TREATMENT AND PREVENTION OF CERVICAL DYSPLASIA IN WOMEN LIVING IN THE ENVIRONMENTALLY POLLUTED AREA OF THE CITY
Abstract
The results of conservative and surgical treatment of patients diagnosed with dysplastic diseases of ectocervix, living in the environmentally polluted area of the city and in the region with more favorable environmental conditions, were examined. The conclusions on the effectiveness of various methods and the need to implement the prevention programs for cervical dysplasia were drawn. The detection of abnormality of enzyme activity and content of chemicals in the cell, observed in the absence of clinical manifestation of the disease, will allow correcting the pathological conditions in the subclinical stage.

HYPERPLASTIC DISEASES OF ENDOMETRIUMIN PATIENTS LIVING IN UNFAVOURABLE ECOLOGICAL CONDITIONS
Abstract
Comparison of the groups of patients living in conditions of ecological trouble and more favorable environmental conditions shows that long-term exposure of the woman’s organism to the harmful impurity of air decreases its adaptive capabilities. This leads to the development of the reproductive system diseases. Based on the research and analysis of the results of therapy of endometrium hyperplastic diseases, the author formulates the priority directions of endometrium diseases prevention programs for residents of the polluted areas.

OBESITY IS AN INDEPENDENT RISK FACTOR FOR DEVELOPMENTOF GESTATIONAL COMPLICATIONS
Abstract
The main goal of this research was to evaluate if there is an impact of obesity on pregnancy outcomes, particularly on weight of newborn infants. 308 delivery cases were reviewed, followed by investigation of placental histology after deliveries in women with normal weight, increased body mass index and obesity. We compared placenta’s morphology findings, biochemical blood parameters and risks for hyportrophic infants born in preterm and normal deliveries of women with different body mass indices. This study shows that obesity increases the risk of hypotrophy of newborn in preterm deliveries on 28-36 weeks of gestation by 4,4, but the highest level (by 6) was noted on 28-32 weeks of gestation, and only 3,4 on 33-36 weeks of gestation.

OPTIMIZATION OF MANAGEMENT OF PREGNANT WOMEN WITH EARLY TOXICOSIS BY SELECTION OF PREDICTORS OF COMPLICATED GESTATION COURSE
Abstract
The aim of the present study was selection of pathogenetically substantiated informative predictors of severe early toxicosis. Quantitative objectification of markers of embryo(feto)placental dysfunction and metabolic disorders was conducted in 172 pregnant women with early toxicosis of varying severity in the course of a prospective study. Clinical diagnostic parallels allowed us to determine the predictive value of diagnostic tests in relation to the development of severe toxemia (blood levels of leptin, fasting glucose, FER, L CD95+, total IgE, CRP, and TNF-a, the maximum amplitude of platelet aggregation, fibronectin). Active implementation of predictors of severe early toxicosis will allow timely prevention of this complication and complications of the second half of pregnancy.

CLINICAL AND MICROBIOLOGICAL CHARACTERISTICSOF CERVICAL DYSPLASIA
Abstract
The nature of vaginal fluid and its microbiocenosis in residents of environmentally disadvantaged areas and areas with more favorable environmental conditions was examined for the purpose of determining the role of microbial factors in the etiology and pathogenesis of ectocervix dysplasia. The study showed that the cervical dysplasia causes a significant abnormality of the microecological system, especially noticeable in women from the main group, which is reflected in a significant increase in the number of different species of microorganisms with a predominance of strict anaerobic bacteria. A direct moderate correlation between duration of patients’ habitation in environmentally disadvantaged areas and the growth of microbial and fungal flora in smears was found.

COMPARATIVE ANALYSES OF DIAGNOSTIC METHODS OF EARLY HYPOGALACTIA USING TESTS OF CLINICAL EPIDEMIOLOGY
Abstract
The article is devoted to assessing the diagnostic value of different methods of diagnosing early hypogalactia to select the most informative and correct this pathology in time. In the study, detection of early hypogalactia was conducted in 200 pregnant women with risk factors for developing this disease with the help of diagnostic tests, including crystallographic method. The study assessed the clinical effectiveness of the method of diagnosis of early hypogalactia, determining the qualitative composition of the mammary glands and secretion of prolactin levels in blood serum, the thermometric method, the method using a 15% soap solution, smear morphotypes of secretion of the mammary glands, crystallographic method. To calculate the sensitivity, specificity, positive predictive value and negative results, the diagnostic accuracy of diagnostic methods for early disorders of lactation function, the daily amount of milk on the 4, 5, 6 days of post-partum period was counted retrospectively, which revealed, taking into account the needs of a newborn, the presence or absence of early hypogalactia. The study showed that the crystallographic method of diagnosing early hypogalactia has the greatest diagnostic significance (87.9% Se, Sp 88.3%, P 88.0%), compared to other studied methods.

