Abstract

The article studies the findings of the biochemical and acid-base composition of the blood of pregnant women with pre-eclampsia, associated with chronic kidney disease. Preeclampsia is considered one of the most dangerous complications of pregnancy. The most severe form of preeclampsia developing in association with extragenital pathology, one of which is chronic kidney disease. The study of biochemical parameters which reflect the severity of multiple organ failure in pre-eclampsia are of particular importance. They contribute to the choice of infusional and systemic therapy.

Abstract

According to studies the majority of women with tubal-peritoneal infertility suffered from some form of chronic endometritis. Only complete examination of women with tubal-peritoneal infertility including immunocytochemical and histological study of the endometrium is necessary for successful IVF.

Abstract

A total of 318 couples undergoing IVF were observed. The multinomial logistic regression model was specified for global process. The main outcome measures were: ongoing pregnancy, miscarriage before and after 12 weeks gestation, live birth. The significant explanatory variables for global process were female age (odds ratio 1,064), previous live births (odds ratio 0,488), diminished ovarian reserve (odds ratio 2,589).

EFFECTIVNESS OF THE THERAPY OF PATIENTSWITH OVARIAN ENDOMETRIOTIC CYSTS

Abstract

The aim of this study was to evaluate the impact of post-operative dienogest (visanna) managment in dosage of 2 mg continiously on the rate of recurrence after laparoscopic excision of ovarian endometrioma. For this purpose we introduced a ‘post-operative dienogest recommendation’ for patients treated with laparoscopic excision of endometrioma and not seeking pregnancy. The endometrioma recurrence rate and clinical signs and severity were evaluated at 3 and 6 moths of post operative follow-up. This research showed effectiveness of the usage of dienogest in order to prevent post-operative recurrence of endometiotic cysts.

Abstract

The article is devoted to the assessment of the prognostic significance of the morphological and crystallographic methods for studying mammary glands secretion in postpartum women with a history of placental insufficiency pregnancy to select the most informative and timely improvement of this pathology. The early detection of hypogalactia in 210 postpartum women with placental insufficiency of varying severity during gestation was performed by determining morphological type of mammary glands secretion smear and crystallographic method. To calculate the sensitivity, specificity, positive and negative predictive value, the diagnostic accuracy of methods of early lactational disorders within the first 7 days of postpartum period we investigated subjective and objective clinical hypogalactia signs in the mother and the child, determined the amount of milk by gravimetric method, additionally measured the temperature in the armpit and lower quadrant of the left breast. Results of the study showed cytomorphological and crystallographic techniques to be comparable, but crystallography has the advantage and the detection rate of early hypogalactia and ease of use. This method has the highest predictive value (Se 99,3%, Sp 97,2%, P 98,4%) for the early hypogalactia compared with another studied method.

Acute kidney injury is one of the most prevalent pathologies of urinary tract in premature infants, affected by an acute hypoxic in perinatal period. AKI is an independent risk factor for poor outcomes in severely ill neonates. In neonatal period, importance and dilemmas of AKI are even more pronounced, as a newborn’s kidneys are more susceptible to hypoperfusion and have low glomerular filtration rate, high renal vascular resistance, high plasma rennin activity, decreased intercortical perfusion, and decreased reabsorbing of sodium in the proximal tubules.

A working mathematical model for a specific recipient allows to make a decision about increasing the frequency of laboratory tests to identify CMV infection. СOMPARATIVE ANALYSIS OF THE RELATIONSHIP OF CLINICAL AND LABORATORY DATA WITH THE OUTCOME OF SEPSIS IN ICU

Abstract

Objective. The study aims to analyze the dynamic relationship of clinical and laboratory data of patients with sepsis and the outcome in an intensive care unit. Materials and methods. A retrospective analysis of 108 case histories of patients with sepsis was made. The average age of patients was 50,5 ± 15,5 (maximum 77 y.o., minimum 21 y.o.). Men accounted for 58% of all incoming patients. 29 patients were in the state of septic shock. On admission the state of 69 patients resulted from abdominal surgical pathology, 25 suffered from severe soft tissue infection, 10 patients had nosocomial pneumonia, 4 patients had angiogenic catheter infection. Relative mortality rate was 32.4%. Criteria included into the integrated severity scales SOFA and SAPS II were studied. Taking into consideration the current studies [10-12, 45-53] the importance of the following laboratory parameters - the rate of lymphocytes, thromboelastography index and Pct level were investigated. Results. The multiple regression analysis in the common group and the group of survived patients with sepsis taking into account the dependent variable of the disease outcome in the ICU and the hospital bed-day in the ICU showed early significant changes in hemostasis which required timely correction. Нypercoagulable syndrome, thrombocytopenia initiation, activation of secondary fibrinolysis are characteristic of the survived patients. Hyperfibrinolysis with the normo- and hypocoagulation syndrome, progressive thrombocytopenia are observed in the common group of patients. Conclusion. Nonspecific clinical symptoms of sepsis necessitate the choice of early laboratory sepsis markers in order to initiate the timely targeted therapy. Dynamics of hemostasis, identified by thromboelastography is the early predictor of sepsis. They characterize the worsening thrombinemia. Valid additional criteria of unfavorable sepsis outcome in order of importance were the percentage of lymphocytes, leukocytosis and procalcitonin levels.

