НЕПОСРЕДСТВЕННЫЕ И ОТДАЛЕННЫЕ РЕЗУЛЬТАТЫ НЕФРЭКТОМИИ С ТРОМБЭКТОМИЕЙ ИЗ НИЖНЕЙ ПОЛОЙ ВЕНЫ ПРИ ПОЧЕЧНО-КЛЕТОЧНОМ РАКЕ

Аннотация

Проанализированы результаты лечения 32 больных почечно-клеточным раком, осложненным опухолевым тромбозом нижней полой вены, после нефрэктомии с тромбэктомией. В исследование были включены только больные с II—IV уровнями тромба НПВ: 26 человек с Т3b — протяженность тромба 5,1±1,75 см; 6 пациентов с Т3с — протяженность тромба 14,8±0,98 см. В ближайшем послеоперационном периоде умер 1 (3,1%) пациент от тромбоэмболии легочной артерии. Осложнения I—II степеней по Клавиен—Диндо были у 11 человек, III—IV — у 2 (6,2%). У 90,4% больных были метастазы в отдаленные органы и (или) регионарные лимфоузлы, что негативно отразилось на выживаемости больных: 5-летняя выживаемость составила 36%. Факторами, влияющими на выживаемость, оказались наличие лимфогенных метастазов, критерии прогноза болезни MSKCC. Зависимости выживаемости больных от степени распространения опухолевого тромба НПВ (стадии Т3b и Т3c) выявлено не было. Результаты послеоперационной системной таргетной терапии оказались статистически достоверно лучше, чем результаты иммунотерапии.