INFORMATIVE PREDICTORS OF MAJOR OBSTETRICAL SYNDROMES IN PREGNANT WOMEN WITH EMBRYPLACENTAL DYSFUNCTION
Abstract
The article evaluates the prognostic significance of the predictor indices that reflect the state of the major biological systems involved in the formation of the embryo(feto)placental disorders and major obstetrical syndromes. As a result of the conducted study of predicting major obstetrical syndromes (PN, PE, PERP, ETC) using tests of clinical epidemiology - the sensitivity, specificity, predictive values of positive and negative results, diagnostic accuracy, the informative predictor indices were selected (PI= FER/TNF x 100; PI= PAMG/FER x 100; PI= L CD95+/PSF x 100), reflecting the degree of impairment of various compensatory and protective-adaptive processes in FPC and system dysfunction. High prognostic value of the proposed predictor index can be recommended for widespread practical use.

THE RISK FACTORS FOR FUNCTIONAL DISORDERS ASSOCIATED WITH ABDOMINAL PAIN IN ADOLESCENTS
Abstract
Etiopathogenetic aspects of functional disorders of the gastrointestinal tract (GI FD) manifested in abdominal pain in adolescents are under-investigated, and currently there are certain problems in treatment of these conditions. At the present stage, biopsychosocial model of GI FD seems to be the most reasonable. From this perspective, we found that the leading factors of formation of the GI FD in these age groups are mainly psychosocial factors.

TEN-YEAR DYNAMICS OF BIOLOGICAL RISK FACTORS IN TODDLERS WITH IRON DEFICIENCY ANEMIA AND THEIR HEALTHY PEERS
Abstract
Iron deficiency anemia has a significant negative impact on various aspects of children’s health. Wide variety of risk factors and com plicity of risk groups are hampering the timely prevention of the disease. The aim of the study was the assessment of the dynamics of known risk factors of iron deficiency anemia in toddlers. A retrospective study of known risk factors frequency in 184 children born in the period from 01.07.1998 to 31.06.1999 (101 with iron deficiency anemia and 83 healthy) and 228 children born in the period from 01.07.2008 to 31.06.2009 (135 with iron deficiency anemia and 93 healthy) was held. We found the change of significance of most antenatal risk factors. Mother’s chronic pathology of gastrointestinal tract and atopic diseases are significant risk factors of IDA. We found increased impact of mother’s endocrine disease on the development of IDA in toddlers. Antenatal risk factors that directly disrupt placental transportation of iron from mother to the fetus such as premature birth and placenta pathology also became more important. Low birth weight (more than -3SD) and length at birth less then median (more than -1SD) are risk factors of IDA. The duration of breast-feeding for more than 9 months of life is a new risk factor of IDA in toddlers.

ESTIMATION OF THE MORPHO-FUNCTIONAL STATE OF THE MYOCARDIUM IN CHILDREN WITH PREMATURE BEATS
Abstract
The article presents information on the structural changes of the heart in children with ventricular and supraventricular premature beats. Statistical processing using contingency tables and the calculation of χ2 allowed determination of significant differences between the values. Analysis of the main indicators of the morpho-functional state of the myocardium in patients with ventricular and supraventricular premature beats made it possible to identify the most adverse changes in the structure of the heart and blood vessels.

SOCIAL FACTORS DYNAMICS IN CHILDREN WITH EARLY ONSETOF IRON DEFICIENCY ANEMIA AND THEIR HEALTHY PEERS
Abstract
The frequency of iron deficiency and iron deficiency anemia (IDA) remains high in the pediatric population, but due to possible adverse effects of iron supplementation, routine prophylaxis is limited. In this regard, it is important to detect risk groups of iron deficiency anemia. After exclusion of antenatal risk factors, one of the most significant risk factors is the low socioeconomic level. The aim of the study was the assessment of IDA socio-economic risk factors dynamics. A retrospective study of known socio-economical risk factors frequency in 184 children born in the period from 01.07.1998 to 31.06.1999 (101 with iron deficiency anemia and 83 healthy) and 228 children born in the period from 01.07.2008 to 31.06.2009 (135 with iron deficiency anemia and 93 healthy) was held. Socio-economic factors in modern conditions have little effect on IDA in toddlers. Protective effect against IDA has mother’s status of a housewife. At the greatest risk of IDA are toddlers who are brought up in large families (3 children or more).

CORRELATION OF POSITIVE FAMILY HISTORY OF ALLERGIC DISEASES IN INFANTS AND THE NATURE OF THEIR FEEDING AT THE FIRST YEAR OF LIFE
Abstract
The aim of our study was to identify frequency of positive family history of allergic diseases in infants and its impact on the nature of their feeding at the first year of life. After analyzing the results of parents’ survey of 1030 young children, we have identified positive family history of allergic diseases in 294 babies (28.5%). The character of their feeding had little difference from the feeding of infants without any allergic family history, which does not meet the current recommendations for the prevention of allergic diseases in children at risk.

