Radical treatment of invasive bladder cancer

Abstract

We have conducted a retrospective analysis of radical cystectomy in 53 patients (45 males and 8 females, 85 and 15%, respectively) with invasive bladder cancer (ВС) treated in the Research Institute of Urology in 1997-2002. Stages T2N0M0, PT3aN0M0, T3bN0M0, pT4aN0M0, pT4aNl-2M0 were in 4(7.5%), 13 (25%), 21 (40%), 7 (12.5%), 8 (15%) cases, respectively. Well differentiated transitional cell ВС (Gl) was detected in 1 (2%) patient, moderately differentiated (G2) in 16 (30%) cases, poorly differentiated (G3) in 36 (68%) patients. The following methods of urine derivation were used: orthotopic plastic surgery (n = 3, 6%), ureterocutaneostomy (n = 4, 8%), Mainz pouch II operation (n = 16, 30%), Hassan operation (n = 5, 9%), Bricker procedure (n = 22, 44%), transureteroanastomosis (n = 3,6%). In the postoperative period there was one lethal outcome, early complications in 5 (9%) patients, late complications in 9 (17%) patients. Distant metastases to the lungs, bones and iliac lymph nodes after treatment were detected in 3, 2 and 3 patients, respectively. One patient had a local pelvic recurrence. For 53 patients 2-year corrected survival was 68+12.0%. We have come to the conclusion that the only radical surgical treatment of invasive ВС is cystectomy, limitations to which are connected only with complexity of subsequent urine derivation.

Organization of care for patients with urinary bladder cancer in the Russian Federation and approaches to its improvement basing on standardization of diagnostic and therapeutic process

Abstract

Present-day medical facilities for treating patients with cancer of the urinary bladder in the Russian Federation (RF) are insufficient. In 1998, urological cancer in RF was managed only in 31 specialized departments working in 24 of 89(27%) administrative units of the RF. In the other 65 (73%) units only urologists deal with diagnosis and treatment of urinary bladder cancer. These nonspecialized departments lack special equipment and personnel. Therefore, effective management and organization of medical care for urinary bladder cancer can be achieved on the basis of standardization of diagnostic and therapeutic methods and definition of stages in treatment policy. This helps to outline priorities in financing health services.

Short-term results of radical cystectomies

Abstract

The paper presents the results of 114 radical cystectomies made in 1996-2002. The age of 114 patients (103 males, 11 females) ranged from 37 to 78 years (mean age 57.5 years). Transient cell carcinoma was diagnosed in 81.5% patients. Supravesical urine derivation was conducted by means of ureterocutaneostomy and transureteroureteronephrostomy in 9 (7.9%) patients, ureterosygmoanastomosis - in 43 (37.7%) patients, artificial orthotopic urinary bladder was created in 7 (6.1%) patients of a gastric segment and in 55 (48.2%) patients of the ileum. Postoperative complications were observed in 28 (24.6%) patients, intestinal obstruction being a prevailing complication. Five patients died: 2 of pulmonary artery thromboembolism, 1 of acute cardiac failure, 1 of sepsis and 1 of gastric bleeding. Continent methods of urine derivation were preferred, such as ureterosygmoanastomosis by Mainz-Pouch II and creation of orthotopic urinary bladder of the stomach or of the ileum.

Long-term results of continent urine derivation on the skin into a detubulized small intestinal reservoir

Abstract

Seventy five patients (48 females, 27 males) at the age of 17 to 78 years (mean age 54.6 years) were radically operated on for locally advanced tumors of the small pelvis organs in the Cancer Research Center of Georgia in 1987-2001. The urine was derived continently on the skin into a detubulized small intestinal reservoir. Invagination valvular device was created in 31 patients, non-invagination (the original technique) - in 44. Conversion of ureterocutaneostomas according to the original technique was made 6-7 months after radical operation in 4 of them. The ureters were transplanted using the antireflux (67) and direct (8) methods. 1-10 follow-up data are available for 52 patients. 4 patients with invagination valve developed postoperative and 2 patients delayed (6-9 months) incontinence. In patients with the original valve incontinence was only in one patient. The difference between the results of antireflux and direct transplantation of the ureters was statistically insignificant.

