Abstract

Arterial hypotension is a systemic hemodynamic disorder that is defined as a decrease in blood pressure below 90/60 mm Hg, caused by changes in cardiac output or systemic vascular resistance or both of these values, and that can lead to organ and tissue hypoperfusion. The review considers the epidemiology, etiology, and pathogenesis of this condition. Particular attention is paid to the classification of hypotension based on various biometric parameters, by the rate of occurrence and time of exposure, taking into account the impact on organ perfusion. The symptomatology and diagnosis of hypotension are analyzed; its complications are described.

Abstract

Infiltrative pulmonary tuberculosis is a clinical form of tuberculosis, it is detected most often by the appeal of patients to the general medical network with complaints. This is the most common form of tuberculosis at the present stage, it is 60-85% in the structure. The timeliness of diagnosis and the effectiveness of treatment depend on the correct tactics of the doctor and the nursing staff at the first contact with the patient. Historical aspects, knowledge of the pathogenesis, the main clinical manifestations and differential diagnostic criteria will allow for the correct differential diagnosis of tuberculosis with community-acquired pneumonia in primary health care.

The importance of monitoring viral activity and viral load of SARS-CoV-2

Abstract

The COVID-19 pandemic has made it possible to study many aspects of the laboratory diagnosis of this infection, including the assessment of the viral load and viral activity of SARS-CoV-2. The review presents data on modern views on the virological characteristics of the SARS-CoV-2 virus, however, many aspects of this problem remain debatable. The issues of the relationship between the level of viral load and the clinical profile of patients are proposed for discussion, and the possibility of using the obtained data in practical healthcare is discussed. The control of viral load in patients with COVID-19 has not only a certain clinical significance, but is also undoubtedly important in the epidemiological aspect. Monitoring of a number of SARS-CoV-2 parameters. will allow to solve practical problems in the context of the ongoing pandemic.

Epidemiological situation of ixodes tic-born borreliosis in the northern territories Krasnoyarsk Region

Abstract

The article presents an analysis of the epidemiological situation of ixode tick-borne borreliosis (TBE) in the Northern territory of the Krasnoyarsk region for the period 2008–2022. The subject of the investigation was the incidence rates of TBE, contamination of Ixodes ticks with Borrelia burgdorferi sl. 293 cases were registered (16,33 on 100 thousand population). The studies have significantly expanded the idea of the number of infections of Ixodes ticks. Monitoring data showed activity and potential danger of natural foci of the Northern territory of the Krasnoyarsk region. The real threat of infection exists both in forests and in cities and suburban areas.

The use of an Internet resource in young people to identify female students at risk for polycystic ovary syndrome

Abstract

Polycystic ovary syndrome (PCOS) is a polygenic endocrine disorder caused by both genetic and epigenetic factors. Depending on the period of a woman’s life, the clinical picture, diagnosis, and treatment tactics of the disease are different. The syndrome has a set of reproductive, metabolic, and psychological features.

Objective. To study the prevalence of PCOS among female students and to evaluate the effectiveness of using Internet platforms in identifying the manifestations that characterize reproductive health disorders.

Subjects and methods. A total of 330 respondents studying in the higher educational establishments of Khabarovsk were interviewed using the Google Forms platform in October to December 2021 in order to identify whether they had irregular menstruation, hyperandrogenism, which was followed by an examination and verification of the diagnosis of PCOS in an outpatient setting.

Results. According to the survey results, 87.58% of the respondents did not plan pregnancy in the next 2 years, so infertility as one of the PCOS symptoms did not cause anxiety in them. According to the survey among the female students with an irregular menstrual cycle, the incidence rate of PCOS was 24.7%.

Conclusion. In our opinion, the active detection of PCOS in patients with an irregular menstrual cycle among young people using Internet resources allows them to focus their attention on reproductive health disorders and on the need to consult a gynecologist.