ANALYSIS OF QUALITY OF LIFE OF PATIENTS WITH COLOSTOMY
Abstract
This article is dedicated to the issue of quality of life of patients with colostomy who were operated on for complicated colorectal cancer. Quality of life after resection of obstructive colon has been studied using a scale FACT-C, which consisted of the sum of the values of the scale FACT-G and a special module for colorectal cancer CRC - colorectal cancer concerns. The comparative analysis of the quality of life after operation was performed for each module scales FACT-G, a special module CRC and the total scale value FACT-G, FACT-C and TOI. The study concluded that the dynamics totals FACT-G, FACT-C, TOI and additional questions show the improving quality of life of patients with colostomy. Colostomy was formed in obstructive colon resection using the technique of sphincter modeling by the proposed method.

Abstract
This study presents the results of the treatment of 168 patients with the diagnosis of colorectal cancer stage 4 with bilobar liver metastases. In the control group (78 patients), cytoreductive surgery was carried out with atypical liver resection. In the study group (90 patients), removal of the tumor was followed by radiofrequency thermal ablation of liver metastases.

ADVANTAGES OF CT-FLUOROSCOPY AS A METHOD TO CONTROLTHE POSITION OF THE ELECTRODE AT RADIOFREQUENCY ABLATION OF LUNG TUMORS
Abstract
From 2008 to 2012 in the Samara Regional Clinical Oncology Center radiofrequency thermoablation (RFA), using different guidance systems, has been performed on 306 lesions in 221 patients with primary and metastatic lung tumors. 22 (7.2%) were completed under the fluoroscopy, 31 (10.1%) under the control of computed tomography (CT), 65 (21.2%) were guided by combinations of CT and fluoroscopy, 147 (48.1%) with CT-fluoroscopy guidance, 19 (6.2%) under the cone-beam CT and 22 (7.2%) under ultrasound control. We have found that CT-fluoroscopy is the best guidance system for lung minimally invasive interventions.

DYNAMICS OF INDICATORS OF HOMEOSTASIS, METABOLISM, HORMONAL, IMMUNE, CARDIAC STATUS OF PATIENTS WITH THYROID CANCER IN PRE- AND POSTOPERATIVE PERIODS
Abstract
As a result of studies the authors identified that 65 patients (who had differentiated thyroid cancer T1-3 N0-1 M0) had multiple hormonal changes, disorders of homeostasis and metabolism, disruption of antioxidant processes of cellular and humoral immunity, of cardiovascular system in the preoperative period and in the early postoperative period. In the recovery period there was no positive trend for the majority of investigated parameters: they marked persistent destabilization of the myocardium with increased frequency and increased graduation of ventricular arrhythmia, indicators of psycho-emotional condition deteriorated. Thus, hormone therapy in combination with traditional cardiotherapy does not solve all the problems of disadaptation, which requires further search for new science-based methods of complex rehabilitation of patients with thyroid cancer.

Abstract
The aim of this study is to explore the possibilities of intraoperative ultrasonography (IOUS) in patients with VATS resections of lung lesions. The article presents practical results of this imaging technique in 391 patients who were treated from 2011 to 2014. The effectiveness of IOUS is compared with classical methods of searching intraparenchymal lesions with subsequent video-assisted thoracic surgery (VATS).

COMPARATIVE ANALISIS OF SURGICAL TREATMENT OF CHILDREN WITH CONGENITAL OBSTRUCTIVE MEGALOURETER
Abstract
The paper presents the evaluation of results of surgical treatment of children with congenital obstructive megaloureter in the Department of pediatric urology of Samara regional clinical hospital named after V. D. Seredavin. The most complete restoration of anatomy and function of the urinary tract was achieved in children who underwent drainage of the urinary tract on the affected side and correction of bladder dysfunction. According to our results, intravesical transplantation of the ureter gives better results compared to extravesical transplantation, due to the technical ability to meet the required ratio of the ureter’s diameter to the length of the submucosal tunnel during the transplantation.

OSTEOSYNTHESIS AT RIB FRACTURES:THE ASPECTS OF SURGICAL TECHNIQUE
Abstract
The article describes modern aspects of surgical treatment of patients with multiple rib fractures and flail chest. The main method of treatment at these patients was rib osteosynthesis. The comparison between different kinds of surgical approaches was carried out. In addition, the basic criteria and conditions for every approach were specified. The advantages and disadvantages of surgical accesses were marked. We described some methods of improvement of the technique and optimization of the approaches. The necessity for performing thoracoscopy in order to eliminate intrathoracic injuries was indicated.

