Challenges and perspeсtives of aesthetiс surgery of low limbs

Abstract

The article is on poorly studied topic of surgical leg malalignment correction for aesthetic indications. These operations must be carried out efficiently, with minimal complications.The article is a review of publications, demonstrates the possibilities of current techniques of correction and outlines the indications for a particular type of surgical correction. The article presents the descriptive clinical cases of aesthetic results after varus deformity correction of the lower limbs.The purpose of this study was to evaluate the possibilities of modern types of external osteosynthesis in the leg shape correction according of the aesthetic indications. In the period from 1996 to 2014 was carried out more than 800 operations on both legs in more than 400 patients. In the majority of cases was received good and excellent results, but met serious complications - inflammation, osteomyelitis, delayed fusion. The resulting complications forced to seek new methods of correcting the shape of the feet. The use of internal fixation seems very perspective.

ILIZAROV FRAME FOR FINAL TREATMENT OF SEVERE ANKLE FRACTURES

Abstract

This article is about treatment of severe ankle injuries.Significant incidence of fractures of this localization and the problem of treatment of severe soft tissue damages make the work relevant. Despite advantages of internal fixation external osteosynthesis is often the only possible solution to a severe clinical situation.A significant number of patients justify rationale for the Ilizarov frame as the primary method of treatment. The most common technical problems and complications in the treatment of fractures the optimal starting date of treatment are analyzed. The criteria of satisfactory and unsatisfactory reduction, indications to a change of fixation for internal one are described. On clinical cases the authors show good results and alternative assembles of external devices.A feature of the work is its practical value and authors’ recommendations.

THE ULTRASTRUCTURE OF BIODEGRADABLE IMPLANTS: CHANGES DURING THE TIME

Abstract

The authors describe in detail the biochemical processes that occur over time and with the collapse of the polymer in the bone. Originally used scanning electron microscopy to assess the condition of clamps, which spent several years in the bone. The data obtained is objective evidence that the biological activity of the implant in the bone and again emphasize the importance of the choice of materials to be implanted in the human body.

OSTEOSYNTHESIS OF CLAVICLE AFTER FRACTURE AND FRACTURE-DISLOCATION

Abstract

The problem of damage of clavicle and its ligamentous apparatus remains relevant because of the high frequency of these lesions, especially in young and active patients. Despite advances in science technology is a high frequency of unsatisfactory results. Authors, on the large clinical material, have analyzed the results of treatment of dislocations and fractures of the clavicle.Together with the national scientific production company «DEOST» the authors have developed and introduced into clinical practice new anatomically curved clavicular plate with monolithic middle part and cortical screws with low profile heads for osteosynthesis of the clavicle. Based on the analysis of 132 patients` treatment results, the authors concluded that osteosynthesis with curved plate, intramedullary pin Roskwood and hook plate for LCP clavicle gives the best results in complex fractures and fracture-dislocation of the clavicle.

Abstract

Successful use in over 10 years of cylindrical titanium cages after anterior decompression for degenerative diseases of the lumbar spine, and the lack of justification for its use, led us to carry out mathematical modeling using finite element methods. For 10 years we have operated on more than 1200 patients with a primary spinal canal stenosis caused by disc herniation, hypertrophy of the elements of the spine, and at reoperation for failed back surgery syndrome.

In the article authors describe the basic pathogenetic mechanisms of development of osteoarthritis, a modern approach to the treatment of this group of diseases. Outlined the problems of treatment of coxarthrosis, gonarthrosis in elderly patients with the presence of concomitant somatic pathology. The study used a unique method for estimating motion in gait. Held objectification of treatment results in aged patients with knee joint hyaluronic acid.

TWO-STEP SURGICAL CORRECTION OF IDIOPATIC SCOLIOSIS 3-4 OF GRADE

Abstract

We report the results of two-step surgical correction of 75 patients with scoliotic deformation of vertebral columns. The first step is surgical correction by expanding endocorrector. In the second step expanding endocorrector replaced by stable screwed system. Our method offered the possibility of primary surgical correction of scoliosis in the period of rapid growth without enbarrassing the growth of the vertebrum. Consequently, the method allowed escaping the disease progression, severe vertebral columns deformation and failure of compensation. By the second step we perform additional correction and reducing the intraoperative trauma.

From 2004 to 2014 there were conducted more than 400 operations with the use of ceramic- polyethylene prostheses in the Hospital №13 (clinical base of the Department of Traumatology and Orthopedics of Peoples’ Friendship University of Russia). In any case there were no signs of aseptic loosening of prosthesis components. Nowadays the use of low friction ceramic-polyethylene friction pair in hip arthroplasty is widespread for most patients with hip joint pathology in all age groups.
