Diseases of the esophagus in the practice of allergologist

Abstract

In the practice of an allergist, there are three most common variants of esophagitis - candidal esophagitis (complication of glucocorticosteroid therapy), eosinophilic esophagitis and reflux esophagitis (confounding factor for bronchial asthma). Visually, with endoscopy, these esophagites look similar and can be combined. The result of a morphological study of the biopsy is critical for the correct diagnosis. Candidiasis is characterized by the identification of the so-called pseudomycelium - the filiform form of yeast-like Candida fungus, eosinophilic esophagitis - by a certain eosinophil infiltration density, and reflux esophagitis - by the signs of nonspecific esophagitis. Esophageal candidiasis is usually treated with fluconazole for 2-3 weeks, reflux esophagitis - with proton pump inhibitors, and eosinophilic esophagitis - with glucocorticosteroids for a long time.