TECHNOLOGY AND SCOPE OF OPERATIONS IN PATIENTS WITH PRIMARY AND TERTIARY HYPERPARATHYROIDISM
Abstract
Purpose: improving outcomes in patients with primary and tertiary hyperparathyroidism by the study of techniques and the volume of intervention in the thyroid and parathyroid glands. Materials and methods. The analysis of treatment of 52 patients with hyperparathyroidism was carried out: 21 patients with primary and 31 patients with tertiary hyperparathyroidism. Among them there were 15 men (28.8%) and 37 women (71.2%). The age of patients ranged from 20 to 80 years. In 71.4% of patients with primary and 77.4% with tertiary hyperparathyroidism the parathyroid gland pathology was combined with pathology of the thyroid gland. In patients with primary hyperparathyroidism, the operation was aimed at the removal of adenomas with high activity and maintenance of unaltered parathyroid glands. In patients with tertiary hyperparathyroidism, it was aimed at total parathyroidectomy. In case of combined pathology in organs, subtotal resection of the thyroid or thyroidectomy was performed. The operation technique is described in detail. Results. In addition to the removal of adenomas in 21 patients with primary hyperparathyroidism, subtotal resection of the thyroid gland was performed in 3 patients, and 12 patients underwent thyroidectomy. Among 31 patients with tertiary hyperparathyroidism, 17 patients had a multi-colloid goiter, which required performing thyroidectomy. Also while searching for parathyroid adenomas, 7 patients required mobilization of the thyroid gland, which was completed by its resection. Conclusion. 55.7% of patients with hyperparathyroidism have a combination of parathyroid and thyroid diseases, which requires implementation of thyroidectomy and removal of adenomas of parathyroid glands. In 19.2% of patients with hyperparathyroidism, the peculiarities of parathyroid adenoma location requires mobilization of the thyroid gland, which causes resection of the thyroid gland and removal of parathyroid adenomas.

THYROIDECTOMY TECHNIQUE AS THE BASIS FOR IMPROVING THE RESULTSOF TREATMENT OF PATIENTS WITH BENIGN THYROID DISEASE
Abstract
Objective: improving outcomes of treatment of patients with diffuse and mixed toxic nodular euthyroid goiters by justification of thyroidectomy and prevention of complications at its performance. Materials and methods. The analysis of the results of thyroidectomy was performed in 243 patients who had diffuse toxic (60/24.7%) and mixed-toxic (86/35.4%) goiter, and also nodular euthyroid goiter (97/39.9%). The patients were divided into two groups according to the method of thyroidectomy. The first group included patients who underwent thyroidectomy performed from the isthmus, the second group - from the side surfaces of the thyroid gland. Results. After thyroidectomy from the isthmus without visualization of recurrent laryngeal nerves in 57 patients, there was a violation of phonation in 8 (14%) patients, after thyroidectomy in a single block from the lateral surfaces of thyroid lobes with visualization of the location of recurrent laryngeal nerves - in 2 (1.07%) patients with substernal goiter. Conclusion. Thyroidectomy technique from the side surfaces of the thyroid gland with preservation of the inferior thyroid artery and ligation of branches reaching the thyroid gland under control of recurrent laryngeal nerves provides the minimal risk of development of phonation disorders.

SURGICAL TREATMENT OF UREMIC TUMORAL CALCINOSIS IN PATIENTS WITH TERTIARY HYPERPARATHYROIDISM
Abstract
Objective - to work out tactics and techniques of operation in patients with tertiary hyperparathyroidism associated with uremic tumoral calcinosis. Materials. Tactics and methods of treatment of uremic tumoral calcinosis are not well-described because of the rarity of the pathology. The article presents clinical observation of the patient A. born in 1954. Diagnosis: chronic renal failure, tertiary hyperparathyroidism, parathyroid adenoma, osteo-articular form; uremic tumor calcification in the right shoulder and arm. Ca = 2,4-2,55 mmol / l, P = 1,7-1,8 mmol / l iPTH = 800 pg / ml. Result. Treatment of patient was carried out in two stages. The first step was removal of 6 parathyroid adenomas, the second - opening three calcification cysts in the right upper limb - 750 ml in total volume, containing white exudate resembling lime solution. The postoperative period was uneventful. IPTH = 60 pg / ml. Conclusion. 0.09% of patients with chronic renal failure on hemodialysis may develop uremic tumoral calcinosis, the treatment of which is aimed at excision of the parathyroid gland adenomas, opening and drainage of para-articular pathologic cysts.

REPEATED SURGICAL INTERVENTIONS IN THE RECURRENCE AND REGIONAL METASTASES OF THYROID CANCER

The article is devoted to repeated surgical interventions in patients with differentiated forms of thyroid cancer. The results of surgical treatment of patients with local recurrence and regional metastasis of thyroid cancer were analyzed. According to the results of own experiments, conclusions on the tactics of management of patients with this pathology were formulated.

EMERGENCY OPERATIONS IN PARATHYROID SURGERY
Abstract
The article deals with analysis of urgent situations in parathyroid surgery. Six patients out of 214, who underwent surgery in 2000-2013, were operated urgently. Three patients out of five, who underwent surgery associated with hypercalcemic crisis, died in immediate postoperative period. Surgery was indicated to one patient with compression of organs located in the neck region.

This article is devoted to the quality of life of 70 patients, who were operated for nodular and multinodular euthyroid goiter in the surgical department of Clinical Road Hospital on station Samara in 2005-2012. The study was conducted using a specific questionnaire evaluating the severity of relevant symptoms of patients with benign thyroid disease and general SF-36 questionnaire. It is shown that severe symptoms in the late postoperative period are neurological (cognitive), cardiac and emotional. Quality of life of patients who have undergone organ operations is significantly higher than after total thyroidectomy.

