Систематический обзор и мета-анализ: предикторы развития псориатического артрита

Аннотация
Обоснование. Псориаз (Пс) - хроническое системное иммуновоспалительное заболевание, которое поражает не только кожу, но и опорно-двигательный аппарат. По данным эпидемиологических и клинических исследований, у каждого четвертого пациента с Пс развивается псориатический артрит (ПсА). Ранняя диагностика и раннее начало терапии помогут предотвратить развитие тяжелых форм и инвалидизацию. Выявление ранних предикторов ПсА и направление таких пациентов на ревматологическое обследование могут помочь решить данную проблему. Методы. Проведен поиск существующей литературы в базах данных PubMed (MEDLINE), EMBASE и Google Scholar за 10 лет -до 20.02.2021. Поиск текущих зарегистрированных исследований проведен в реестрах клинических исследований США (Clinicaltrials.gov), Китая (Chinese Clinical Trial Registry) и на международной платформе регистрации клинических исследований ВОЗ (WHO International clinical trial registry platform). Статистический анализ подготовлен в соответствии с международными рекомендациями для систематических обзоров и мета-анализа (PRISMA) при помощи программы Statistic SPSS 26.0 (Производство США). Мета-анализ подготовлен с использованием программы RevMan 5. Результаты. Нами идентифицировано 342 статьи, опубликованные до 20 февраля 2021 г. После дальнейшего анализа были исключены нерелевантные статьи. Всего в обзор и анализ включено 35 статей. К предикторам ПсА относят энтезиты, поражения ногтевого аппарата, увеит, а также аксиальные поражения. Кроме того, существуют и биомаркеры ПсА. При проведении метаанализа отмечено статистически значимое возрастание рисков развития ПсА в 1,52 раза (95% ДИ 1,22-1,88) при наличии поражения ногтевого аппарата. Кроме того, со статистической значимостью возрастали не только риски развития ПсА, но и шансы его развития в 2,76раза (95% ДИ: 1,94-3,92). Выводы. Пс и ПсА - многофакторные заболевания, при которых резко снижается качество жизни пациентов, возможно развитие тяжелых форм и инвалидизация. Поэтому очень важна ранняя диагностика таких состояний. На дерматологическом приеме в качестве предикторов ПсА можно рассматривать поражение ногтей, поэтому дерматологам очень важно оценивать не только гладкую кожу. Необходимо проведение дальнейших исследований для более глубокого понимания данной проблемы.
