About appropriateness of macrolides use for the treatment of patients with comorbide infectious inflammatory diseases of upper and lower segments of respiratory system

Abstract

The functional linkage between the nasal cavity and paranasal sinuses and other organs and systems, primarily with bronchopulmonary system, is emphasized. The number of respiratory diseases, particularly chronic rhinosinusitis, increases in proportion to the number of lower respiratory tract diseases. Streptococcus pneumoniae, Haemophilus influenzae, less frequently Moraxella catarrhalis and atypical flora play a major role in the etiology of bacterial exacerbation of rhinosinusitis and chronic obstructive pulmonary disease. The beta-lactams (aminopenicillins, cephalosporins), macrolides-azalides and modern fluoroquinolones with antipneumococcal activity are promising drugs for the treatment of respiratory infections. Azalides (azithromycin) from the group of macrolides have highest spectrum of activity in regard to pathogens.
