ПРЕПАРАТЫ ГАЛВУС И ГАЛВУС МЕТ В ТЕРАПИИ ПОЖИЛЫХ ПАЦИЕНТОВ С САХАРНЫМ ДИАБЕТОМ 2 ТИПА

Аннотация
В пожилом возрасте распространенность сахарного диабета 2 типа (СД2) прогрессивно увеличивается, достигнув 20 % среди лиц старше 70 лет. Пожилые пациенты с СД2 являются гетерогенной группой с многочисленными сопутствующими заболеваниями, высоким риском гипогликемий и предрасположенностью к развитию побочных эффектов лекарственной терапии, что значительно затрудняет лечение этой сложной популяции больных. Риск развития тяжелых гипогликемий, вероятно, представляет собой самую сложную проблему в терапии пожилых пациентов с СД. Инкретины, к которым относятся глюкозозависимый инсулинотропный полипептид (ГИП) и глюкагоноподобный пептид-1 (ГПП-1), являются гормонами, выделяемыми в ответ на прием пищи в слизистой оболочке кишечника, обеспечивают глюкозозависимую стимуляцию секреции инсулина и подавление постпрандиальной секреции глюкагона. Терапию, основанную на инкретинах, включая аналоги ГПП-1 и ингибиторы дипептидилпептидазы-4 (ДПП-4), можно считать обоснованным вариантом выбора для пожилых пациентов. Эти препараты характеризуются хорошей переносимостью, высокой эффективностью и низким риском гипогликемий. Недавно было опубликовано несколько мета-анализов по оценке эффективности и риска терапии пожилых пациентов ингибитором ДПП-4 вилдаглиптином в монотерапии и в комбинации. В данной статье анализируются опубликованные данные по применению препаратов Галвус (вилдаглиптин) и Галвус Мет (вилдаглиптин + метформин) в популяции пожилых пациентов.
