Аннотация
Лекарственно-индуцированное увеличение массы тела (МТ) является серьезным побочным эффектом многих рутинно используемых препаратов, приводящим к несоблюдению правильного режима терапии и обострению сопутствующих хронических заболеваний, связанных с ожирением и повышенной МТ. Эта нежелательная лекарственная реакция влияет на повышение артериального давления, нормальную работу опорно-двигательного аппарата, нарушение липидного профиля, а также ухудшение контроля уровня глюкозы. Применение таких сахароснижающих препаратов, как инсулин, препараты сульфонилмочевины (гликлазид, глимепирид, глибенкламид) и тиазолидиндионы (пиоглитазон, росиглитазон), обычно сопровождается увеличением МТ. Прибавка МТ может быть уменьшена или предотвращена путем соблюдения низкокалорийной диеты и регулярных физических упражнений или комбинированной терапии с метформином. Увеличение МТ также распространено при применении психотропной терапии. Известно, что как атипичные нейролептики (клозапин, оланзепин, рисперидон, кветиапин и др.), так и классические (галоперидол, хлорпромазин, тиоридазин) вызывают заметное увеличение МТ. Многие антидепрессанты, такие как амитриптилин, миртазапин и некоторые селективные ингибиторы обратного захвата серотонина и ингибиторы моноаминоксидазы, также могут способствовать значительному увеличению МТ, что не может быть объяснено только усилением симптомов депрессии. Противоэпилептические препараты, способствующие увеличению массы тела, включают вальпроат, карбамазепин и габапентин. Аналогичный побочный эффект наблюдается при терапии гормональными, антигипертензивными, химиотерапевтическими лекарственными средствами. Поэтому при назначении пациенту терапии данными лекарственными препаратами необходимо помнить о побочном эффекте в виде увеличения МТ. Особенно это важно для пациентов с изначально высоким индексом МТ.
