IRRITABLE BOWEL SYNDROME: EPIDEMIOLOGY, ETIOPATHOGENESIS, DIAGNOSIS AND THERAPEUTIC CORRECTION

Abstract

The article is dedicated to the one of the most common functional bowel disease - Irritable bowel syndrome (IBS). The current definition of IBS, epidemiological data, and key issues of the pathogenesis are presented. Criteria for diagnosis of IBS according Consensus Rome III are particularized. The main methods of diagnosis and principles of treatment of this pathology are discussed. Particular attention is paid to the role and importance of correction of microbiocenosis in the treatment of IBS, in particular, probiotic therapy with the drug Hilak Forte. The preventive measures and prognosis are described.

IDIOPATHIC SLOW-TRANSIT CONSTIPATION: MECHANISMS OF DEVELOPMENT AND TREATMENT POTENTIALS

Abstract

The article is dedicated to the features of idiopathic slow-transit constipation (ISTC) of unknown etiology (Russian equivalent - chronic colonic stasis), sporadic disease, which differs from irritable bowel syndrome, symptomatic, proctogenic and other forms of constipation. ISTC occurs mainly in women (70-80%), this pathology develops in early childhood or adolescence, long life persist, and is very refractory to medical treatment. The article discusses the pathophysiology of ISTC, in which the imbalance of intestinal regulatory peptides and neurotransmitters, including opioids (enkephalines and endorphins) plays important role. It is emphasized that trimebutin (Trimedat) is the only really available drug that stimulates the motor activity of the colon, and this drug is full agonist for all three types of opioid receptors. Available clinical data show positive therapeutic effects of trimebutine on the various functions of the gastrointestinal tract, including stimulation of activity of colon smooth muscles in patients with ISTC .

FEATURES OF CLINICAL COURSE AND THERAPEUTIC TACTICS IN ELDERLY PATIENTS WITH CHRONIC HEPATITIS C

Abstract

In everyday clinical practice, elderly patients with chronic hepatitis C (CHC) are very rarely considered as candidates for antiviral treatment. Such therapeutic tactics is not always justified. The present article discusses the features of the course of CHC in the elderly patients, the efficacy and safety of antiviral therapy (AVT) in patients of this age group. It is noted that in persons aged 65 years and older HCV infection is more severe than in younger patients, with more rapid progression of fibrosis and low levels of ALT. As a consequence, cirrhosis occurs more frequently in elderly patients than in younger. There is evidence that old age is an independent risk factor for the formation of hepatocellular carcinoma. The issue of advisability of AVT in elderly patients needs to be addressed individually; the use of methods alternative to liver biopsy for definition of stage of disease and degree of fibrosis is preferable. One of the main problems of AVT is the development of ribavirin-induced anemia, which is especially dangerous in persons with cardiovascular diseases. It is emphasized that old age is not an absolute contraindication for the AVT for CHC, but possible benefits/harm ratio should be clearly determined in such patients.

NONALCOHOLIC FATTY LIVER DISEASE AND CARDIOVASCULAR RISK: INFLUENCE OF FEMALE SEX

Abstract

The article considers the non-alcoholic fatty liver disease (NAFLD), one of the components of metabolic syndrome according to modern concepts. It is emphasized that NAFLD is associated with cardiovascular diseases (CVD) and apparently predicts their development. The article discusses the influence of female sex on the incidence of NAFLD; it is noted that risk of cardiovascular diseases increases dramatically against the background of menopause or gynecological pathology. Essential phospholipids (EFL) are effective means for the treatment of NAFLD. Preparations of EFL (Essentiale forte, Essentiale N) competitively replace endogenous phospholipid phosphatidylcholine of cell membranes and restore damaged membrane structures of cells through the incorporation of EFL molecules in membrane and filling gaps in membrane, stimulate the synthesis of endogenous phospholipids. In the treatment of patients with NAFLD and CVD, the benefits of use of EFL are improvement of receptor function, including insulin receptors, and normalization of lipid metabolism.

