HELICOBACTER-ASSOCIATED FORM OF PEPTIC ULCER OF STOMACH AND DUODENUM: PROBLEMS OF THERAPY

Abstract

The article is dedicated to the main issues of the pathogenesis of peptic ulcer of stomach and duodenum, and their close relationship with the Helicobacter pylori infection. The main indications and purposes of treatment of peptic ulcer associated with H. pylori are presented. Modern approaches to the treatment of helicobacteriosis, the recommendations of international consensus, and the possible ways to resolution of the issue of increasing resistance of microorganisms to antibiotics are considered. Most of the focus is on new regimens of antihelicobacter therapy, in particular a sequential scheme and a new quadruple therapy based on bismuthate tripotassium dicitrate.

Abstract

The article discusses a functional disorder of the gastrointestinal tract - irritable bowel syndrome (IBS). The basic mechanisms of the pathogenesis of IBS, clinical manifestations are discussed; treatment approaches in accordance with the mechanisms of clinical symptoms are described. Much attention is paid to the role of modern selective myotropic spasmolytics with combined effect - calcium channel blocker pinaverium bromide (Dicetel) and sodium channels blocker mebeverine (Duspatalin) in the correction of abdominal pain, dyspeptic symptoms and other manifestations of IBS; current algorithms of diagnosis and therapy of this pathology are presented

Abstract

The article is dedicated to the modern idea of the effect of opiates and opioid peptides on the function of the gastrointestinal tract (GIT), their interaction with different types of opioid receptors (OR). The possibilities of use of OR agonists (opiates, dalargin, loperamide, trimebutine) are discussed. The greatest interest is attracted to trimebutine (Trimedat) - a full agonist for all three major types of OR (μ, κ, δ), from whence trimebutine is universal regulator of motility. The mechanism of action Trimedat is associated with the stimulation of OR throughout the GIT. Trimebutine can be used for the treatment of irritable bowel syndrome in patients with functional non-ulcer dyspepsia and various dyskinetic disorders in children.

Abstract

Gastrointestinal bleeding is one of the main causes of emergency hospitalization in developed countries. Strict adherence to the algorithm of action and continuity of care for bleeding at different stages of specialized medical care provides significantly better prognosis and survival in patients with this terrible syndrome. Effective treatment of non-variceal bleeding while minimizing the risk of relapse is possible only with combination of modern methods of endoscopic hemostasis and conservative treatment, including the use of high doses of proton pump inhibitors, which halves the risk of recurrent bleeding. Prophylactic eradication of Helicobacter pylori and/or subsequent prolonged parallel use of proton pump inhibitors in patients with suspected long-term use of aspirin and nonsteroidal anti-inflammatory drugs can significantly reduce the risk of ulcers and bleeding.

TREATMENT OF ULCERATIVE COLITIS: EXPERIENCE OF USE OF METHOTREXATE IN SSC OF COLOPROCTOLOGY

Abstract

The article is concerned with possibilities of use of methotrexate in patients with ulcerative colitis (UC) with hormone dependence or hormone resistance. Authors present the experience of use of methotrexate in the treatment of 72 patients with severe UC. Hormone resistance was indication for administration of methotrexate in 59 patients, and hormonal dependence - in 13 patients. The drug was administered at a dose of 25 mg per week for one year against the background of gradual withdrawal of prednisolone. The overall effectiveness of methotrexate in severe UC was 63.8%. During the year, aggravation of UC was registered in 23.3% of patients against the background of maintenance therapy with methotrexate. It is emphasized that the use of methotrexate allows completely discontinuing hormone therapy in hormone-related forms of UC.

Abstract

The article discusses diagnosis strategy and treatment of nonalcoholic fatty liver disease (NAFLD). It is emphasized that the identification of NAFLD based on the detection of relevant risk factors is always associated with high risk of cardiovascular and renal complications. Treatment of NAFLD involves an aggressive effect on the risk factors such as abdominal obesity and disorders of lipoprotein metabolism, particularly hypertriglyceridemia and insulin resistance. Possibility of application of statins, metformin, ursodeoxycholic acid, and other drugs in NAFLD is considered.

