CLINICAL AND LABORATORY CHARACTERISTICS OF PATIENTS WITH HBEAG-NEGATIVE CHRONIC HEPATITIS B WITH DIFFERENT LEVELS OF VIRAL LOAD IN THE SERUM
Abstract
Our study demonstrates the relationship between of viral load with clinical, biochemical and immunological (IL-2, INF-α, INF-γ) tests in patients with HBeAg-negative НBV, n=132 analyzed the relationship. It was found that 75% of patients with HBeAg-negative НBV had viral load more than 20,000 IU\ml (n=99), in other cases, the amount of virus in the blood serum was below 20.000 IU\ml (n=35). These patients comprised two comparison groups, in the control group the blood of healthy donors was studied, n=50. The study revealed that patients with a higher viral load such as clinical syndromes asthenovegetative and hepatomegaly, and changes in biochemical parameters (increased levels of total bilirubin, Alt, AST, GGT) were significantly more common (p<0.001) compared to both the comparison group and the control group. Cytokine status indicators (IL-2, INF-α, INF-γ) in initial examination of patients had significant differences depending on the initial level of viral load in the serum. When the amount of virus was over 20,000 IU\ml there was observed decrease in the concentration of cytokines, in HBV DNA below 20.000 IU\ml their level was close to reference values. The low level of cytokines IL-2, INF-α, INF-γ in patients with HBeAg-negative НBV and high viral load can be explained by the consequence of the negative impact on the immune system due to long-term persistence of immunological body reaction in HBV.

MALT-LIMPHOMA OF THE STOMACH AND LYMPHOID HYPERPLASIA IN THE BACKGROUND OF HELICOBACTER-ASSOCIATED GASTRITIS: MORPHOLOGICAL AND IMMUNOHISTOCHEMICAL FEATURES
Abstract
The aim of the study was to identify new mechanisms of MALT-lymphoma of the stomach in patients with chronic H. pylori-associated gastritis. The article shows that the lowest level of the expression of Ki-67 and the molecule Bcl-2 is observed in lymphoid follicles of gastric mucosa in patients with chronic non-atrophic H. pylori-associated gastritis. Expression of Ki-67 and Bcl-2 has consistently increased in patients with chronic atrophic H. pylori-associated gastritis, reaching maximum values in patients with MALT-lymphoma of the stomach, which allows us to consider this as an option in the tumor progression of H. pylori-associated gastritis. The data obtained are able to improve the forecasting of H. pylori-associated gastritis.

THE ABILITY TO PREVENT SEVERE PRE-ECLAMPSIA DUE TO INDIVIDUAL PREGRAVID PREPARATION
Abstract
Goal. The aim of the article is to assess the effectiveness of individual pregravid preparation with patients at risk of severe pre-eclampsia. Methods. The authors used the method of retrospective identification of risk factors for severe pre-eclampsia in 118 patients (study group) who did not receive pregravid preparation and delivery of up to 33 weeks, inclusive. Prospective evaluation of the effectiveness of individual pregravid preparation in the comparison group, formed by accident - control of 90 patients planning pregnancy was used. Results: In the first phase of the study comparative evaluation of medical histories and clinical characteristics of 118 patients in the main group and 100 pregnant women in the control group revealed the following risk factors of severe preeclampsia: pre-eclampsia in past medical history, chronic pyelonephritis, chronic hypertension, a combination of polymorphisms of hemostasis and folate cycle; obesity; essential hypertension in family history; age over 31 years. The average number of risk factors per one patient in the control group is 2,64±0,9 and 2,78±0,3. The course of pregnancy and its outcomes in patients with the individual pregravid preparation were significantly more favorable than in those without one: preeclampsia manifested only in 60% of cases and was moderate; delivery date was delayed (31,2 ± 1,7 and 37,2 ± 0,8, p <0.001), the frequency of operative delivery was less (100% and 30%, p <0.001), perinatal mortality (3.4% ) and severe lesions of the CNS in neonates (24.6%) occurred only in the main group. Conclusion: Individual pregravid preparation of high risk patients for severe pre-eclampsia an prevent its development in 40% of cases, reduce the severity of its symptoms in 60% of cases, prevent perinatal moertality and severe damage to the central nervous system of newborns with a decrease in frequency of operative deliveries in three times.

IMPACT OF OBESITY ON FEMALE REPRODUCTIVE FUNCTION
Abstract
Standard body mass index or BMI categories fail to demonstrste corelatetion between obesity and the risk of poor reproductive function in young women. This article focuses on more specific risk factors associated with reproductive health in women apart increased BMI. The aim of this study was to evaluate the investigation of vaginal flora effectiveness as an existing screening routine technique on predicting poor reproductive outcome in obese women. For this reason we examined vaginal flora in 32 obese non-pregnant and 36 obese pregnant women. Also biochemical blood tests and examination of hormone levels was performed and compared with data obtained from 96 non-pregnant females with normal body mass index and without endocrine disorders. Unlike females with normal BMI obese patients were found to have dyslipidemia, increased level of inflammative proteins and leucocytes, activation of production of gonadotropin stimulating hormones and TSH with decreased level of prolactine, progesterone and free T4. In addition we found direct correlation between FSH, LH levels and BMI. The risk of vaginal dysbioses is not increased in obese women before and during pregnancy. Therefore further investigation and data are required in order to figure out valid route screening method in order to predict reproductive health risks in obese patients.

