分娩性臂丛神经损伤和先天性多关节畸形伴上肢功能障碍儿童的语言治疗和教育分析

论证。伴有上肢受累的肌肉骨骼系统病理往往是导致孩子在不同学习阶段出现特定困难的因素之一。患有先天性多发性关节发育不全和分娩性臂丛神经损伤的3岁以下儿童通常语言发育迟缓。还可能出现行为障碍,这在幼年时期就已决定了需要进行矫正,包括语言治疗。

目的。本研究主要分析分娩性臂丛神经损伤和先天性多发性关节发育不良伴上肢功能障碍儿童的语言治疗和教育。

材料与方法。2020年至2022年,H. Turner国家儿童矫形和创伤外科医学研究中心对46名6至15岁因先天性多发性关节发育不良和分娩性臂丛神经损伤导致上肢畸形的儿童(平均年龄为9.57±2.91岁)进行了检查。研究采用了言语医学、矫形医学和统计学方法。

结果。本研究包括25名先天性多关节畸形患儿(54.3%)和21名分娩性臂丛神经损伤后遗症患者(45.7%)。上肢功能障碍从轻度到重度不等,其中以轻度(17名患者,占36.9%)和中度(20名患者,占43.5%)为主。30人(65.2%)发现手部功能障碍。11名儿童(23.9%)出现早期语言发育迟缓。在41名儿童(89.1%)中发现了语言病理现象,其中包括一般发育不全(20名儿童,43.9%)、词汇-语法发育不全(12人,26.1%)和语音-音位发育不全(9名患者,19.8%)。25名不同年龄组的患者(54.3%)被诊断出构音障碍。18名学生(52.9%)出现了书写语言障碍。根据言语病理学的性质,先天性多发性关节发育不良患者组和分娩性臂丛神经损伤患者组之间的差异无统计学意义(p>0.05)。与分娩性臂丛神经损伤的患者相比,先天性多发性关节发育不良的患者更容易进行个体化训练或无组织训练(p<0.05)。

结论。患有先天性多发性关节发育不良和分娩性臂丛神经损伤的儿童和青少年有特殊的教育需求。此外,考虑到矫形病理学的特殊性,他们往往需要语言治疗。
