РАСПРОСТРАНЕННОСТЬ ДИАБЕТИЧЕСКОЙ НЕФРОПАТИИ И ХРОНИЧЕСКОЙ БОЛЕЗНИ ПОЧЕК ПРИ САХАРНОМ ДИАБЕТЕ В РОССИЙСКОЙ ФЕДЕРАЦИИ

Аннотация
Материалы и методы: В 20 регионах РФ было обследовано 7174 больных СД 1 и 2 типа. Лабораторное обследование включало определение уровня HbA1c, креатинина крови, мочевины, ХЛ, экскреции альбумина в разовой порции мочи, измерение АД, осмотр глазного дна. Концентрация альбумина в разовой порции мочи от 20 мг/л до 200 мг/л расценивалась как МАУ, выше 200 мг/л - ПУ. СКФ вычисляли по формуле Кокрофта-Голта. Статистический анализ результатов исследования выполняли с применением программ STATISTICA 6. Данные представлены в виде медианы и значений 25-го и 75-го процентилей (Ме [25%;75%]). Статистически значимым для всех показателей считался критерий достоверности р < 0,05.
Результаты: Фактическая распространенность ДН составила 40,1% (СД 1) и 39,3% (СД 2), регистрируемая - 28,8% и 8,7% соответственно. Патологическая экскреция альбумина с мочой была установлена у 42% больных СД 1 и 46,2% больных СД 2. Распространенность этого показателя возрастала с увеличением возраста больных и длительности СД, а также при неудовлетворительном контроле углеводного обмена и артериального давления. Распространенность ХБП 2-3 стадий была достоверно выше при СД 2 типа, чем при СД 1 типа. 15% (при СД 1 типа) и 41,2% (при СД 2 типа) больных с МАУ и СКФ < 60 мл/мин/1,73 м² требуется дополнительное обследование с целью выявления недиабетических причин ХБП при СД.
Заключение: Проведение ежегодного скрининга позволяет выявить осложнения СД на более ранних стадиях. Компенсация углеводного обмена и артериального давления у больных СД замедляет прогрессирования ДН.
