studied. The study revealed the signs of chronic inflammation in all cases and the morphological signs of an exacerbation of chronic inflammatory process. Based on the examination of pathomorphological changes, the authors determined the magnitude of an inflammatory process in the vermiform appendage depending on recurrences and its removal and substantiated the necessity of appendectomy in patients with AI or PAA as the appendage is a source of a chronic inflammatory process. Moreover, the risk of development of a destructive form substantially increases particularly when foreign bodies are present in the wall thickness.

Abstract

The paper considers the problem of early diagnosis of retrocochlear demyelinating lesion of the auditory analyzer by complex audiological examination and gives criteria for measuring the parameters of acoustic reflexes (AR), which correspond to retrocochlear demyelinating lesion of the auditory analyzer. Sixty-six of 70 patients with the established diagnosis of multiple sclerosis were found to have pathological AR changes. The results of AR examination were comparable with those of short-latency auditory evoked potential studies, computed electronystagmography, and brain magnetic resonance imaging.

Abstract

Examination of lipid-protein relationships in the red blood cell membranes and placental homogenates of women with obesity and type 2 diabetes mellitus (T2DM) has indicated that activation of lipid peroxidation along with decreased antioxidant defense and imbalance in the placental system of thyroid hormones-thyreotropin. The above impairs the structural and functional properties of cellular membranes, increases lipid microviscosity, and changes the structure of membrane-binding proteins, which correlates with the parameters of glucose and insulin homeostasis and the level of lipid peroxidation. Impaired placental homeostatic processes are a ground to improve therapy aimed not only at compensating T2DM and reversing obesity, but also at eliminating placental insufficiency.

Abstract

Clinical, instrumental, and enzymological studies were carried out in 15 children with the acute course of intrauterine cytomegalovirus infection (CMVI) to estimate the extent and depth of cardiac disorders. A control group consisted of 15 noninfected children. The specific features of cardiac lesion and the level of cardiac enzymes were studied in children with CMVI. Irrespective of the severity of cardiac pathology, enzymemia was ascertained to reflect the depth of cardiomyocyte morphological changes. The remaining high levels of enzymes over time are indicative of the on-going cardiac morphological changes in the persistent course of the disease, which lead to the development of chronic cardiac diseases.

Abstract

The presence of Mycobacterium tuberculosis antibodies (Ab) is one of the most characteristic signs of tuberculosis. However, the informative value of serodiagnosis is less in children than in adults, in this connection the prognostic value of Ab was determined in children with pulmonary tuberculosis. Eighty-four children with new-onset active pulmonary tuberculosis were followed up. A control group included 25 apparently healthy children from risk groups for tuberculosis. The standard test system EIA-ANTI-TUB® was used to measure the level of Ab. The study was conducted over time: before and 2 and 5 months after initiation of therapy. The sensitivity of the technique was 85.3% and its specificity was 55.1%. The control group showed a direct correlation between the duration and extent of infection and the level of Ab (p < 0.01). The follow-up revealed a correlation between the form of tuberculosis, the severity of its process, and the level of Ab in the children with pulmonary tuberculosis (p < 0.05).

Abstract

Children's health formation substantially depends on the status of the lymphoepithelial pharyngeal ring, the components of which are constituents of the uniform immune system, by developing the body's immunological resistance. The study used a number of metabolic and immunological parameters to assess a risk for a decreased health status in 210 children (115 boys and 95 girls) aged 4 to 10 years with various conditions of the pharyngeal tonsil. The increased and high levels of a skin automicroflora (SAMF) test were noted in children at risk for the development of chronic adenoiditis, which is suggestive of a diminished systemic immunological responsiveness. The levels of calcium and magnesium in the expired air condensate and the SAMF test may be used as markers of a decreased health status in children with pharyngeal tonsil pathology and as an additional risk factor for development of chronic adenoiditis.

The paper shows the significance of a complex of macro-, microscopic, chemical, and biochemical studies in the solution of this problem when a forensic medical examination is made.