THE DISEASE VON HIPPEL-LINDAU TYPE 2C (VHL SYNDROME)IN TWO GENERATIONS OF ONE FAMILY
Abstract
The disease von Hippel-Lindau (VHL) is an autosomal dominant inherited disease characterized by predisposition to various cancers. It is caused by a mutation of the 3rd chromosome at the site 3p25/26, where suppression gene of tumor growth is localized. The clinical course of the disease is divided into two types: type 1 - without development of pheochromocytoma (an affection of the retina, tumors of the brain and spinal cord, pancreatic, renal, and splenic cysts, solid pancreatic tumors), type 2 - with the development of pheochromocytoma (subdivided into subtype 2A with a low risk of development of kidney cancer, 2B - high risk of developing kidney cancer, 2C - development of only pheochromocytomas). The paper presents authors’ own observation of the family in two generations with type 2C VHL syndrome.

PRACTICE OF SURGICAL TREATMENTOF TERTIARY HYPERPARATHYROIDISM
Abstract
Tertiary hyperparathyroidism (THPT) is a disease often followed by adenomatous transformation and autonomation of parathyroid glands; it is more common in patients older than 50, who are on hemodialysis. Ultrasonic examination and scintigraphy with Tc99m of parathyroid glands reveals only 70-75% cases of parathyroid adenoma. That is why the examination must include MRT (magnetic resonance tomography) of parathyroid glands. After surgery, patients with THPT need constant follow-up because of possible relapse.

CARNEY COMPLEX, ASSOCIATED WITH ENDOGENOUS HYPERCORTISOLISM
Abstract
We present a clinical case of endogenous hypercortisolism. After examination, 18-year-old patient was diagnosed with Cushing's syndrome, presumably caused by corticosteroma of left adrenal gland. Left laparoscopic adrenalectomy was performed. Pathologohystological conclusion was adrenal pigmented nodular dysplasia. In postoperative period hypercortisolism symptoms regressed. During the 2-year lasting observation of patient no relapse was found. In retrospect, the patient revealed symptoms of a rare hereditary disease - Carney complex. The interest of submitted observation lies in its exceptional rarity. Unilateral adrenalectomy was effective, but the probability of recurrence of Cushing’s syndrome and other manifestations of Carney complex still exists.

CLINICAL FEATURES OF PRIMARY HYPERPARATHYROIDISM IN PATIENTS WITH UROLITHIASIS
Abstract
Patients with PHPT combined with urolithiasis are a special group of patients, which is characterized by some epidemiological, clinical and biochemical features. The most notable features were hypercalcemia and male to female ratio. Both parameters were higher in the group with urolithiasis. The incidence of urolithiasis in PHPT population was 28.1%. In patients with urolithiasis calcium levels were significantly higher (3.1 vs. 2.57 mmol / l). They were younger (49.5 vs. 55.4 years) and among them there were more men (male to female ratio - 1:5 to 1:10,5).

CAN WE CONSIDER THE VOLUME OF THYROID REMNANT THE ONLY RISK FACTOR FOR RECURRENCE OF HYPERTHYROIDISM?
Abstract
The article presents long-term results of treatment of 58 patients with diffuse toxic goiter after surgical subtotal resection of the thyroid gland. The analysis of risk factors for recurrence of hyperthyroidism was performed. It was concluded that the volume of thyroid remnant is not prognostically significant factor in the risk of recurrence of hyperthyroidism.

COMMUNITY-ACQUIRED PNEUMONIA IN THE PRACTICE OF A PHYSICIAN IN PULMONOLOGY CENTER
Abstract
The analysis of the data of patients hospitalized with the diagnosis of “community-acquired pneumonia” to the pulmonology center based in Samara City Hospital №4 over the period two years was carried out. Every year the highest number of hospital admissions is registered among young patients of working age, mainly men. One of the main causes of mortality in patients with community-acquired pneumonia remains HIV-infection, often at the stage of severe immunosuppression. Meanwhile, the commitment to highly active antiretroviral therapy (HAART) in patients with HIV-infection and community-acquired pneumonia is 5,14%. The number of newly diagnosed patients with tuberculosis (TB) also remains high (17%) and requires the active cooperation of pulmonology and TB-services.

MICROCIRCULATION DISTURBANCES IN PATIENTS WITH POLYCYTHEMIA VERA AND ARTERIAL HYPERTENSION
Abstract
This article presents the features of the processes of microcirculation in patients with polycythemia vera and hypertension. Microcirculation was assessed using laser Doppler flowmetry. It was found that the studied patients mainly showed spastic type of microcirculation, which is characterized by the decrease in the arithmetic mean value of perfusion and the increase in average flow reserve. The increase in the bypass rate was determined. The revealed features of microcirculation indicate the increased risk of cardiovascular complications in observed patients.