Gastric mucosa in patients with benign prostatic hyperplasia early after surgery at initial stages of chronic renal failure

Abstract

Biochemical, endoscopic and morphological tests were made to study changes in gastric mucosa and activity of antioxidant defense enzymes of the blood in patients with benign prostatic hyperplasia and initial chronic renal failure early after transvesical adenomectomy. Acute erosive-ulcerative lesions of gastric mucosa and acute gastric hemorrhages arose most frequently in patients with latent stage of chronic renal failure operated on without preoperative preparation. Preoperative medication lowers probability of the complications.

Assessment of cyproteron acetate monotherapy efficacy in prostatic cancer

Abstract

The trial conducted in the Primorsk Regional Cancer Hospital of Vladivostok and P. A. Herzen Moscow Research Cancer Institute studied effectiveness and tolerance of cyproteron acetate (CA) monotherapy and maximal androgenic block (MAB) (CA+surgical or CA+drug castration). A total of 70 patients with localized, locally advanced and generalized prostatic cancer entered the trial. They were divided into three groups: group 1 (54.3%) patients received CA monotherapy - 300 mg/day, group 2 (27.1% patients) was exposed to MAB by surgical castration and received CA in a dose 200 mg/day, group 3 (18.6% patients) were exposed to MAB by drug castration and received 200 mg/day CA. The results obtained show that CA monotherapy and MAB have similar effectiveness in favourable prognostic signs. С A monotherapy was less toxic than CA combination with castratio.

Indirect electrochemical blood oxidation in the prophylaxis of pyoinflammatory complications after TURP

Abstract

To investigate efficiency of indirect electrochemical blood oxidation in prophylaxis of infectious complications, 28 patients were treated with 0.06% hypochlorite solution (2 infusions, 2-3 days before TURP). These patients belonged to a high-risk group of infectious complications (80% of them had chronic prostatitis, 8 patients had bladder stones, 8 had undergone cystostomy). The results demonstrated that the complication rate in the above prepared patients fell four times as compared to the control group and the postoperati hospital stay was reduced by 3 days.

Lactoferrin assay in urethritis and urethroprostatitis

Abstract

An immunochemical study of prostatic secretions in 97 patients with urethritis and iirethroprostatitis has detected a significant rise of lactoferrin level in mixed infections (trichomonads+chlamydia and trichomonads+uroplasma - 17 patients) versus control (14 healthy males) and group as a whole. In the control group a mean lactoferrin level was 5l.2±8.4 mg/1 (12 mg/1-67.3 mg/1), in the group as a whole 106±41.5 mg/1 (19.5 mg/1 to 334.8 mg/1) in patients with mixed infection 221.25 (102.9 mg/1 to 334.8 mg/1). The assessment of diagnostic significance of the immunochemical test for lactoferrin in detection of mixed infections in urethritis and iirethroprostatitis has shown that in threshold level of 150 mg/1 the test's specificity was 87.5%, sensitivity 94.1%, effectiveness 88.65%, prognostic value of positive result 61.5%, of negative result 98.59%; in threshold level 200 mg/1 the respective values were 100, 52.9, 91.75, 100 and 90.9%. Practical application of the proposed immunochemical test in detection of mono- and mixed infection is discussed.

Laboratory tests in acute epididymitis etiology

Abstract

A laboratory study in order to establish etiology of acute epididymitis (AE) was made in 243 patients. The complex of etiological diagnosis included: microscopic investigation of urethral discharge or urine, bacteriological examination of an initial urine portion and wound discharge in the operated on patients. In addition, urethral discharge/urine was studied with polymerase chain reaction (PCR) in 96 patients; in 46 men PCR examined operative material. It was found that in sexually active young patients AE was caused primarily by infectious agents of sexually transmitted diseases. Targeted etiotropic therapy results in clinical cure in the majority of the cases. In older patients the disease was of bacterial origin (gram-negative bacteria, as a rule, are causative agents)and in some cases is associated with development of pyodestructive epidydimitis.

