THE AFFECT OF INFLUENCE OF GENES' POLYMORPHISMS CYP2C9 AND VKORC1ON THE SAFETY OF THE THERAPY BY WARFARIN

Abstract

To study the distribution of alleles and genotypes of polymorphic markers of genes CYP2C9 and VKORC1 of Russian patients who live in Moscow, and in order to assess the influence of genetic factors on warfarin therapy 400 patients have been genotyped. The dosage of warfarin which is required for achievement of INR target values has been different among owners of different geno- types of polymorphic markers of genes CYP2C9 . Meanwhile the highest average dose has been required for genotype *1/*1 and the lowest – for owners of alleles *2 and *3 . For polymorphism G(- 1639)A of the gene VKORC1 the dosage of warfarin which is required for achievement of the INR target values, has been different among owners of different genotypes. The highest average dose has been required for genotype GG, and the lowest – for genotype AA. The results will allow to work out more accurate algorithm of choosing of the initial dose of warfarin depending on the genotypes of polymorphic markers of genes CYP2C9 and VKORC1.

It is shown that in patients with anxiety, depressive symptoms and emotional factors as triggers of disease exacerbation, inclusion of antidepressants in the complex treatment of acute back pain greatly increases the efficiency and reduces the time of treatment

Abstract

Discussions so far continue about optimal terms and methods for plastic operations on face and neck after burn injuries that is necessary for reduction the time of medical rehabilitation of burned patients. In the given work authors' view at this problem based on the experience of treatment of more than 800 patients is presented. The expediency of correct timing of plastic operations and features of techniques is proved for achievement of optimal aesthetic and functional results.

Abstract

We presented the case history of successful surgical treatment of patient with aortic arch aneurysm, thoracoabdominal aortic aneurysm and stenosis of brachiocephalic arteries. She was underwent hybrid reconstruction (open and endovascular intervention): open reconstruction of both carotid bifurcations and the right subclavian artery and arc and descending thoracic aortic endoprosthesis.

Abstract

The present report describes a 40-year old female in whom CT showed tumor of the right lung with bronchus compression. VTS showed tumor of the right lung with pericardium involvement but VTS biopsy could not identify the type of tumor. The patient underwent right pneumonectomy with pericardium resection. The diagnosis of a rare lung disease – primary leiomyoma of lower respiratory tract was confirmed only by immunohistochemistry.

STEM CELL THERAPY OF MALIGNANT BRAIN TUMORS: REALITY AND PROSPECTS

Abstract

Modern methods for the treatment of malignant brain tumors are insufficiently effective. One reason for this is that the existing technologies and methods are focused on removing all neoplastic cellsfrom the body. Understanding the mechanisms of systemic migration of stem cells provides a new view on the role of this phenomenon in the development of malignant tumors. Migration and homing of normal stem cells, being originally the regulatory process, ensuring revascularization and remodeling of ischemic or traumatic injury of brain, play a role of the axial conductor of neoplastic process in carcinogenesis. The use of the phenomenon of migration and homing of stem cells in the tumor center for therapeutic purposes opens the possibility of overcoming the blood-brain barrier, reducing the toxicity of chemotherapy and increasing the radiation therapy efficiency, makes possible the directed influence on the hypoxic zone of the tumor, can directly affect to the key life processes of tumor stem cells. These arguments allow to consider the mechanisms of systemic migration and homing of stem cells to neoplastic foci as a fundamental theoretical platform for the creation of a fundamentally new class of anti-cancer, cell personalized medicines.

THE PROBLEMS OF THEORETIC NEUROLOGY: INFORMATION-COMMUTATIVE THEORY OF HUMAN BRAIN AND PRINCIPLES OF ITS FUNCTIONING

Abstract

The article summarizes contemporary scientific concepts of brain organization and structure. For the first time we developed information principles of brain functioning and the basics of information communicative theory of brain. Here we demonstrate that information commutative organization (ICO) of brain has multilevel structure of open type represented by the block of information capture and transfer (BICT) and the block of information processing (BIP). BICT is a switchboard with information dispatcher and router in nervous tissue (NT). BICT has system vertical and complex horizontal commutation. Brain ICPs are presented as interthecal information registers (ITIRs) that provide extraneural information processing in subarachnoid space (SAS) and subdural space (SDS) above brain. The arcs of conditioned and unconditioned reflexes close in SAS extraneurally when electro-magnetic waves (EMWs) of cortical ICM subscribers are reflected from the arachnoid mater and information is automatically sent to ICM receivers of brain cortex (reflectory ITIRs). Information processing (analysis, synthesis, calculations) is based on dissipation of EMWs of cortical NT ICMs in SAS and SDS cerebrospinal fluid (CSF) forming support and object EMWs, which, being reflected from the arachnoid or dura mater form holograms of information images matrixes (IIMs) by interference, diffraction and superimposition while IIMs are the basics of brain cognitive functions (cognitive ITIRs). The theory of brain can become a new milestone in the development of therapies of nervous diseases, neuromorphic computation, innovative systems of artificial intellect and novel approaches to brain-computer interface.
