INTEGRATED APPROACH TO THE TREATMENT OF CHRONIC LARYNGITIS

Abstract

Chronic laryngitis is multifactorial disease. At the moment, there is no common view on the treatment of this disease among otolaryngologists. The use of antibacterial agents in case of protracted course of disease or frequent relapses is not in doubt. In recent years, proton pump inhibitors and antacids are increasingly used for the treatment, and it should be considered that the ineffectiveness of these drugs can not eliminate the influence of the gastrointestinal pathology on the course of the disease. Case report presented in the article indicates the need to introduce 24 hour pH-metry in the practice of examination of patients with chronic laryngitis for the differential diagnosis of gastroesophageal reflux disease and correction of therapy.

Abstract

Against the background of the steady increase in the incidence of acute upper respiratory tract pathology, acceleration of transition of acute inflammation to chronic inflammation is observed, and therefore timely diagnosis and adequate treatment are particularly important. There are not a lot of drugs, which are effective and safe in the treatment of inflammatory diseases of the upper respiratory tract in children and adults in terms of evidence-based medicine. Recent studies show that the silver particles are harmful to a wide spectrum of Gram-positive and Gram-negative bacteria. In addition, there are reports of antifungal, antiviral and anti-inflammatory activity of silver. The disadvantages of solution silver proteinate. used since 1964 includes its inaccessibility because of the limited edition and small period of validity (30 days). New standardized form of solution silver proteinate for self-compounding, easy to use and has a longer period of validity than its existing analouges, and can be recommended for use in ENT practice.

LEVOFLOXACIN: BENEFIT-RISK RATIO

Abstract

The article discusses pharmacological properties and safety of the use of levofloxacin-fluoroquinolone, whose consumption in the world is constantly increasing. The reason for the increase in the use of levofloxacin is its more favorable benefit-risk ratio compared to many other fluoroquinolones, as evidenced by the results of not only clinical, but also widely conducted pharmacoepidemiological studies. The risk of developing serious adverse reactions with levofloxacin is generally lower than with other fluoroquinolones, and it can be significantly reduced by the rational use of the drug, which implies taking into account risk factors for the development of certain adverse reactions in a particular patient. To reduce the cost of treatment with levofloxacin, it is advisable to use its effective generics.

TRANSETHMOIDAL ORBITAL DECOMPRESSION ORBITS FOR A RARE FORM OF OPTIC NEUROPATHY: CASE REPORT

Abstract

Endocrine ophthalmopathy is a complex pathology in endocrinology and ophthalmology. This is multidisciplinary problem of endocrinology and ophthalmology. In October 2009, in Amsterdam, international experts on Graves’ ophthalmopathy adopted a document entitled «The Amsterdam Declaration on the need to improve health care delivery to patients with Graves’ ophthalmopathy». Currently, the treatment of endocrine ophthalmopathy includes not many surgical approaches which differ in their effectiveness and safety. One of the most popular methods is transethmoidal endoscopic approach with the removal of the medial and bottom walls of the orbit. In addition, it can be said unequivocally that this method slows down the progression of the disease, thereby improving visual acuity and quality of life. The article describes own authors’ experience of surgical treatment of patient with a diagnosis of «I degree diffuse toxic goiter, moderate thyrotoxicosis, medically induced subcompensation. Endocrine ophthalmopathy OU. Toxic rapidly progressive leftsided exophthalmos, left-sided keratopathy. Left-sided optic neuropathy. Deflected nose septum». After repeated ineffective courses of conservative treatment, transethmoidal orbital decompression was carried out. In the postoperative period, there were no significant oculomotor disorders and strabismus. As a result of the treatment, a significant reduction in the degree of exophthalmos was observed already at the first day after surgery.

Abstract

The article presents the history of the issue of creating a single information space in the sphere of drug circulation in the Russian Federation. The stage of attempts to implement the Unified Pharmaceutical Information Network in separate regions is described. The article provides information on the current state of the issue and the implementation of the Unified State Information System in the field of public healthcare, the Federal State Information System for monitoring the movement of medicinal products from the manufacturer to the consumer using marking, the Information and Analytical Monitoring and Control System in the Sphere of Procurement of Medicines for State and Municipal Needs. The role of the State Register of Medicines for the formation of a single information space in the sphere of drug circulation as a mean of implementing the concept of the introduction of CALS/PLM technology is described.
