При травме спинного мозга патологические сдвиги детально описаны в области первичного повреждения и прилегающих сегментах. Накапливаются данные о реакции ткани в отдалённых от эпицентра повреждения отделах спинного и даже головного мозга. Представление о пластичности и посттравматических реакциях в отдалённом от эпицентра травмы поясничном отделе, содержащем специфическую сеть интернейронов в виде центрального генератора паттернов, имеет особое значение для восстановления двигательной функции. Среди факторов, влияющих на пластичность нейронных связей и регенераторный потенциал в отдалённой от эпицентра повреждения области, практически только начинают систематически исследовать миелоидную инфильтрацию, активацию микроглии, нейровоспаление и молекулярную перестройку внеклеточного матрикса. При отдалённой от поясничного отдела травме в нём развиваются быстрые реакции, связанные с дезинтеграцией аксонов нисходящих трактов. Эти сдвиги сопровождаются реактивностью глии, элиминацией синапсов, возникновением дисбаланса между возбуждением и торможением и нарушением связей в нейронных сетях. Выраженность патологических реакций в отдалённом от эпицентра травмы отделе спинного мозга зависит от характера травмы, который определяет разную вовлечённость и степень деструкции нисходящих двигательных путей. В обзоре представлен анализ экспериментальных данных по реакциям и реорганизации нейронной сети в поясничном отделе спинного мозга при травме в проксимальных отделах. Для выявления возможных причин патологических сдвигов в отдалённом от эпицентра травмы отделе спинного мозга особый интерес представляют биомаркёры повреждения. Эти молекулы высвобождаются из гибнущих клеток в эпицентре травмы, появляются в спинномозговой жидкости и, выступая в роли ассоциированных с повреждением молекулярных паттернов и аларминов, могут оказывать нейротоксическое действие во всех отделах спинного мозга.