Nephrolithotomy in the treatment of severe nephrolithiasis

Abstract

Nephrolithotomy results are available for 63 patients with coral and multiple nephroliths operated in 1990-2002. These patients underwent 64 operations (one patient with bilateral nephrolithiasis had 2 consecutive nephrolithotomies within a month). The sites preferable for transverse nephrolithotomy are described and validated. Ligation of the renal artery with pharmacological antiischemic protection of the kidney with lasix and verapamil was performed in 40 cases.

Spa treatment with mineral water Serebryany klyuch in postoperative treatment of patients with nephrolithiasis after extracorporeal shock-wave lithotripsy

Abstract

27 patients with nephrolithiasis were exposed to extracorporeal shock-wave lithotripsy (ESWL). They were divided into two groups: group 1 of 10 patients who were exposed to ESWL and drank tap water and group 2 of 17 patients who were exposed to ESWL and drank silver-containing mineral water Serebryany klyuch. In group 2 there was a marked diuretic effect manifesting with intensive excretion of electrolytes (K, Na), phosphorus and uric acid. This led to improvement and normalization of renal function, promoted faster elimination of concrement fragments and more frequent (by 52.3%) complete elimination of the concrements.

Spasmex treatment of patients with hyperactive urinary bladder

Abstract

Overactive urinary bladder (HAUB) is encountered in 16-20% patients with various voiding disorders. Etiologically, detrusor hyperactivity can be neurogenic and idiopathic. We studied efficacy of the antimuscarine drug spasmex in patients with imperative voiding disorders and compared its efficacy and tolerance with those of driptan and detrusitol. 193 female patients with imperative voiding disorders were examined. Spasmex given in a dose of 5 mg 3 times a day for 12 weeks produced a good effect in 56 patients: increased the effective capacity of the urinary bladder, reduced the number of voidings and episodes of incontinence for a day. A 3-month course of spasmex proved highly effective as shown by the patients' subjective response and positive changes in objective evidence. Being equally effective with detrusitol and driptan, spasmex is much superior to oxibutynin in safety.

Injuries of the urogenital system

Abstract

To analyse the results of treatment of isolated and concomitant urogenital injuries, a retrospective analysis was made of 608 case histories for patients treated in 1986-2000. Of them, 530 patients were males, 78 were females, the age ranged from 16 to 70 years, most of the patients were 20 to 50 (71.2%) years of age. Most frequently the injury was inflicted by beating. Fall from the height, transport accidents ranked second among the causes of injury (17 and 14.3%, respectively). Stab and gunshot injuries occurred in 4.4 and 8% cases, respectively. Ultrasound and x-ray methods were employed for examination. An isolated injury was found in 481 patients, the rest had urogenital injury and skeletal (53%), craniocerebral (25%), abdominal (20%) lesions. 29 patients had a severe concomitant wound: urogenital injury combined with abdominal, skeletal, cranial lesions. Surgery was performed in 267 patients. The rest patients received conservative treatment. Most of the operations were made on the kidney and urinary bladder because of rupture. In some patients these interventions were conducted during laparotomy and were combined with abdominal intervention (44 cases). 25 patients with the compound injury underwent 2-stage operations. 552 patients were cured. 56 (9.2%) lethal outcomes were due to progression of traumatic shock. This low percent of lethality may be explained by an optimal choice of treatment policy in the above patients.

Plastic repair of the urethra with a vascular fasciocutaneous and combined flap (an experimental study)

Abstract

The experiments on 15 dogs with preformed urethral stricture or urinary urethral fistula have demonstrated that microsurgical transplantation of the vascular fasciocutaneous pedicle flap provides good functional long-term results. The best effect was achieved in cases of intraflap inclusion of previously transplanted mucosa of the urinary bladder. In all 7 experiments x-ray picture was similar to normal. Recurrent strictures or urethral fistulas were not registered. Urodynamic indices late after the experiment did not differ significantly from normal values.
