Патогенетические механизмы антиноцицептивной активности антидепрессантов

Аннотация
В статье предпринята попытка обобщить известные на сегодняшний день патогенетические механизмы антиноцицептивных эффектов наиболее широко применяемых антидепрессантов, осветить данные, полученные при изучении экспериментальных моделей различных болевых расстройств на лабораторных животных, и результаты клинических исследований в группах пациентов как с коморбидной болевой симптоматикой, так и с болевой симптоматикой различного генеза без наличия выраженных аффективных расстройств. Приведены результаты доказательных исследований противоболевой активности ТЦА, СИОЗС, СИОЗСН и препаратов с особым рецепторным механизмом действия. Рассмотрено участие различных нейротрасмиттеров – серотонина, дофамина, норадреналина, мелатонина, энкефалинов, ГАМК, глутамата, аденозина и пр. – в реализации противоболевого эффекта антидепрессантов. Освещены наиболее распространенные побочные эффекты антидепрессантов указанных групп, которые являются частой причиной отказа пациентов от лечения. С учетом рассмотренных особенностей терапии коморбидных пациентов с болевым синдромом, даны рекомендации по выбору препарата не только согласно выраженности его антидепрессивного и антиноцицептивного действий, но и возможного развития нежелательных явлений долгосрочной терапии. Особого внимания в данном аспекте заслуживают новые группы антидепрессантов с альтернативным механизмом действия.
