Abstract

The article reviews the up-to-date views on the role of endothelial dysfunction in the development of hemorrhagic syndrome in immune thrombocytopenic purpura. Metabolic base of clinical manifestations in the patients with psoriasis

Abstract

The assessment of cellular composition of blood and metabolism in the patients with varying severity psoriasis is carried out. 52 patients are included in the research with average and heavy severity of psoriasis. Laboratory criteria of weight are revealed: the decrease of the contents osteocalcin and level increase parathyroid hormone, increase in quantity of leukocytes, speeds of subsidence of erythrocytes, the increase of concentration of hemoglobin in an erythrocyte. In the patients with psoriasis in the process of the increase of severity of a skin syndrome changes of system character accrue.

Hemostatic system state in hemorrhagic vasculitis and genetic polymorphism: is there any connection?

Abstract

The published data on the changes of the hemostatic system in hemorrhagic vasculitis are surveyed. The effect of mutations of genes primarily responsible for hyperhomocysteinemia, on the course of hemorrhagic vasculitis is pathogenetically proved. Preliminary results of the research and MTHFR gene mutations MTRR in patients with this disease are shown. It was noted that at present there is no clear evidence of a statistically significant effect of mutations of the genes controlling the hemostatic system, on the course of hemorrhagic vasculitis, and the study of this question seems to be very promising.

Characteristics of liver fibrosis stages in patients with HCV-infection based on the instrumental and histological research

Abstract

The work purpose is to compare data on a stage of liver fibrosis in patients with HCV-infection, obtained as a result of the ultrasonic and histological research, to give the comparative analysis of distribution of the patients depending on a stage of liver fibrosis treated during 2005-2012. In addition to directly measured ultrasonic structure parameters liver and spleen hemodynamics with calculated indicators were obtained: the portion of the evaluated section of liver and spleen lobes, and also their ratio. It is shown that the ultrasonic research executed on the modern equipment, including a dopplerography, in most cases (70–80%) allows specifying the degree of liver fibrosis without resorting to the histological verification.

The comparative characteristic of the calculated method of estimated glomerular filtration rate in senile patients with chronic kidney disease

The examination of 71 patients ranged from 75 to 90 years in age (mean age 83,3 ± 4,4 years) with chronic kidney disease (CKD) to compare the calculated methods of estimated glomerular filtration rate (GFR) from serum creatinine and cystatin C, and comparison performance indicators of the level of GFR with erythropoietin (EPO), and intact parathyroid hormone (iPTH). The data on the over-counting of the estimation of GFR from cystatin C with GFR above 60 ml/min/1, 73 m ² and underreporting of the estimation GFR by Cockcroft-Gault equation for creatinine clearance, especially at values below 45 ml/min/1,73 m².

In the article the cooperation of patients with bronchial asthma (BA) at various levels of control achievement is examined. Indicators of cooperation are estimated using the questionnaire ICBA - 50. 212 patients with bronchial asthma have been included in the research. All the patients have been divided into two groups depending on the tool of an estimation of control (GINA and ACQ-5). It has been established that the cooperation does not depend on tools of an estimation of control BA (GINA and ACQ-5), the level of its achievement and makes 62 %. The obtained data can improve the treatment BA efficiency.

Dynamics of full recovery of swallowing function in patients with acute ischemic stroke undergone neuromuscular electrical stimulation

Abstract

Dysphagia in acute stroke occurs in 20-90% of patients. The results of the examination and treatment of 112 patients with acute ischemic stroke are presented. The main group (group I, n = 55) received standard therapy of dysphagia and the patients of comparison group (group II, n = 57) - sessions of neuromuscular electrical stimulation. Full recovery (total score on a scale of FOIS ≥ 6) observed in 22 (38.5%) patients in Group I and only in 9 (16.3%) in group II (χ2 Yates correction = 5.85, p = 0.0156). After the treatment only 2 patients (3.5%) of group I had a nasogastric tubes and 10 patients in group II (18,2%) (χ2 Yates correction = 4.86, p = 0.0275). Aspiration pneumonia occurred in 3 (5.3%) patients in group I and in 11 (20.0%) patients in group II (χ2 Yates correction = 4.29, p = 0.0383). The factors of a poor prognosis for recovery of swallowing function were detected. A statistically significant correlation between the duration of submental muscle contraction group and on a scale MASA (r = -0,63, p <0,05) was revealed.

The results show that the development of connective tissue dysplasia may be related to the destabilization of the system of exchange of magnesium and hydroxyproline.