The article is concerned with the role of gastrointestinal microflora in the maintenance of normal function of intestinal tract against the background of digestive diseases. The problems, etiology, clinical presentations, diagnosis, and treatment of small bowel bacterial overgrowth syndrome (SBBOS) are discussed. Based on their own researches, authors in detail reviews current indirect methods of diagnosis, and tests based on examination of metabolites of microflora, in particular the hydrogen breath test with lactulose. Detailed description of the mechanism of action, indications and conditions of practical application of lactulose (Duphalac) for the correction of SBBOS, chronic constipation, hepatic encephalopathy and other pathological conditions are presented.

Abstract

Autoimmune hepatitis (AIG) is a relatively rare liver disease that affects primarily women of any age; it accounts 10 to 20% of the total spectrum of chronic hepatitis. The article presents modern point-based system to identify signs of AIG; clinical presentation of AIG is considered. Other variants of autoimmune liver disease are primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC). The overlap syndromes between different autoimmune liver diseases AIG-PBC and AIG-PSC are discussed. Modern approaches to the treatment of AIG and overlap syndromes with glucocorticoids, azathioprine, and ursodeoxycholic acid are presented.

Abstract

The problem of modern treatment of chronic pancreatitis is highly relevant. This study was aimed to examination of impact of miotropic spasmolytic Odeston (Hymecromone) on exocrine pancreatic function in patients with chronic pancreatitis. During the study, therapeutic efficacy of Odeston was observed, in particular pain and diarrhea syndromes were decreased, the flatulence phenomenon disappeared. Odeston had a selective spasmolytic effect on the Oddi's sphincter and Lutkens' sphincter, increased the enterohepatic recirculation of bile acids, jugulated maldigestion syndrome and contributed to the regression of duodenum dyskinesia. The drug was well tolerated; complications and side effects were not observed.

Abstract

The study that included 73 patients with gastroesophageal reflux disease was aimed to evaluation of comparative effectiveness of prokinetic monotherapy with Itopride (Ganaton), proton pump inhibitors monotherapy (PPI, Omeprazole), and combination therapy in the treatment of GERD 0-II stages (according to the Savary-Miller classification). The dynamics of clinical symptoms of GERD on the background of different regimens was assessed, and the duration of period of esophageal acidification to pH < 4 with assessing the effects of different treatment regimens on this indicator was determined. It was shown that Ganaton as monotherapy is effective for control of clinical symptoms of heartburn and belching and reduction of period of acid reflux in esophagus. According to the influence on these characteristics of GERD, Ganaton is equal to monotherapy with Omeprazole at dose of 40 mg per day. Combined therapy with Omeprazole and Ganaton is more effective for the reduction of period of esophageal acidification as compared to any type of monotherapy, but no different from monotherapy in terms of control of heartburn and belching.

Abstract

The study was aimed to assess the effectiveness of Metadoxile (metadoxine) in the complex treatment of gastropathies against the background of alcoholic liver disease. The study enrolled 137 patients with stage II of alcoholism verified by narcologist, including 72 patients with alcoholic steatohepatitis, 65 patients with alcoholic cirrhosis, and 68 nondrinkers with chronic digestive diseases. All patients with erosive-ulcerous defects received combined treatment with Metadoxile. It was found that Metadoxile as a part of complex treatment of erosive-ulcerous defects contributes to early epithelialization, and has a positive impact on the local endocrine homeostasis in patients with alcoholic liver disease.

EXPERIENCE OF USE OF DRUG DIBICOR IN THE COMPLEX THERAPY OF NON-ALCOHOLIC FATTY LIVER DISEASE

Abstract

Effectiveness of use of taurine (Dibicor) in the treatment of non-alcoholic fatty liver disease (NAFLD) in the stage of steatohepatitis (NASH) was estimated. In the MCH № 7, Novosibirsk, 36 patients of both sexes with NASH with minimal biochemical activity received comprehensive treatment, 20 of them received the drug Dibicor. All patients underwent anthropometric and laboratory physical examination before the study and after 1 month of treatment. Statistical analysis of the results was performed using SPSS 13.0 software package with the definition of levels of statistical significance. Value of P<0.05 was considered statistically significant. Improvement of general condition was observed in patients of both groups. Side effects against the background of the treatment with drug Dibicor were not revealed. In the group treated with Dibicor, improved lipid profile, decreased levels of free bilirubin, fibrinogen, alkaline phosphatase, γ- glutamyltranspeptidase, glycemia, decreased body mass index and diastolic blood pressure were observed. It is concluded that Dibicor (taurine) is a physiological and safety drug with hepatoprotective and hypolipidemic effects in patients with NASH associated with obesity, and may be included in the treatment regimen of NASH.