Assessment of the rehabilitation programs effectiveness in patients with painful external genital endometriosis

Abstract

In patients with a painful form of external genital endometriosis (EGE), the prevailing symptom is chronic pelvic pain, which significantly reduces the quality of life (QoL). It causes disorders of the physical, psycho-emotional and sexual health of women of reproductive age, leading to distress, maladaptation, anxiety-depressive disorders, disruption of social functioning and family relationships.

Objective. To evaluate the effectiveness of the rehabilitation program for women with a painful form of EGE after radical treatment in the rehabilitation period.

Materials and methods. In the examined cohort were included a total of 104 patients of reproductive age 42.4±3.7 years with a painful form of EGE after radical treatment: Group 1 – 49 women with "active" rehabilitation; Group 2 – 55 patients with "passive" rehabilitation tactics in accordance with standards and National Clinical Guidelines. Integral assessment of QoL (objective) was performed using clinical, laboratory (cancer antigen-125 (CA-125), cancer antigen-19.9 (CA-19.9), interleukin-6 (IL-6), tumor necrosis factor (TNF), femoflor – 16, pH-metric titration), and instrumental studies (ultrasound, magnetic resonance imaging (MRI)). At the control points: before, in 5–9 days, 1,3,6 and 12 months after surgical treatment.

Results. Patients with "active" rehabilitation tactics (Group 1) had a significantly more pronounced decrease in the levels of the assessed biomarkers in blood serum, normalization of the urogenital tract microbiocenosis and restoration of pelvic organs and structures, unlike patients in the comparison group.

Conclusion. The use of comprehensive personalized rehabilitation in patients with painful form of EGE after radical treatment has demonstrated its effectiveness. Widespread introduction of rehabilitation programs into routine clinical practice is necessary.

Examination of a cardiac patient, by using carotid artery ultrasound screening: possibilities of telemedicine

Abstract

In clinical practice, there is a high workload of ultrasound (US) diagnostic rooms and a long waiting time for patients to be examined. For targeted selection of patients for expert US, it is possible for clinicians to conduct focus studies according to short protocols, which can be implemented using extramobile US devices.

Objective. To assess the possibility of using telemedicine technologies to train cardiologists in an US-assisted primary examination of patients at an outpatient stage, as well as to evaluate the quality of this US by expert doctors.

Material and methods. After briefly learning how to use a Philips Lumify handheld US device, two cardiologists independently examined the carotid arteries of patients, made conclusions about the presence of atherosclerotic plaques, and recorded the time spent on the examination. After each examination, the physician sent a video loop to an US expert, who assessed the quality of scanning.

Results. The investigation enrolled 104 patients. The opinion of a cardiologist on the presence of an atherosclerotic plaque coincided with that of the expert in 66% of cases. The physicians spent an average of <3.5 minutes per examination. The interaction between the cardiologists and the expert enables a 48% load reduction for an US room.

Conclusion. The introduction of a carotid US into a cardiologist’s outpatient practice will be able to substantially decrease the burden on healthcare facilities, by reducing the time required for appointments for cardiovascular diseases. At the same time, not only US diagnosticians, but also cardiologists are unloaded, since there is a decline in the number of secondary visits.

Stenting of coronary arteries in acute myocardial infarction in first degree relatives with familial atherosclerosis

Abstract

The hereditary predisposition of coronary heart disease is well known, especially in first-degree relatives. We present two cases – mother and son with coronary artery involvement. Initially, endovascular treatment was performed on a 44-year-old son admitted with acute myocardial infarction, 7 years later developed restenosis in the stent, which required repeated intervention. Four months later coronarography and stenting was performed his mother 78 years old with acute myocardial infarction. Genetic factors can influence both the risk of coronary heart disease and the risk of in-stent restenosis.

Abstract

Objective. To study the immediate and long-term results of the new modified technique of eversion carotid endarterectomy (ECEAE).

Material and method. The study involved 150 patients with 60–99% stenosis of the internal carotid artery (ICA) who were divided into 2 groups depending on the type of surgical treatment: Group1 (n=61) – patients underwent ECEAE according to the method developed by V.L. Sergeev et al.; Group 2 (n=89) – patients underwent ECEAE according to the method described by R. Kieny. The immediate and long-term results of the two ECEAE methods were evaluated.

