Dystrophic diseases of the salivary glands that occur in the background steroid deficiency (hypogonadism)

Abstract

The authors conducted a comparative clinical and laboratory analysis of patients suffering from hypogonadism and sialadenosis before and after endocri-nological and dental treatment. The results obtained allowed us to conclude that sialadenosis, which occurred in patients with a deficiency of steroid hormones, had no special symptoms and it was clinically hidden for both patients and General practitioners. The most typical symptoms of sialadenosis were: an increase in LF, decrease in the level of secretion and the increase in the viscosity of mixed saliva. The results of complex treatment of patients with sialadenosis on the background of Hypo-nadism were the best than only mono-dental treatment. The obtained data indicate the need for complex dental and endocrinological treatment of patients with sialadenosis on the background of hypogonadism.

Surgical method for treating ductal sialadenitis of the large salivary glands

Abstract

The authors indicated that pronounced deformities of the ducts of the large salivary glands occur in 70% of patients with chronic sialadenitis. Due to the fact that conservative treatment of ductal sialadenitis, especially pronounced, does not give long-term results, the authors proposed various methods of surgical treatment of ductal sialadenitis, which have a low level of trauma of collateral tissues and provides a stable long-term result.

Description of the expression index index for patients with gnatic from of mesial occlusion

Abstract

An Index has been developed to assess the severity of dentofacial anomalies. The index values reflect the severity of maxillofacial abnormalities and is the rationale for choosing a method for treating patients with a gnatic form of mesial occlusion or for compensatory orthodontic treatment. The proposed technique is a convenient express method for the diagnosis and treatment planning of dentoalveolar anomalies.

Features of clinical manifestations in patients suffering from dental abnormality

Abstract

Anomalies of the dentition represent one of the most common groups of pathologies in dentistry. Many complaints made by patients with dentofacial anomalies (CCA) make this problem quite relevant. In this regard, of particular interest is a more detailed study of the features of clinical manifestations in patients suffering from RF.
Material and methods. The study involved 118 people with deep incisal overlap (hydraulic fracturing), the average age was 38.7 ± 8.5 years; women accounted for 54.24%; men-45.76%. Determining the dental status for studying the clinical manifestations of abnormalities included: studying the anamnesis, patient complaints, internal and external oral examinations. When analyzing the results of the study, biostatistical methods were used.
Results. An analysis of the data allows clustering of patients' complaints into the following groups: painful, functional and aesthetic. During a dental examination of patients with hydraulic fracturing, increased abrasion of the teeth, increased tone of the chewing muscles proper, and a change in the ratio of elements of the temporomandibular joint (TMJ) were visualized.
Conclusions. The variety of patient complaints with this pathology indicates the relevance of this problem. Most complaints of an aesthetic nature both among women and among men are a manifestation of dissatisfaction with the appearance.

Comparative analysis of speech therapy and orthodontic effects of orthodontic equipment used in the replacement bite to expand the upper dentition

Abstract

The period of the first phase of replacement bite in children in the Russian Federation corresponds to the period of primary school age, which is considered one of the critical periods in the child's development, due to increased emotional loads, and children aged 6—7 years are at risk for developing combined orthodontic and speech therapy pathologies.
The complexity of the correction of orthodontic treatment at this age patients associated with lack of coordination between the orthodontist and speech therapist: a speech therapist does not accept the correction of the patient, until he had reached the incisors contact, and the orthodontist begins orthodontic treatment, since it prevents the wrong position and function of tounge.
Most modern removable orthodontic appliance restrict the mobility of the tongue to some extent, thus forcing the child to move it down and back, suppressing its tone. In addition, the use of these devices prevents speech therapy correction, since it violates the automation of sound reproduction. The use of removable orthodontic appliance has positive orthodontic effects, but at the same time, contributes to the development of the second and third level of incorrect sound reproduction.
In our experience, the device of choice in such situations is the Haas appliance. The use of the Haas appliance gives the fastest orthodontic effects, and myofuction therapy, speech therapy, as well as ottorhinolaryngological treatment can be carried out at the same time, which helps to consolidate the result and reduces the likelihood of relapses to a minimum.

Assessment of mediator regulation of inflammatory processes in children with congenital malformations of maxillo-facial area

Abstract

In the article results of mediator regulation of inflammatory processes in patients after one-sided (15 children) and reconstructive (15 children) rinocheiloplasty are presented. It is determined that in patients after reconstructive rinocheiloplasty at 5—7th day post operation antioxidant capacity of blood plasma is below normal values, that points on lack of compensatory increasing of anti-radical ferments. Specified deficit is necessary to be considered in conservative treatment by applying antioxidants.

Abstract

The article presents the results of the analysis of the success of treatment and survival of dental implants in the treatment of patients with bone deficiency in the framework of multi-stage and single-stage approaches for bone grafting operations using standard-sized dental implants and in the framework of the approach using narrow/short dental implants without bone augmentation surgery. Based on the results obtained, the degree of expediency (p<0.05) of choosing an approach to treatment can be selected depending on the type, location of the defect and the number of missing teeth.

Dental screening questionnaire for identification of the risk of oncological pathology development (on the example of the moscow region)

Abstract

In order to identify the risk of oncological pathology in patients of dental clinics, a questionnaire has been developed that allows for a number of targeted questions to identify a high or very high risk of cancer in the patient. These questions reflect the presence (including in the past) of oncological diseases among respondents and their relatives, the presence of diseases and foci of trauma to the oral mucosa, smoking and systematic use of alcohol, the carriage of herpes virus, human papilloma or fungal flora, work in harmful working conditions and climate. The frequency of positive responses increases with age (with the exception of smoking and drinking) and is less common in women. Interviewed 574 people in 6 dental clinics in the Moscow region.
According to a screening questionnaire at a dental appointment, a high and very high risk of developing cancer pathology according to WHO criteria applies to 44.1% and 36.4% of the population of the Moscow region, respectively. At the age of 40 years, the high risk does not exceed 21.4%, in people 40-60 years old it increases to 88.0% and decreases to 11.7% after 60 years; at the same time, a very high risk increases from 5.8% in people under 40 to 12.0% at the age of 40-60 years and to 88.3% after 60 years. Up to 40 years of age, cancer risk is 2 times more likely to be detected in men, then evening out in men and women.

Analysis of personnel resources of doctors according to «dentistry» profile in units providing medical care in outpatient and inpatient conditions

Abstract

In the health-care system in the Russian Federation, the units providing medical care in outpatient and inpatient conditions employ more than 55,000 individuals of dental doctors of various specialties, dental doctors and dental hygienists every year from 2014 to 2018. The availability of dentistry by doctors tends to decline from 4.20 (2014) to 3.75 (2018). By 2018, 99.45% of dentistry doctors and 99.3% of dental doctors have a specialist certificate and accreditation certificate. 38.16% of dentistry doctors and 54.88% of dental doctors confirmed their qualification category. In dynamics there is a tendency to decrease the passing of certification for obtaining qualification category by these specialists. These facts show a decrease in the availability and quality of dental care, as it is ensured by the availability of the necessary number of health workers and their level of qualification.