INFLUENCE OF INTRADERMAL AUTO-SERUM THERAPY ON THE LEVEL OF IGE-ANTIBODIES IN CHILDREN WITH ATOPIC BRONCHIAL ASTHMA
Abstract
Objective: study of the influence of intradermal auto-serum therapy on the level of IgE-antibodies in children with atopic BA (bronchial asthma). Methods. A course of intradermal auto-serum therapy was conducted in 44 children with atopic BA with sensitization to house dust mite allergens. The quantitative analysis of IgE-antibodies to the mixed of mites D. pteronyssinus and D. farinae in serum was performed before the course of auto-serum therapy and afterwards. Results. There was a statistically significant decrease of the levels of IgE-antibodies to the mixed of mites D. pteronyssinus and D. farinae (p=0,0000001) after the course of auto-serum therapy. Conclusion. Intradermal auto-serum therapy leads to the decrease of the level of IgE-antibodies in children with atopic BA.

OPTIMIZATION OF RESPIRATORY SUPPORT IN THE ICU FOR ACUTE RESPIRATORY FAILURE
Abstract
The article is devoted to optimization of the respiratory support of patients with acute respiratory distress in the first hours of hospitalization in the intensive care unit and intensive pulmonary therapy profile for gas composition of arterial blood. The study presents two groups of patients: in the first group the respiratory support was administered by standard pulseoximetry (SatO2), and in the second group it was administered only after the analysis of gas composition of arterial blood. The results of own study confirmed that pulseoximetry (SatO2) is not sufficiently informative when choosing a method of respiratory support, and analysis of gas composition of arterial blood will help to optimize the treatment of acute respiratory failure in conditions of resuscitation and intensive therapy.

CHARACTERISTICS OF PATIENTS WITH CHRONIC VIRAL HEPATITIS IN THE SAMARA REGION
Abstract
The purpose of the research is to give description of patients with CVH observed in the Samara regional hepatological centre of Samara State Medical University and to identify groups for antiviral therapy based on its potential effectiveness for known predictors: gender, age, presence of HBeAg, viral load, genotype of the virus. 336 of CVH patients aged 18 to 78 years with disease duration from 3 to 12 years were observed. Examination of patients by the standard criteria for patients with chronic viral hepatitis showed that among patients with chronic viral hepatitis В in Samara region the main group of patients is HBV DNA «+», HBeAg-negative, genotype D. Among 336 patients 65.5% of the observed patients are subject to antiviral therapy, 18.4% of which have high probability of achieving SVR. In the rest 47.1% of patients it is necessary to consider the ratio of cost of treatment, its effectiveness, motivation of the patient that requires an individual approach in each case.

COMPARISON OF BIOMETRICS CORNEA IN PRIMARY OPEN-ANGLE AND PRIMARY ANGLE CLOSURE GLAUCOMA
Abstract
The article is devoted to assessing the state of the central corneal thickness in different forms of primary glaucoma. The results of the study of the central corneal thickness in patients with primary open-angel (POAG) and angel closure (PACG) glaucoma compared with the control group of healthy individuals. According to the study, in the POAG central corneal thickness was significantly smaller than in PACG and in the control group. In the PACG «thin» and «ultrathin» corneas occurred twice rarer and “thick” and “ultrathick” ones occurred almost twice more frequently in comparison with POAG. The research also revealed an inverse relationship of the central corneal thickness values depending on the stage of glaucomatous process.

CONE-BEAM COMPUTED TOMOGRAPHY IN THE DIAGNOSIS AND TREATMENT OF CHRONIC MAXILLARY SINUSITIS
Abstract
CBCT was performed in the treatment of 505 patients with pain syndrome of the maxillofacial area and 906 patients in preparation for surgery. Chronic sinusitis was detected in 418 patients (29.6 %), among which 147 (35.2 %) were diagnosed rhinogenous cause of the pathological process, 218 (52.1 %) - odontogenic, in 53 (12.7 %) the cause could not be determined because of the presence of pathological processes in the nasal cavity and in the teeth area. Sanitation of the maxillary sinus was performed in collaboration with an otolaryngologist, which contributed to the elimination of the inflammatory process at the dental clinic.

COMPUTED TOMOGRAPHY IN DIAGNOSTICS AND TREATMENT PLANNING OF NONCARIOUS LESIONS
Abstract
Cone-beam computed tomography (CBCT) was applied in diagnostics and planning of treatment in 73 patients with noncarious lesions. For the first time, the density of hard tissues was estimated by the Hounsfield scale, which allowed accurate identification of areas of hypermineralization and demineralization. Presented data were used for determination of invasion areas at preparation for the operation. 3-D model and estimate indicators of the lesion area size helped to specify the type, form and rate of the noncarious lesion.

CORRECTION OF RHEOLOGICAL PROPERTIES OF SALIVA IN CHILDREN WITH PATHOLOGY OF THE GASTROINTESTINAL TRACT
Abstract
The paper studies the dental status in children with erosive gastritis and duodenitis, and provides the materials on the detection of changes in oral health in patients with somatic pathology compared to a group of healthy children. It was found that patients with inflammatory and destructive lesions of the stomach and small intestine showed significantly higher performance intensity of dental caries and periodontal tissue destruction. Changes were detected in the rheological characteristics of the saliva. The results of authors’ own studies have confirmed the effectiveness of the developed “Composition for the prevention of dental and periodontal diseases” in changing the surface tension of saliva, and as a consequence - decreasing the reduction of dental caries and the severity of inflammation in periodontal tissues.