The article is devoted to modern views on the development of fatty hepatosis in patients with chronic hepatitis С associated with lipidic distress syndrome. The analysis of the effect of antiviral therapy on the level of fatty hepatic infiltration depending upon the genotype of the virus in patients with chronic hepatitis С is performed. Received findings revealed the degree of severity of fatty hepatosis not to depend on the genotype of the virus. Antiviral therapy reduces the severity of fatty infiltration of the liver in case of genotype 3a in patients with chronic hepatitis and lipidic distress syndrome.

Abstract

About 30% of COPD exacerbations are due to viral infections, and influenza A and B viruses are among the most common causes. The aim of the study is to assess the exacerbation rate, the number of courses of antibiotic chemotherapy, and immunological effects of mono vaccination with a new immunoadjuvant influenza vaccines vs. combined vaccination against pneumococcal infection, Haemophilus type b infection, and influenza in COPD patients. Both complex vaccination against S. pneumoniae, H. influenzae type b, and influenza and mono vaccination with a new immunoadjuvant influenza vaccine lead to statistically significant reduction in the number of COPD exacerbations and the number of courses of antibiotic chemotherapeutic agents. The study represents the immunogenicity of the new influenza immunoadjuvant vaccine administered as monovaccine to COPD patients in accordance with СРМР requirements.

ABOUT THE POSSIBILITY OF METASTATIC THYROID LESIONS IN KIDNEY CANCER

Abstract

The article is devoted to modern views on the frequency, the preconditions, the characteristics of the course, the difficulties of early diagnosis and treatment of metastatic thyroid lesions in kidney cancer. The impact of metabolic changes in the tissues of the thyroid gland in various diseases on the incidence of this disease is analyzed. Difficulty in clinical diagnosis of metastasis of clear cell kidney cancer in the thyroid gland is emphasized. The data of 9 observations of clear cell kidney cancer metastasis in the thyroid gland identified in Ryazan City Clinical Hospital № 11 are presented in the article. It is necessary to take into account the medical history data of the patient, characteristic ultrasound findings, cytological findings and the possibility of clinical manifestations of thyroid metastasis in the late postoperative period. Surgical treatment is the method of choice in case of metastatic lesions of the thyroid gland.

SURGICAL TACTICS ON NUMEROUS RELAPOROTOMY ON PATIENTS WITH GENERAL PURULENT PERITONITIS

Abstract

Of the total 495 patients diagnosed with generalized purulent peritonitis were enrolled in the study, 160 patients underwent elective relaporotomy. The patients were stratified into those having a high or low risk of developing intra-abdominal purulent complications. Relaporotomy was indicated if the risk of developing intra-abdominal purulent complications was high. Based on the severity stratification score, the patients were divided into three groups of those having low (0-8 score), high (9-15 score) and extremely high (16-18 score) risk of developing intra-abdominal purulent complications. The use of severity stratification score resulted in a 13,2% reduction in mortality in patients who had elective relaporotomy compared with 38,7% of deaths in patients who underwent emergency relaporotomy.

RADIOFREQUENCY ABLATION FOR METASTATIC LUNG LESIONS

Abstract

Metastatic lung lesions remain the “issue of the day”. The frequency of lung metastases according to different authors vary from 1.6 to 55.4% depending on the localization and histological structure of the primary tumor. The objective of this study was to evaluate the effectiveness of radiofrequency ablation in patients with lung metastases. 317 radiofrequency ablations were performed in 170 patients with lung metastases. The effect of primary tumor localization and the amount of metastasis on overall survival was investigated.

Abstract

The purpose of the study was to improve the treatment results for patients with suppurative necrotic complications of diabetic foot syndrome (DFS) without critical ischemia by using the programmed aspiration and irrigation rehabilitation (PAIR) method. The patients with DFS were divided into two groups depending on the way of postoperative rehabilitation of the suppurative focus after radical surgery. In the reference group, rehabilitation was carried out according to conventional techniques. In the study group, the combined treatment was supplemented with a new method of PAIR using the original equipment AMP-01. The results of the performed investigations confirmed the efficiency of the PAIR method, the use of which provided normalization of laboratory findings in the study group at an earlier date than in the reference group. This has contributed to an earlier relief of inflammation in the suppurative focus and to the transition from the inflammatory to the proliferative phase of the wound healing, as well as to reduction of the time of treatment and rehabilitation in this group of patients.

Abstract

Objective. The specific features of disease course and results of surgical corrections of hiatal hernias, and analyses of possibly most effective method of choice in elderly people are analyzed. Methods. 49 cases of hiatal hernias surgical corrections were analyzed. Patients average age was - 66, 7 ± 4,2. Results. We found out that tension-free methods of laparoscopic plastics of giant hiatal hernias did not result in lower complications rate in postoperative period or higher level of quality of life. Conclusions. We found no statistically significant evidence of higher efficacy of tension-free surgical corrections in comparison to traditional method of Nissen, in cases of surgical treatment of elderly people with hiatal hernias (frequency of functional complications, rate of recurrent hernias, quality of life were analyzed).

