自然唇腭裂儿童康复治疗最后阶段旨在使颌骨咬合关系正常化的骨重建手术分析(文献综述)

论证16-18岁先天性唇腭裂儿童的医疗康复通常不可能在没有手术矫正颌骨咬合关系和改善面部美学的情况下完成。处理口面部裂儿童治疗的专家的主要工作是纠正上唇、鼻子、口腔的局部整形手术和使用固定弧形器械进行正畸治疗,以及纠正咬合受损。

目的是研究和分析先天性唇腭裂患者颌骨最终骨重建手术的问题。

材料与方法。本文根据61个资料来源,对这一主题的文献进行了回顾。从1938年到2020年,使用PubMed、ScienceDirect、eLibrary、GoogleScholar数据库中的关键词进行文献搜索。对患有牙颌面联合畸形和先天性唇腭裂畸形的儿童的治疗资料进行了分析,这些儿童在颌骨上进行了骨重建手术,以使其咬合关系正常化。

结果。为了获得良好的形态功能和美学效果,并在康复的最后阶段使先天性唇腭裂患者早日社会化,需要复杂的器械手术治疗。对患者进行颌骨重建手术的适应症是牙颌面畸形和畸形以及面部不协调,这是正畸治疗无法纠正的。手术应基于仔细的计划,包括计算机断层成像建模,使用保留技术,包括Le Fort I截骨术各种改良的上颌,下颌磨牙后矢状截骨术和生殖器成形术(如有必要)。

结论。两种旨在恢复正确咬合的颌骨重建手术,最近已开始用于先天性唇腭裂患者,特别是青少年的复杂医疗康复。与此同时,正畸和手术治疗还有许多未解决的问题需要进一步研究。这首先需要清楚地了解计划的年龄和此类手术的时机,不仅要考虑到骨骼生长完成的阶段,还要考虑患者准备好进行手术的标准。通过咬合接触的多样性,确定对面部和口腔软组织进行矫正手术的顺序,当然,还需要开发新的颌骨重建手术技术。