The aim of the study was to investigate the stability of the performance of dental implants based on titanium nonwoven material with continuous porosity, set by the traditional technology and the technology of autologous modification. To accomplish the objectives of the study, 19 patients with partial or complete lack of teeth in the lower jaw were operated and 55 implants were installed in the lower jaw. The findings suggest that autologous bone tissue included in the porous material optimizes dental implant osteointegration.

COMPUTER STABILOMETRICS IN THE DIAGNOSIS AND TREATMENT MONITORING OF POSTURAL VIOLATIONS IN ELDERLY PATIENTS
Abstract
Postural violations (PV) is a topical issue for people of advanced age. Studying of features of postural control in elderly people by means of computer stabilometrics (CS) was the research objective. This technique also can serve as control of efficiency of performed therapy of PN. In literature, there are only single researches of change of the main rack in persons older than 60 years. Materials and methods. 138 patients, whose age fluctuated from 60 to 102 years (average 77.8+2.3 years) were examined and divided into three age groups with 46 people in each one: elderly, seniors and long-livers. Results of research testify to marked decrease in compensatory opportunities of maintenance of balance in people over 60; the growth in postural instability was also noted according to the increase in age. Prior to therapy, a significant increase in the area and length of statokineziograms, as well as in the speed of movement of the center of pressure was revealed; average value of the “average position of the center of pressure” parameter in the frontal and sagittal planes considerably differed from normal indicators. After carrying out a standard course of treatment, the average values of the Deviation, Way and Speed parameters decreased in comparison with similar values before treatment. The average value of the Area parameter proved to be the most sensitive to medical manipulations. Conclusion. The CS method allows specialists to carry out rather objective early diagnostics of PN in elderly patients and to evaluate their condition in dynamics.

OPPORTUNITIES AND PROSPECTS OF APPLICATION OF INTRAVENOUS LASER THERAPY IN MILD AND MODERATE VASCULAR COGNITIVE DISORDERS
Abstract
Disorder of cognitive functions is one of the earliest manifestations of chronic cerebrovascular pathology in patients with various cardiovascular diseases. Mild and moderate cognitive disorders worsen quality of patients’ life, influencing their professional activity. However, early diagnostics of cognitive disorder allows its correction and also administration of adequate therapy of the basic disease. Results of research of efficiency and safety of application of intravenous laser therapy at mild and moderate vascular cognitive frustration are presented. The research included 120 patients with mild and moderate vascular cognitive disorders. Among them, 60 patients received standard treatment in combination with intravenous laser therapy, other patients made a group of comparison. It was established that affected by intravenous laser therapy, manifestations of cognitive disorders, first of all mnestic and visuospatial, decreased. Besides, the decrease in aggregation of erythrocytes was observed in the main group of patients.

ANALYSIS RANGE OF PHARMACEUTICAL COMPANIES VOLGA FEDERAL DISTRICT
Abstract
The paper presents an analysis of the assortment policy of the enterprises operating in the sector of pharmaceutical production of the Volga Federal District. In particular, we analyzed the structure of the range, dosage forms and types of dosage forms. It was found that as of 01.01.2015 on the pharmaceutical market of the Volga Federal District (PFD) represented 26 pharmaceutical manufacturers, which produce 693 international nonproprietary names for drugs, which corresponds to the 3100 trade names. In the overall structure of assortment largest share of company-producers of Samara, Penza and Nizhny Novgorod regions. The results of this analysis will be used to develop computer programs and databases «The potential of the pharmaceutical industry PFD».

EVALUATION OF PERSONNEL POTENTIAL IN PHARMACEUTICAL MARKET OF THE VOLGA FEDERAL DISTRICT
Abstract
This paper presents an analysis of the human resource capacity of the pharmaceutical market of the Volga Federal District. It was found out that the problem of staff shortages is one of the urgent problems. The proportion of persons of retirement and pre-retirement age in the retail sector of the pharmaceutical market is about 30%. At the same time, the number of budget places in universities during the last five years has decreased by 37%. In five years about 10 500 pharmacy professionals will come to the pharmaceutical companies of the Volga region. Thus, the shortage of personnel will continue.

PHARMACOGNOSTIC STUDY ON SUBSTANTIATION OF MULTIPLE USING OF SILYBUM MARIANUM (L.) GAERTN
Abstract
In the present paper the results of investigations on substantiation of multiple using of milk thistle [Silybum marianum (L.) Gaertn.] are discussed. As a result of the conducted research, the flagship approaches to produce soluble silymarin were studied. Perspective sorts of milk thistle [Silybum marianum (L.) Gaertn.] with the highest content of flavolignans were examined. The approaches to increase the bioavailability of silymar for inclusion in transdermal dosages forms were considered.