CLINICAL EVALUATION OF PHYSIOLOGICAL AND PATHOLOGICAL LACTOGENESIS
Abstract
To accomplish the objectives of the study to evaluate the diagnostic value of the clinical method in relation to the pathological course of lactogenesis 180 women in childbirth were studied. 140 women suffered placental insufficiency of varying severity during pregnancy, 40 had physiological course of gestation. Subjective and objective clinical criteria of early hypogalactia were investigated in dynamics of lactogenesis on the third and the sixth day. Verification of the diagnosis of early hypogalactia was carried out by assessing the daily amount of milk and its conformity with the needs of a newborn. The results of the study have confirmed the negative impact of the functional placenta state disorder during pregnancy on the development of lactational function. The method of evaluation of objective and subjective clinical signs of pathological lactogenesis course showed low informativeness (Se = 57,1%, Sp = 61,7%, P = 46,5%). It was concluded that the clinical method has prognostic and diagnostic value in conjunction with more informative methods.

THE CLINICAL REALIZATION OF FETAL PATHOLOGY ASSOCIATED WITH PLACENTAL INSUFFICIENCY
Abstract
The clinical realization of fetal pathology during gestation in pregnant in high risk group of decompensation of chronic placental insufficiency has been studied. The study revealed burdened obstetrical histories, somatic diseases, complicated gestation course and postpartum period, which justifies the necessity to use the prognostic scale of the risk factors for decompensation of placental insufficiency. This helps to select women of high risk for structural and functional abnormalities in placental system and perinatal pathology. It was revealed that a high morbidity and incidence of impaired early neonatal is due to the disruption in the functioning of the system «mother-placenta-fetus». This is the leading pathogenetic mechanism of impaired fetal condition. High frequency of the combination of growth retardation and fetal hypoxia proves pathogenetic unity of the two states associated with chronic placental insufficiency

DIAGNOSIS OF PROLIFERATIVE DIABETIC RETINOPATHY USE OF FLUORESCE IN ANGIOGRAPHY IN
Abstract
Proliferative diabetic retinopathy (PDR) is one of the most common reasons of blindness among working age people. The use of modern methods of primary VE is accompanied by a low surgical risk and causes an improvement of visual and anatomical results. The main diagnostic procedures used in the PRD are direct ophthalmoscopy, fluorescein angiography (FAG) and optical coherence tomography (OCT). In the PDR their implementation is difficult because of violation of the vitreous transparency. Intraoperative FAG allows you to diagnose macular edema and ischemia, to determine the location of the retinal avascular regions and areas of neovascularization. The result is a more precise holding of the ELKS and therefore improve treatment outcome and reduction in the number of complications.

THE USE OF PLATELET-RICH AUTOPLASMA IN THE TREATMENT OF PATIENTS WITH PYOINFLAMMATORY DISEASES OF SOFT TISSUES, BONES AND JOINTS
Abstract
The advances of modern medicine are largely determined by the development and introduction of new technologies, which change the traditional understanding of treatment options. One of the most promising areas of scientific research, of course, is cell technologies. As for pyoinflammatory diseases of soft tissues, bones and joints it’s necessary to search for ways of stimulating angio - and osteogenesis in conditions of severe degradation of extracellular matrix and cell pool does not provide a favorable microenvironment for injected active cells. Hence, we can observe different performance of the methods based on the introduction of allo- or autologous cells and tissues from different patients associated with chronic inflammation. The solution seems to involve the drugs and methods which prepare the connective tissue due to remodulation of its structure, increase of hydrophilism, saturation of the growth factors necessary for the successful functioning of the cells. Such influence is sometimes ample for the achievement of the expressed clinical effect. The group of multimodal methods can reasonably be attributed to the treatment with platelet-rich plasma (PRP, PRP). Having been confirmed by numerous experimental and clinical studies, the biological effects of PRP can be considered such therapy as one of the most promising and not completely studied treatments.

THE FIRST EXPERIENCE OF USING 320-SLICE CT PERFUSION IMAGING IN PROSTATE CANCER
Abstract
Introduction: Visualization of prostate cancer remains an actual problem in the urology and oncology. Purpose of the study: Evaluation of prostate cancer visualization using 320-slice perfusion computed tomography (PCT). Materials and methods: PCT results of 15 patients with suspected prostate cancer were evaluated. Studies were performed with the 320-slice spiral computed tomography Aquilion One (Toshiba, Japan). Perfusion of the prostate was calculated by the maximum gradient. Next to each perfusion map placed the zone of interest. For each area of interest the following indicators were taken into consideration: the average velocity of blood flow, normalized blood flow velocity, the difference of blood flow velocity in the contralateral areas of interest, the difference of the normalized blood flow velocity in the contralateral areas of interest. Differences between indicies and the results of histological examination for each zone of interest were evaluated by the variance analysis and by pairwise comparison with the Tukey’s criterion. Statistical significance of differences was assessed by the specialized computer language R v 3.2, using pROC packets. Results and conclusions: 180 biopsies were obtained. 19 ( prostate cancer Σ Gleason 6), 17 (prostate cancer Σ Gleason 7), 5 - (prostate cancer Gleason Σ 8), 7 (inflammation), 121 (no pathological), 11 (prostatic intraepithelial neoplasia (PIN)). The areas under the ROC curves for the studied parameters were: average blood flow rate 0.6343, normalized blood flow rate 0.5300, blood flow velocity difference in contralateral areas of interest 0.5875, normalized blood flow velocity difference in the contralateral areas of interest 0.6263. The obtained findings show a low sensitivity of the method in the detection of low-grade prostate cancer.
