CLINICAL SIGNIFICANCE OF DETERMINATION OR URINARY CONCENTRATION OF ENDOTHELIAL DYSFUNCTION AND FIBROGENESIS MARKERS IN HYPERTENSIVE NEPHROPATHY

Аннотация
Материал и методы. В исследование включен 71 больной эссенциальной артериальной гипертензией (ЭАГ) I-III степеней по классификации Российского медицинского общества по артериальной гипертонии в возрасте от 18 до 67 лет (в среднем - 40,9 ± 14,9 года). Наряду с общеклиническим обследованием всем больным была определена концентрация в моче трансформирующего фактора роста-β1 (TGF-β1), сосудисто-эндотелиального фактора роста (VEGF), ингибитора активатора плазминогена типа 1 (PAI-1) и коллагена IV типа.
Результаты. При сопоставлении с нормотензивными представителями контрольной группы у больных ЭАГ наблюдалось достоверное повышение мочевой экскреции TGF-β1, VEGF и PAI-1. Концентрация данных медиаторов, а также коллагена IV типа в моче оказалась достоверно выше и при наличии микроальбуминурии.
Заключение. Повышение концентрации в моче TGF-β1, VEGF и PAI-1 может рассматриваться как ранний признак гипертонического поражения почек; рост мочевой экскреции коллагена IV типа ассоциирован с микроальбуминурией и свидетельствует в пользу активации процессов почечного фиброгенеза