GENETIC MARKERS CIMBINED WITH CLINICAL AND LABORATORY PARAMETERS ON THE PROGNOSIS OF COMPLICATIONS AFTER PERCUTANEOUS CORONARY INTERVENTION IN PATIENTS WITH ST-ELEVATION ACUTE CORONARY SYNDROME

Abstract

Aim: the aim of the study is to investigate the effect of genetic variants combined with clinical and laboratory parameters on the prognosis of complications after PCI in patients with ST-elevation and acute coronary syndrome. Material and methods. 171 patients, consequently hospitalized with ST-elevation ACS from 15.08.2011 to 02.11.2014 were observed. Patients with the recurrence of coronary insufficiency after PCI formed the group I (n = 43, 38 male patients, 88.4 %, average age 56.7 ± 10.2). Recurrent coronary angiography revealed stent restenosis of the infarct-related artery and repeated stenting was performed. Group II consisted of 128 patients (103 males, 80.5%) with an uncomplicated postoperative period, average age 56,3 ± 10,8). The results of genetic study by 7 polymorphic genetic variants, coronary angiography data, ECG, laboratory findings were analyzed. The results. Groups significantly differed in outcomes of PCI: recurrence of myocardial infarction was 9,3% and 0%, resp. (p=0,004), frequency of myocardial reinfarction was 21,0% and 0%, resp.(p<0,0001). The combination of alleles (4G/4G) of gene PAI-I + (G455A) of gene FI + (Т1565С) of gene I GPIII was defected in 9 patients (21,0%) of the group I and it was detected in 11 patients (8,6%) of group II when analized, resp.(p=0,03). The combination of alleles (G455A) of gene FI + (Т1565С) of gene I GPIII was detected in 10 patients (23,3%) of group I, this combination was met in 15 patients (11,7%) of group II. The combination of alleles (4G/4G) of gene PAI-I + (G455A) of gene FI + (Т1565С) of gene I GPIII + (G10976A) of gene VII was observed in 4 patients (9,3%) of group I and it was observed in 1 patient (0,8%) of group II, resp.(p=0,01). The combination of alleles (G455A) of gene FI + (Т1565С) of gene I GPIII + (G10976A) of gene VII was detected in 4 patients (9,3%) of group I and it was detected in 2 patients (1,6%) of group II, resp.(p=0,04). Conclusion. The combinations of genetic polymorphisms FI + PAI-I + GPIII, PAI-I + FI + GPIII + FVII, FI + GPIII, F1+ GPIII +FVII and FI + GPIII + FVII are associated with restenosis of stented infarct-related artery.

Abstract

Acute myocardial infarction is a condition associated with a complex of haemodynamic, inflammatory and metabolic sequelae. In 65 males the pathogenic role of leptin, resistin, adiponectine was investigated in acute myocardial infarction and early recovery period. Studies revealed the dynamic changes of adipokines in acute myocardial infarction. These adipokines are involved in postinfarction inflammation and contribute to insulin resistance. The imbalance of adipokines causes decrease of patients’ life quality. Authors revealed positive effect of adiponectin on the body composition parameters according to bioimpedance.

Traumatic brain injury is a current problem. Research objective is to improve the diagnostics of a slight craniocerebral injury by means of heart rate variability investigation in order to objektivize of vegetative disorders. Materials and methods: 120 patients with cerebral concussion were studied. They underwent cardiointervalography in the acute period of the trauma and the dynamics of their state was investigated. Results: the obtained data allow to draw a conclusion that the cardiointervalography helps to reveal vegetative dysfunction at a slight craniocerebral injury and the recovery of the vegetative status on the treatment.

This article presents an analysis of hospital purchase of drugs on the pharmaceutical market of the Volga Federal District. It was found that hospital purchase accounting for 14 % of all on the pharmaceutical market in the Volga Federal District, through 2011 to 2015 its volume increased by 48% in monetary terms. The purchased medical agents of foreign origin are of the maximum rate in monetary terms. Meanwhile the analysis of hospital medical purchase shows domestic medical agents to be more preferable in kind.

THE ANALYSIS OF AGE-RELATED DYNAMICS AND THE METHOD OF BODY MASS INDEX INTERPRETATION IN CHILDREN OF SAMARA REGION

Abstract

The body mass index (BMI) is an indicator of for the assessment of the adequacy of nutrition. Due to the lack of developed regional standards, BMI is rarely used in pediatric practice. Currently there’s a tendency for increasing obesity among children which requires the optimization of the method BMI assessment in children and adolescents. Research findings demonstrate some positive correlation between BMI and the age of children in Samara region (r=0,588; p < 0,001). Comparative analysis of the body mass index of city school-aged children and those of rural areas in Samara region showed the significant differences (p < 0,05) of BMI in 8 - 14 y.o. boys and 9 - 14 y.o. girls. High age variations of the body mass index and its heterogeneneity in the study area, indicate the need to use regional standards of BMI. In this connection the suggested nonparametric statistical method is more preferable for the interpretation of the values of the BMI.