Efficiency estimation of partial pressure of oxygen in venous blood in the process of gestation treatment in the second trimester of pregnancy

Abstract

The level of partial pressure of oxygen in venous blood of 25 women with uncomplicated gestation and 70 patients with pregnancy complicated by preeclampsia, in the process of treatment of this complication of gestation in the second trimester of pregnancy was investigated. In the treatment of preeclampsia in 46 pregnant women from the main group the decline of the partial pressure of oxygen in the venous blood was noted, it was accompanied by a decrease in symptoms of this complication and prolongation of gestation, and 24 women from the main group there was an increase in the partial pressure of oxygen in the venous blood, and the severity of preeclampsia grew, followed by early abdominal delivery caused by the lack of proper treatment effect on pregnancy complications.

The influence of latent iron deficiency in women with infertility on the pregnancy frequency in the IVF programs

Abstract

This prospective study evaluated 125 women, commencing IVF. All women were screened for latent iron deficiency. Depending on the results of the survey, the women were divided into two groups - with and without latent iron deficiency. The clinical pregnancy rate (17% versus 32%) was significantly lower in women with latent iron deficiency. This paper suggests that a common condition known as latent iron deficiency is associated with a reduced success following fertility treatment such as IVF.

Treatment result dynamics in patients with hernias depending on an operation way

Abstract

The assessment of the plastics in patients with postoperative ventral hernias (PVG) is of great practical importance. The purpose of the study is to analyse and evaluate the dynamics of the historical aspect of the treatment results of the patients with hernias in the transition from tension to non-pull way of close abdominal wall defects. Ways of operating applied in patients with hernias, for the time period were significantly different. Until 1998, the foundation of the constituent tension by means of plastic suggested by Mayo, Sapezhko and local fabrics were basic. Non-pull combined methods of hernia repair with the use of synthetic grafts, sewn on the first and second embodiments, have been introduced since 1998. The change in the treatment approach to the patients with PVG reduced the number of the lack of operation possibility for the sick persons, for technical reasons, regardless of their size from 24.5% to 8.4%, and the complications arising in the postoperative period in 2.7% of patients with large hernias and giant sizes were comparable to those in patients with hernia of small size. Plastic of hernial ring by local tissues was believed to be possible only in patients with hernia of small size, regardless of their location.

A new way of getting the bone tumors biopsy

In the course of the experiments of in-vitro the biopsy of bone tumors device (the patent of the Russian Federation № 109395, 2011) was developed. The purpose of the device creation is to get non-deformed column of biopsy material without damaging the layers of layers differentiation of in-vivo fabrics that increases reliability of the morphological diagnosis. On the basis of the office of the general oncology of the Samara Regional Clinical Oncological Clinic 42 biopsies were carried out by this developed way. Diagnostic accuracy of the clinical diagnosis making was 97,6%.

Cytotoxic treatment complications in the patients with lung cancer

Abstract

Lung cancer - the leading cause of cancer mortality in most developed countries. Despite of the extensive development of the prevention, diagnosis and treatment, lung cancer has the highest incidence of all cancers among men and is the consistent upward trend in the incidence among women. Chemotherapy is one of the main methods of the treatment required for the most patients with lung cancer. A side effect of cytotoxic drugs is often limited. For the majority of side effects of anticancer agents corresponding grade 3 toxicity, and more, is an indication to stop chemotherapy and requires intensive treatment cover. Resuming chemotherapy depends on the completeness and rate of elimination of side effects and complications, which in turn, worsens the prognosis.

The author discussed questions of radionuclide study of morphological and functional state of kidney transplantation to the patient. Diagnostic opportunities of renal scintigraphy for such cases haven’t been fully investigated yet. This particular study determined the effect of focal lesions of the parenchyma kidney transplantation on the functional state of renal allograft using scintigraphy dynamic. Scintigram data handling and interpreting are presented by the author’s best knowledge. The obtained results show great diagnostic value of renal scintigraphy.

Terms of restoration of the regional blood flow after successful vascular reconstruction in patients with lower limb ischaemia

Terms of restoration of the regional blood flow after successful vascular reconstruction of 128 patients with lower limb ischaemia and necrosis of the distal portions of the foot were assessed. The ankle brachial index was revealed to be maximum to the end of 10th days at 76,5% cases after lower limb revascularization. Thus, any aggression on tissues of the distal portions of lower limbs performed before this time does not seem to be appropriate and can be even dangerous.

