青少年儿童关节镜下前交叉韧带重建技术的比较分析

论证:国内外文献中有很多矛盾的信息,在其他条件相同的情况下,解剖重建和胫骨重建前交叉韧带都可以提供良好的功能治疗结果。然而,前交叉韧带重建的个别方法的比较效果和前景仍然是非常相关的课题。需要分析解剖技术重建前交叉韧带的可能性和优势,这决定了本研究的目的。
目的比较不同方法重建青少年前交叉韧带的效果。
材料与方法。分析了94例青少年前交叉韧带关节镜重建的结果。所有的病人被分成三组。第一组
(32例—34%)采用前交叉韧带等长整形手术,其中采用经胫骨隧道技术形成股管。第二组患者
(30例—32%)通过关节前内侧入路进行前交叉韧带解剖整形手术,并形成股管。第三组(32例—34%)
行前交叉韧带重建采用全关节腔内(all inside)的技术。
结果。通过对青少年儿童前交叉韧带重建结果的对比分析,发现第三组患者膝关节的正位和旋转稳定性优于第一组和第二组患者。第三组的患者进行轴移试验(Pivot-Shift Test)阳性的可能性显著
降低(0度—87.5%;I+度—12.5%)较第一组(0度—46.8%;I+度—25%;II+度—21.9%;III+度—6.3%)
和第二组的患者(0度—86.6%;I+度—6.7%;II+度—6.7%)。
结论。采用全关节腔内重建前交叉韧带,对关节内结构安全,股骨管解剖定位更理想,对膝关节术后正位及旋转稳定性有较好的效果。