Results. In the immediate postoperative period, no advantages of any of the ECEAE methods were revealed. In the distant postoperative period, 24 (16.8%) patients died. The incidence of non-fatal ischemic stroke on the operation side was 0% in Group1 and 7,1% in Group 2; the incidence of ICA restenosis formation was 1,7 and 11,7% respectively (p<0,05); 34,6 and 10,4% of patients returned to their labor activity and the complete regression of complaints was achieved in 61,5 and 20,9% respectively.

Conclusion. Thus, ECEAE according to the method V.L. Sergeev in the distant postoperative follow-up period showed its safety and efficacy and can be recommended for wider implementation in practical medicine.

Clinical and neuroimaging correlations in patients with chronic cerebrovascular disease and type 2 diabetes mellitus

Abstract

A comprehensive examination was made in 81 patients with chronic cerebrovascular disease (CVD) concurrent with type 2 diabetes mellitus. The patients were divided into 3 groups: 1) patients with mild cognitive impairment (CI) (Study Group 1); 2) patients with moderate CI (Study Group 2); 3) patients without CI (a comparison group). The investigators studied the neuroimaging structure of the brain substance (BS) and the state of the brachiocephalic vessels. The study showed that the morphofunctional changes in the brain were most pronounced in moderate CI Group 2, which confirmed the association between the severity of clinical manifestations of CVD and atherosclerotic lesions of the cerebral vessels and degenerative changes in the BS. Analysis of MMSE and MoCA scores indicated statistically significant differences between the indicators of the comparison group and the study groups, as well as between those of Groups 1 and 2. There were statistically significant differences in the incidence of postischemic subcortical cysts in the brain between Group 2 and the comparison group, as well as between Groups 1 and 2.

Characteristics of cerebral hemodynamics in patients with chronic HBV- and HCV-associated hepatitis

Abstract

Objective. To study the characteristics of cerebral hemodynamics in patients with chronic viral hepatitis (CVH) associated with HBV (CVHB) and HCV (CVHC) infection.

Subjects and methods. The investigation involved 69 patients with CVHB and 71 patients with CVHC. The control group consisted of 50 apparently healthy individuals compared with the sex- and age-matched study group. All the patients underwent extracranial Doppler imaging of the common, internal carotid and vertebral arteries, as well as transcranial Doppler ultrasound of blood flow in the area of the M1 segment of the middle cerebral artery using a Vivid-7 (GE) ultrasound machine.

Results. The patients with CVH showed remodeling of the vascular bed in the cerebral arteries: great vessel lumen expansion, decreased blood flow velocity in the carotid basin and middle cerebral artery, as well as reduced elasticity and increased stiffness of the vascular wall.

Effect of Femoklim on the state of blood vessels in hypoestrogenemia and arterial hypertension

Abstract

The article presents the results of duplex scanning of the common carotid arteries (CCA) and assessment of the structural and functional state of the vascular bed of 57 women with arterial hypertension (AH) with hypoestrogenemia. It was found that with hypertension with a reduced level of estradiol, the lumen of the OCA increases, the speed of blood flow in the carotid arteries decreases, which indicates vascular remodeling. In the studied patients, in every third case, an inertial type of vasomotor reaction is detected before treatment, which indicates the contribution of dishormonosis to the progression of hypertension due to the development of endothelial dysfunction. Combined therapy with phytoestrogens and ACE inhibitors improves the structural and functional characteristics of the vascular wall, reduces arterial stiffness. The drug Femoklim is a source of phytohormones, hormones of entomological origin and natural antioxidants, has a positive effect on the state of the vascular system, improves blood microcirculation, is a capillary protector, has a complex harmonizing effect on the body.

Correction of peroxidation of lipids with upadacitinib, a Janus kinase 1 inhibitor, in patients with true eczema

Abstract

Objective. To study the effect of upadacitinib (UPA), a Janus kinase inhibitor 1 kinase, on the status of peroxidation of lipids (POL) and the antioxidant system in patients with true eczema.

