Взаимодействие алкоголя и лекарственных средств: фокус на препараты, применяемые для лечения заболеваний сердечно-сосудистой системы, желудочно-кишечного тракта, противомикробные препараты

Аннотация
По данным национального исследования потребления лекарственных средств (ЛС) и здоровья населения, проведенного в 2019 г. в США, 85,6% людей в возрасте 18 лет и старше сообщили, что употребляли алкоголь в какой-то момент своей жизни, 169,5% лиц сообщили, что употребляли алкоголь в прошлом году, и 54,9% - в прошлом месяце. Также в США в 2015-2016 гг. 45,8% населения употребляли как минимум одно рецептурное лекарственное средство в течение последних 30 дней, большинство из которых относились к группе препаратов для лечения сердечно-сосудистых заболеваний, и антибиотикам. Потенциально данные ЛС могут вступать в фармакодинамические или фармакокинетические взаимодействия. Например, возможно подавление активности алкогольдегидрогеназы в желудке посредством ее неконкурентного ингибирования ацетилсалициловой кислотой, что может приводить к увеличению Cmax и средней AUC этанола, а значит, повысить его токсичность. Другим примером взаимодействия является снижение концентрации амиодарона в сыворотке крови и уменьшение его эффективности на фоне потребления больших доз или длительного употребления этанола. Алкоголь может влиять и на эффективность антибактериальной терапии. Так, этанол может изменять скорость, но не степень абсорбции амоксициллина; снижать Cmax и AUC эритромицина; повышать биодоступность тетрациклина. Таким образом, мы видим, что риски развития осложнений или недостаточной эффективности крайне высоки у пациентов, принимающих одновременно алкоголь и препараты для лечения заболеваний сердечно-сосудистой системы, желудочно-кишечного тракта или инфекционных заболеваний. Для снижения рисков развития осложнений потенциальные взаимодействия алкоголя и лекарственных средств следует обязательно принимать во внимание при подборе лекарственной терапии, но все же более эффективной стратегией будет исключение совместного применения алкоголя и ЛС.
