ASSOCIATION OF TNF AND LTA GENES WITH ATHEROSCLEROSIS COMPLICATIONSIN PATIENTS WITH HISTORY OF ACUTE CORONARY SYNDROME

Abstract

The aim of this study was to investigate an association of polymorphic markers G(-308)A of TNF gene and Thr26Asn of LTA gene with the frequency of poor outcomes in patients with the history of acute coronary syndrome.Methods. A total of 1145 patients admitted to cardiological hospitals of Moscow, St. Petersburg, Kazan, Chelyabinsk, Perm, Stavropol, and Rostov-on-Don with ischemic heart disease exacerbation were examined. The maximum follow up time was 3.2 years. The identification of polymorphic marker allele was carried out by hybridization-fluorescent analysis using real-time polymerase chain reaction.Results. In case of Thr26Asn polymorphic marker of LTA gene we have not found any association with the frequency of poor outcomes in patients with the history of acute coronary syndrome. However, in case of G(-308)A polymorphic marker of TNF gene we have found the reliable association. The carriers of GA and AA genotypes has higher frequency of poor outcomes in comparison with the carriers of GG genotypes. The survival time to the endpoint for carriers of the GA and AA genotypes was 43.3 months (95% CI = 40.04 - 46.56) vs. 49.6 months (95% CI = 47.38 - 51.82) for carriers of theGG genotype (χ2 = 15.4; р < 0.001).The results of our study allow to make a conclusion that the G(-308)A polymorphic marker of TNF gene is significantly associated with hereditary predisposition to unfavourable outcome in patients with history of acute coronary syndrome.

Abstract

The results of the clinical trial of a new air cleaning method in the breathing zone of the patient with asthma exacerbation are presented. It is shown that in patients treated with additional tech- nology Protexo®, the bronchodilators usage and dispnoe score significantly reduced. Significant effects on respiratory parameters has not been established.

EFFICIENCY OF PLASMASORBTION IN PATIENTS WITH ENDOTHELIAL DYSFUNCTION WITH CHRONIC KIDNEY DISEASE

Abstract

The influence of plasmasorbtion on vasodilatation activity of blood vessels in 85 patients with chronic kidney disease compared with therapy by inhibitors of angiothensine-converting enzyme was studied. It is proved the efficiency of plasmasorbtion on normalization of endothelinum-dependent vasodilatation in 74% patients and by lowing concentration of endothelinum-1 on 59±7,9% by the end of first month of trial. We noticed that therapy with inhibitors of angiohtensine-converting enzyme allowed to reach that level of endothelinum-1 only by the 6th month of trial. Normalization of endothelium functions can afford to decrease the level of daily proteinuria (p<0,05) and to increase the speed of glomerular filtration within 1 year by 7,6±1,5 ml/min in patients with I-II stage of chronic kidney disease and by 2,2±0,6 ml/min in patients with III-IV stage of chronic kidney disease.

Abstract

A photosensitizer from polycationic metallophthalocyanines class is shown to have a high antimicrobial activity and a wide antimicrobial spectrum. It is established that the effective pho- todynamic inactivation of bacteria in the presence of photosensitizer of polycationic nature is determined by its electrostatic binding to negatively charged structures of bacterial cell walls. The usage of antimicrobial photodynamic therapy in medical practice is discussed from the point of view of the obtained results.

Abstract

Due to recent advances in minimally invasive surgery, there is a new ablation technique – the irre-versible electroporation (IRE), based on the effect of electric fields. This report describes the firstcase of colorectal liver metastases irreversible electroporation in Russian Federation. This early analysis of IRE treatment of hepatic tumors demonstrates safety for treating liver malignancies. Larger studies and longer follow-up are necessary to determine long-term efficacy.

Repair techniques based on direct cartilage substitution (mosaicplasty, osteochondral allotransplantation, minced cartilage autotransplantation in gel), bone morrow stimulation techniques (abrasion, drilling, microfracture, matrix-induced chondrogenesis) were characterized in terms of biology and clinics.