Subjects and methods. The investigation involved 64 patiens (43 males and 21 females) with true eczema (the mean age was 58.5±3.4 years). The patients were randomized into 2 groups: 1) 33 patients who received basic drug therapy (BDT); 2) 31 patients who received the Janus kinase 1 inhibitor UPA 15 mg/day during 2 months. The components of the antioxidant system and the state of POL were determined by enzyme immunoassay.

Results. After 2 months of therapy, Group 1 showed no statistically significant changes in the analyzed parameters from the baseline level (p>0.05). In Group 2, the total oxidative capacity of blood reduced by 1.27 tines; the level of oxidized low-density lipoproteins decreased by 1.28 times; the total antioxidant activity of blood increased by 1.24 times; the level of superoxidase dismutase increased by 1.43 times (p < 0.001 for all values).

Conclusion. The combined use of BDT and the JAK1 inhibitor UPA in patients with true eczema contributed to a statistically significant decline in the oxidative capacity of blood and POL products along with the enhanced antioxidant activity of blood.

Relationship of vitamin d level with some biochemical indicators of blood in patients over 60 years of age with osteoarthritis before surgery and after prosthetics

Abstract

Objective. To study the relationship between vitamin D level and biochemical blood parameters in patients over 60 years of age with osteoarthritis (OA).

Material and methods. In the investigation 200 patients aged 60–80 years (mean age – 69,03±2,34 years; 170 [85%] women and 30 [15%] men) with OA being observed in the out-patient medical institutions of Murmansk including 100 patients after total joint replacement took part. Vitamin D, triglycerides, thyroid stimulating hormone (TSH), total cholesterol, low and high density lipoproteins, blood glucose, uric acid, creatinine, total protein, and blood calcium levels were determined in all patients.

Results. Vitamin D deficiency was more frequently observed in the group of patients who did not receive surgical treatment compared to patients who underwent arthroplasty. A statistically significant relationship between vitamin D level and blood glucose was detected in the group of non-prosthetic patients. The relationship between vitamin D level, albumin and TSH was revealed in the group of prosthetic patients.

Development and clinical validation of a reagent kit for simultaneous detection of influenza A, B, influenza A H1pdm09 and coronavirus SARS-CoV-2

Abstract

Respiratory tract infections are the main cause of temporary disability for work and take a major toll on economies in whole world. Early and accurate detection of etiological agent will improve treatment efficiency and prevent spreading of epidemy. The reagent kit for simultaneous detection of influenza A, B virus, H1pdm09 influenza A strain and coronavirus SARS-CoV-2 was developed. For clinical validation TIB Molbiol (E gene) kit (for detection of SARS-CoV-2) and CDC Influenza Virus Real-Time RT-PCR Panel Influenza A/B typing Panel were used. The assay has shown high sensitivity and specificity against different strains of SARS-CoV-2 and influenza viruses. The analytical sensitivity was 500 copies/ml for detection of Influenza A and B viruses and 250 copies/ml for SARS-CoV-2. No cross-reaction with other respiratory pathogens was detected. Clinical sensitivity and specificity against comparator test were 100%. Developed reagent kit intended for use in clinical laboratories for differentiating diagnosis of coronaviral infection and influenza A and B.

The effect of hypertension on the course and severity of psoriasis

Abstract

Hypertension is one of the significant comorbid diseases in psoriatic patients that have an important impact on the quality of life of patients. Early studies and literature reviews mostly considered psoriasis as an inducing factor in the development of many comorbidities, including cardiovascular diseases in general, and also highlighted the influence of psoriasis on the severity and course of comorbid conditions. In this review, special attention is paid to hypertension as an independent risk factor for the onset of psoriasis, pathogenetic mechanisms indicating the possibility of psoriasis induction by existing hypertension are presented, as well as information on how hypertension aggravates the course, clinical manifestations, severity and the possibility of therapy of the underlying disease.
