Перкутанная нефролитотрипсия при камнях единственной почки

Аннотация
Перкутанная нефролитотрипсия (ПНЛ) служит методом выбора при крупных и коралловидных камнях почки, в том числе и единственной. Целью обзора литературы стал анализ результатов ПНЛ у пациентов с крупными камнями единственной почки. Поиск проведен в базах данных Embase, Medline, Google Scholar, Scopus за период с 2010 по 2019 г. В итоге отобрано 94 статьи, из которых после изучения абстрактов в обзор были включены данные 16 публикаций. Данные были объединены и проанализированы с помощью программы SPSS Statistics 22.0. Начальная эффективность ПНЛ для этих больных составила 68,3± 14,2%, итоговая - 86,5±4,9%. Время операции в среднем было равно 86±25 (43,7-138,3) мин, длительность госпитализации - 5 (2-6) дням. Предоперационный уровень креатинина составил 137 (110-200) ммоль/л, послеоперационный - 142 (122-183) ммоль/л. Разница между уровнем гемоглобина до и после ПНЛ составила в среднем 7,8 (1,3-17,5) г/л. Скорость клубочковой фильтрации (СКФ) до операции была равна 62,3+7,5мл/мин/1,73 м2, после нее не превышала 62,5+9,3 мл/мин/1,73 м2. Общая частота осложнений варьировалась от 10,6 до 68,8%, составив в среднем 29,9%. В большинстве случаев имели место осложнения Iи IIстепеней по Clavien-Dindo, не требовавшие дополнительных вмешательств. ПНЛ - высокоэффективный метод лечения больных с крупными камнями единственной почки. Осложнения развиваются сравнительно редко, что достигается индивидуальным подходом с учетом состояния и всех факторов риска.
