Thе article summarizes some results of work of the Southern Regional Medical Centre FMBA of Russia, analyses the results of the carried-out work for organizational structure improvement, restructuring of hospital bedspace and increasing the efficiency of treatment process. Examines the achieved indicators of access to health care, as well as forms and methods to enhance management skills of senior management. Much attention is paid to the basic directions of information technologies use in practical activities of medical organization.

The article summarizes the results of the implementation in the Southern Regional Medical Center FMBA of Russia one of the directions of the Programme for modernization of the healthcare system - information technology. Considered the basic directions of the Programme implementation and methods for their realization.The conclusion is that the use of information technologies is absolutely necessary for fully-fledged activities of any medical organization.

Occlusion of terminal aorta is mostly often caused by atherosclerosis and the development of lower limb ischemia requires prompt treatment. In addition to the open aortic surgery recanalization and stenting can also be used. The article presents the first experience of endovascular treatment in patients with good immediate and mid-term results. This method is suitable to use in patients with contraindications to general anesthesia.

The aim of the article is to review the results of regional lower limb haemodynamics, haemostatic disturbances in patients undergoing laparoscopic surgery for hiatal hernia and intermittent pneumocompression in terms of embolism prevention. The impact of laparoscopic surgical technique on linear bloodflow velocity and venous diameter in lower limbs.

Introduction. Quality of life and survival rate of post-cystectomy patients depend on the urinary diversion methods [1,2].Purpose of the study. A comparison of morbidity, survival rate, and quality of life in patients after heterotopic continent or incontinent urinary diversions.Materials and methods. During the period from 1998 to 2016, 36 heterotopic intestinal diversions were performed using urine containment method according to the Kock pouch scheme in our modification, and 61 incontinent heterotopic intestinal Bricker diversions. We evaluated the comorbid conditions (Charlson comorbidity index), postoperative complications (acc. to Clavien-Dindo), survival rate (acc. to E.L. Kaplan, P. Meier), and the quality of life of patients (SF-36) using complex statistical analysis.Results. Early and late complications not related and related to urinary diversion prevailed in the group of patients after Bricker surgery without statistical significance. With longer postoperative monitoring periods the share of complications related to urinary diversion increases, while the frequency of delayed complications and survival rate show no statistical difference in both groups. No reliable differences were revealed in the quality of life in all cases of cutaneous urinary diversion.Discussion. 80% of patients after the Kock pouch surgery (mod.) throughout the medium follow-up period of over 5 years remain continent and enjoy high quality of life. It is necessary to develop and implement in clinical practice new and more advanced statistical tools for evaluation of the patients’ quality of life depending upon the type of urinary diversion after cystectomy.Conclusion. Heterotopic continent diversion (Kock pouch (mod.)) in comparison with the incontinent method (Bricker) does not increase morbidity or decrease the survival rate, and provides for higher quality of life.

Presents a clinical case of successful surgical treatment of kidney cancer with tumor thrombus of the inferior vena cava (IVC) to the level of the atrium, with simultaneous resection of segment I of the liver containing a single metastasis, in 57-year patient.

Analysing the example of unusual case of a successful direct constant His bundle pacing, we would like to draw specialist’s attention to details of procedure and some important electrophysiological particularities we have come across during operation. Long-term (7 years) results ( pacing options, ECG data, changes of left ventricle ejection fraction and patient’s quality of life) were considered too.