Abstract

The authors have conducted an open two-phase comparative study for the purpose of evaluation the results of differentiated treatment with various enzyme preparations, depending on the mechanisms of exocrine pancreatic insufficiency in patients with chronic pancreatitis. Long-lasting therapy with modern high-dose microencapsulated enteric-coated preparations of pancreatin has proven effectiveness in relieving the markers of exocrine pancreatic insufficiency and abdominal pain syndrome in patients with chronic pancreatitis, as well as in disease prevention, improvement of trophological status of patients and improvement of their quality of life. Use of modern pancreatin enzyme preparations as enzyme replacement therapy is the key to successful treatment in both inpatient and outpatient conditions.

INFLIXIMAB IN ULCERATIVE COLITIS: REVIEW OF STUDIES AND OWN RESULTS

Abstract

The article is concerned with international experience of use of infliximab (Remicade) in patients with ulcerative colitis (UC), indicating that this drug is effective for achieving clinical response and induction of clinical remission; influximab accelerates the repair of the mucous membrane of the colon and reduces the frequency of emergency colectomy. Authors present the results of own study, which included 37 patients with UC not underwent surgery. All patients have received Remicade according the Additional pharmacological support program (DLO) for Moscow region in a dose of 5 mg/kg of body weight per day as induction course (3 infusions at the initial, 2-nd and 6-th week) and then every 8 weeks. The effectiveness of infliximab induction therapy was high, positive clinical and endoscopic dynamics was observed in more than 90% of patients receiving the drug. In most cases (96.3%), supportive therapy with Remicade has provided clinical and endoscopic remission or significant improvement. Corticosteroids were abolished more than in 80% of patients, which have received more than 3 infusions of infliximab.

Abstract

The study was aimed to evaluation of efficacy and tolerability of mebeverine (Duspatalin) which was used to relieve spastic pain syndrome in patients with functional disorders of biliary tract. After exclusion of organic pathology and other functional digestive diseases, the study enrolled 69 patients with functional disorders of biliary tract. Patients were divided into 2 groups: Group 1 - 31 patients with preserved gallbladder, and Group 2 - 38 patients previously had undergone cholecystectomy. Nature of the disorders of the motor function of the biliary tract was estimated by dynamic hepatobiliary scintigraphy. All patients have received mebeverine at a dose 200 mg twice a day, duration of therapy was 28 days. Against the background of treatment, significant positive changes in frequency and intensity of spastic biliary pain were registered in both groups. The efficacy of treatment was estimated as "very good" or "good" by most of the patients. Adverse events in patients receiving mebeverine were not observed.

Abstract

Prohepar is synergistic hepatoprotective combination based on liver hydrolysate received from bullocks and enriched by cyanocobalamin, choline, myo-inositol and cysteine. The available data for the experimental and clinical pharmacology of Prohepar and its components indicate its undoubted hepatoprotective effect. The authors for the first time have investigated experimental changes in biochemical markers of liver function and histology of liver tissue, as well as other organs in alcohol- and paracetamol-induced liver damage against the background of therapy with Prohepar. Improvement in functional and structural state of the liver against the background of use of Prohepar was shown. Hepatoprotective effect was most noticeable on the model of alcohol-induced liver injury. Histological examination of tissue samples showed a marked morphological differences, which were expressed in radically different degrees of damage of parenchymal cells and circulatory disturbances. Revealed differences in the structure of liver tissue, as well as changes in biochemical markers levels, indicate pronounced protective effect of Prohepar in alcohol- or acetaminophen-induced liver damages.

EXPERIENCE OF USE OF ITOPRIDE FOR THE TREATMENT OF ENT-MANIFESTATIONS OF LARYNGOPHARYNGEAL REFLUX

Abstract

Laryngopharyngeal reflux (LPR) is closely associated with gastroesophageal reflux disease and involves reflux of gastric material in laryngopharynx with appropriate clinical manifestations. The authors have assessed the effectiveness of two-week course of therapy with prokinetic itopride (Ganaton) in the treatment of otorhinolaryngologic manifestations of LPR in 20 patients. Administration of itopride led to a decrease in subjective complaints of patients and reduction of severity of laryngoscopic signs as redness and swelling of the mucous membrane of arytenoid cartilages. This observation allows to recommend the use of Ganaton in the combined treatment of LPR manifestations.