THE STUDY OF PHENOLIC COMPOUNDS OF AERIAL AND UNDERGROUND ORGANS OF POTENTILLA RECTA L
Abstract
The qualitative and quantitative analyses of flavonoids and tannins contained in aerial and underground organs of Potentilla recta L. were done by using of thin layer chromatography and spectrophotometry. The quantitative content of the target biologically active compounds was determined.

CLINICAL ASPECTS AND PECULIARITIES OF IMMUNE HOMEOSTASIS IN SKIN ALLERGIES
Abstract
The article presents the peculiarities of the clinical course of allergic skin diseases. The changes in humoral immunity and in the cytokines in the peripheral blood are revealed. Major changes in quality of life for allergic contact dermatitis, eczema and atopic dermatitis were identified.

DIFFERENTATION OF CD34+ CELLS DERIVED FROM HUMAN CORD BLOOD TO MEGAKARYOCYTES AND PLATELETS EX VIVO
Abstract
Despite of the fact that the basic essentials of thrombocytopoesis are well known, there are still a lot of unanswered questions regarding to the mechanism of this process. And because of the very low frequency of megakaryocytes in bone marrow (<1%), they often need to be produced in cell culture in order to study the process. The development of the approaches for ex vivo production of megakaryocytic progenitors, megakaryocytes and platelets cuts a swath and opens a prospect for their use in clinical practice. This paper examines the protocol of using serum free medium with addition of thrombopoietin, stem cell factor, interleukin-6 and -9 for production of megakaryocytic progenitor and megakaryocytes from CD34+ cells derived from cord blood, which are capable to produce platelets.

ANALYSIS OF COMPOSITION AND ANTIBIOTIC RESISTANCE OF MICROFLORA ISOLATED FROM PATIENTS IN INTENSIVE CARE UNIT
Abstract
This article is devoted to the analysis of the microflora spectrum that was isolated from patients of resuscitation and intensive care unit. The resistance of the main predominant microorganisms to the antimicrobial agents is described. The results of own studies have confirmed the need for the dynamic monitoring of the microbial scene in the hospital. The basic directions for prophylaxis and management of organizational work in the workplace in a laboratory are proposed.

APPROACHES TO IMPROVEMENT OF QUALITY OF MEDICAL REHABILITATION: EXPERIENCE OF THE CITY CENTER FOR MEDICAL REHABILITATION OF SAMARA CITY POLICLINIC №6
Abstract
The article describes the algorithm of rendering services in medical rehabilitation and experience of the city center for medical rehabilitation of Samara city policlinic №6. The analysis of patients’ satisfaction showed that all the patients were contented with quality of services in medical rehabilitation by the results of questioning of 100 patients treated in the Center for 6 months in 2015. Conclusion: presented approaches to improvement of quality management in the city center for medical rehabilitation increased the quality of medical rehabilitation, overall performance and satisfaction of consumers (the personnel and patients).

SOME ASPECTS OF HEALTH OF THE POPULATION
Abstract
The article presents the scientific assessment of the health of the population of Zhigulyovsk in Samara region with the medical and demographic aspects and morbidity statistics for the period of 2004-2013. The results of the study revealed population decline, largely due to the decreased inflow of migration; slowing down of the natural population decline in conditions of rising fertility and decreasing mortality; growth of the revealed pathology at high appealability of the population in health care organizations. The identified patterns allow using of the data as a basis for management decisions in the field of public health.

MODERN ASPECTS OF MORTALITY AMONG RURAL POPULATION
Abstract
The paper presents the scientific assessment of mortality in the Volga rural district of Samara region for the period 2009-2013. There is a decrease in total mortality of the population against the background of stable fertility rates, resulting in a small natural population loss. The mortality rate from diseases of the circulatory system is decreasing, and mortality from neoplasms and injuries is increasing. For purpose of further mortality reduction, a set of measures, including preventive measures aimed at eliminating the risk of premature mortality, is required.

THE EXPERIENCE OF BINARY LECTURES BY THE HEADS OF NURSING SERVICES IN HOSPITALS
Abstract
The article reviews the experience of the binary lectures by chief and senior nurses in hospitals. During a conversation, two lecturers demonstrate the interdisciplinary relationship between the problems of psychology and office management, transferring knowledge on the basis of integration; they show the development and improvement of intellectual, creative and communicative abilities of nursing managers. The study focuses on the principles of andragogy in the process of conducting lectures by two lecturers, including 3 simulated production situations from real practice of managers of nursing services.

ORGANIZATION OF EXPERTISE OF TEMPORARY DISABILITY IN CITY HOSPITAL
Abstract
In order to improve the quality of medical care in a City Hospital, we studied the system of organization of expertise of temporary disability, which has a two-level structure that includes attending doctors and medical commission under the supervision of the heads of departments and deputy chief physician of the hospital. Despite the problems in health care system, the majority of patients and physicians are satisfied with the quality of care and solution of issues during the examination of temporary disability. Based on the survey and testing of physicians, organizational and management problems were identified, as well as difficulties in the organization of information support services during the examination of temporary disability.