Abstract

The article presents results of open randomized prospective comparative pharmacoeconomic study in parallel groups, which compared results of treatment of patients with chronic pancreatitis with exocrine pancreatic insufficiency by two various preparations of pancreatine - enteric-coated pellets and microtablets in equivalent doses of lipase (not less than 100,000 units of lipase per day). More modern microtablet drug Ermytal showed clear clinical benefits. Despite the higher cost of microtablet pancreatine, costs for pain relief and jugulation of manifestation of exocrine pancreatic insufficiency were lower by 339 and 463 rubles per patient, respectively. The use of microtablet pancreatine reduces the cost of illness by 1.3 times compared with the use of tablet pancreatine, significantly increasing the quality of life of patients. Therefore, the use of microtablet polyenzyme drugs despite their greater cost is more profitable.

Abstract

The article presents the results of evaluation of effect of myotropic spasmolytic (selective blocker of calcium channels in smooth muscle of the gastrointestinal tract) pinaverium bromide (Dicetel) on the intensity of abdominal pain and dyspeptic symptoms in 46 patients with irritable bowel syndrome (IBS). Dicetel can be successfully used for the treatment of abdominal pain in IBS patients. The inclusion of pinaverium bromide in the general complex of therapeutic interventions is most appropriate; in some cases, specifically in moderate pain syndrome, drug is effective as monotherapy.

Abstract

The development of inflammatory bowel disease (IBD): Crohn's disease (CD) and ulcerative colitis (UC) are due to violation of the interaction of the intestinal microbiota and the immune system of the intestine. Taking account of favorable effect of probiotics on the protective intestinal barrier and formation of immunological tolerance, they can be used in the treatment of IBD. Only a few studies were aimed to this problem in children. The purpose of this study was evaluation of effectiveness of Saccharomyces boulardii (Enterol) in remission maintenance in 24 children with BC, who were randomized into 2 groups of 12 persons after remission induction. In the first group, Mesalazine at a dose of 50 mg/kg was used as maintenance therapy and in the second group in the first 3 months Enterol at a dose of 500 mg/day of treatment was used in addition to Mesalazine. The dynamics of clinical picture and calprotectin level in feces were assessed before treatment and after 6 months. Exacerbation occurred in 8.3% of children receiving Enterol, and in 25% - not receiving Enterol. Calprotectin level in feces was significantly decreased in the group receiving Enterol.

Abstract

The article presents a case of ulcerative colitis complicated by generalized cytomegalovirus (CMV) infection in woman 48 years old. Infection has developed due to immunosuppressive therapy against the background of disease-associated immunodeficiency. Diagnosis was made just posthumously: morphologic study of autopsy revealed a typical owl's eye cells in the tissues of rectum, lung, brain, thyroid gland. Polymerase chain reaction confirmed the presence of CMV DNA in tissues of the rectum, lung, and brain.

MALIGNANT TUMORS OF THE SMALL INTESTINE: CLINICAL OBSERVATION

Abstract

The article presents classification of malignant tumors of the small intestine (MTSI) - rare tumors of the gastrointestinal tract. The approaches to diagnosis of MTSI, especially difficult at early stages of the disease, are discussed. The clinical observation of 55 years old patient with complains of nausea, abdominal pain for a long period, and signs of progressive anemia is described. In-depth examination of the patient has revealed moderately differentiated adenocarcinoma of the small intestine, T4N1M1. The enterectomy was performed. During operation, metastases in the mesenteric lymph nodes and left lobe of the liver, and ascites were detected. The patient was referred to the oncologist to decide the issue of advisability of radiation therapy or chemotherapy. It is emphasized that diagnosis was suspected in routine X-ray examination at hospital stage, and this examination could be performed in outpatient settings. The "unexplained" iron-deficiency anemia requires differential exclusion of any disease of the small intestine, even in absence of malabsorption syndrome.