Refraction features in patients with open-angle glaucoma

Abstract

We examined 110 patients with open-angle glaucoma in total 214 eyes. All the patients with an average age of 58.3 underwent refractometry to objectively determine the presence and degree of astigmatism. The degree of astigmatism was found to be more or equal to 0,5 D in 73% (158 eyes), with more or equal to 1.5 D in 17% (36 eyes). The prevalence of astigmatism in the patients with impaired hydrodynamics was revealed to be higher than in the general population. Astigmatism more than or equal 0.5D was accordingly observed almost 2 times more, and astigmatism more than or equal to 1.5 D was almost 4 times. By taking into consideration the association of optical, accommodative and simulation systems we can assume that the astigmatism can be one of the factors in the pathogenesis of open-angle glaucoma.

Two-stage surgery treatment of patients with macular holes and cataracts

Abstract

In the article features and results of the surgical treatment of patients with macular holes and cataracts with the application of two-step surgery are studied. The first stage, phacoemulsification with IOL implantation was performed to all the patients. The second stage, microinvasive 25+ Gauge chromovitrectomy with application of endovitreal dye «Brilliant Blue G» on deuterium water was performed. Such a two-step concept allows the doctors to achieve high anatomic and functional results in treatment of macular holes and cataract in the early and follow up period of supervision. Intraoperative and postoperative complications are studied with anatomic and functional results.

Concentration of protein S100B in patients with fat embolism syndrom

Abstract

Fat embolism syndrome (FES), a complication associated with fractures of long tubular bones or pelvis bones, is discussed. Classically, the clinical picture of FES develops after the «light interval» and followed by lung and neurological symptoms in combination with pin point bleeding. The diagnosis of FES is usually made on the base of clinical symptoms, and the set of large and small diagnostic criteria described by A.R. Gurd (1970) is used. Laboratory and instrumental techniques are used as well to confirm the clinical diagnosis or to monitor the therapy. In present study, we investigate a concentration of protein S100B in blood serum of the patients with acute trauma. An increase in the concentration of S100B is noted in patients with FES already in the first day of the post-traumatic period. The obtained data indicate a correlation between the concentration of S100B protein in blood serum and the severity of central nervous system defeat associated with FES.

Endothelium dysfunction at an experimental electrotrauma

Abstract

Some indicators of endothelium dysfunction at 20 laboratory rats under experiment with electrotrauma were investigated. By means of a simulated device the immobilized animals under anesthetic were run the risk of a dosed nonlethal electrotrauma with current tension of 220 V, current frequency of 50 Hz within 2 seconds. It was stated, that at electrotrauma the number of desquamative endotheliocytes increased, the production of nitric oxide decreased. Thus, the influence of current caused evident violations in endothelium function.

Complex use perspectives of Camelina silvestris L

Abstract

The optimum extraction conditions of Camelina silvestris L. seeds were studied. The method of quantitative determination of flavonoids in the Camelina silvestris L. seeds was developed. For the first time from the Camelina silvestris L. seeds there was isolated 3-O-rutinoside of the isorhamnetin (narcissin). By having used of gas сhromatography there was studied fatty acid composition of fatty oil of Camelina silvestris L. seeds, cultivated in the Samara region. The dominant fatty acids, namely linolenic (30.7%), linoleic (20.6%), oleinic acid (17.6%) were identified. For the first time in the fatty oil of Camelina silvestris L. seeds there were found eicosadienic, docosadienic and selaсhoevic acids.

Morphologic diagnostics of prostate ductal adenocarcinoma

Abstract

The morphologic features of tumors in patients with locally confined or advanced prostate ductal adenocarcinoma who had undergone radical prostatectomy were evaluated. Volumes of prostate gland, volumes of tumors, Gleason scores, pathologic stages, PSA levels were examined. We identified patients who had histologically pure ductal adenocarcinoma (18%), predominantly ductal adenocarcinoma (32%), whereas the other 50% had mixed ductal and acinar prostate cancer. 15 patients of the mixed pathology group also had evidence of neuroendocrine, sarcomatoid, mucinus or squamous differentiation. Differential diagnosis must exclude high grade prostatic intraepithelial neoplasia, non-ductal intraductal adenocarcinoma, cribriform acinar adenocarcinoma.

Abstract

By having used of the method of a gas chromatography there was studied the composition of fatty oil of Carthamus tinctorius L. seed’s cultivated in the Samara region. In the content of seed oil of this plant there were determined 10 fatty acids. The main fatty acid of the Carthamus tinctorius L. seed oils is linoleic acid (78,5%) was shown. By having used of spectrophotometry there was determined in the Carthamus tinctorius L. seed oils the content of total carotenoids which was 1,40 ± 0,02 mg%. Basic physical-chemical constants for Carthamus tinctorius L. seed oils were studied.
