Механизмы боли при хроническом панкреатите: что и кому назначить практикующему врачу?

Аннотация

Хронический панкреатит (ХП) - распространенное заболевание с потенциально тяжелым течением и высоким риском развития серьезных осложнений: панкреатогенного сахарного диабета, трофологической недостаточности, рака поджелудочной железы. Только своевременная диагностика и адекватное лечение больных ХП позволяют надеяться на уменьшение риска развития тяжелых осложнений и исходов. Болевой абдоминальный синдром ХП является ранним и наиболее ярким. Правильная интерпретация жалоб больных ХП, целенаправленное обследование и лечение лежат в основе улучшения качества и увеличения продолжительности их жизни. В подавляющем большинстве случаев для купирования панкреатической боли в отсутствие показаний к эндоскопическому и хирургическому лечению необходимо использовать комбинированную фармакотерапию, включающую ненаркотические анальгетики, миотропные спазмолитики (гиосцина бутилбромид), препараты панкреатина с высоким содержанием протеаз, блокаторы желудочной секреции.
