Роль вируса папилломы человека возможно высокого канцерогенного риска в развитии злокачественной патологии шейки матки: систематический обзор и метаанализ

Аннотация
Цель исследования. Изучение распространенности вируса папилломы человека (ВПЧ) возможно высокого канцерогенного риска (возможно ВКР) и его вклада в развитие злокачественных новообразований шейки матки с оценкой достоверности получаемых данных в зависимости от используемых диагностических подходов.
Материалы и методы. Поиск выполняли в базах данных MEDLINE (PubMed), «КиберЛенинка», eLibrary (глубина поиска – 21 год). Всего проанализировано более 400 статей на русском, английском, немецком языках. Статистическая обработка результатов и визуализация данных выполнены в среде R.
Результаты. Установлено, что доля ВПЧ возможно ВКР от всех ВПЧ-положительных результатов среди условно-здоровых женщин (n = 20 989) составляет 14% (95% ДИ 9–20), в группе с цитологическим заключением LSIL и ASCUS (n = 2785) – 20% (95% ДИ 13–29). Среди пациенток с цитологическим заключением HSIL и «рак шейки матки» (n = 6316) частота выявления ВПЧ вероятно/возможно ВКР 26, 53, 66, 67 68, 70, 73, 82 типов составляет суммарно 6% (95% ДИ 3–16). При одновременном использовании разных диагностических подходов, позволяющих исключить ложноотрицательные результаты, число ВПЧ-отрицательных случаев рака шейки матки составляет 1% (95% ДИ 0–2). Сопоставимый уровень типоспецифической трансформирующей активности ВПЧ ВКР и возможно ВКР указывает, что ВПЧ возможно ВКР может быть единственным прогностическим фактором развития рака шейки матки.
Заключение. Полученные данные свидетельствуют о необходимости рекомендовать включение ВПЧ возможно ВКР в эпидемиологический мониторинг и скрининговые программы злокачественной патологии шейки матки.
