SAXAGLIPTIN (ONGLYZA ®) IN CONCEPTION OF EFFECTIVE MANAGEMENT OF TYPE 2 DIABETES MELLITUS

Abstract

Effective management of type 2 diabetes mellitus (DM2) is one of the most topical problems of modern diabetology. This is determined by progressive nature of the disease and treatment complications (hypoglycemia and weight gain), which develop on the background of a number of oral glucose-lowering drugs and have significant effect not only on treatment compliance of DM2 patients, but also on risk of cardiovascular diseases. In this regard, strict requirements for efficiency and safety of modern pharmacotherapy of type 2 diabetes mellitus are required. Saxagliptin - dipeptidyl peptidase-4 inhibitor - meets these criteria. Saxagliptin belongs to a new group of antidiabetic drugs; its glycose-dependent action is based on the effect of incretins. Clinical studies have confirmed the efficacy and safety of saxagliptin in DM2 patients both as monotherapy and in combination with other antidiabetic drugs.

APPROACHES TO THE TREATMENT OF DIABETES MELLITUS IN ELDERLY PATIENTS: POTENTIALS FOR USE OF INSULIN NOVOMIX 30

Abstract

The article presents a review of current approaches to the treatment of elderly patients with diabetes mellitus. The target levels of blood glucose and glycated hemoglobin in these patients are discussed; non-drug and drug methods of treatment, including therapy-related risks are considered. The concept of geriatric syndromes is presented. Results of studies on the use of insulin NovoMix 30 in elderly diabetic patients are presented in detail. It is shown that the use of insulin NovoMix 30 is the choice of an effective, safe and simple therapy for elderly patients.

DIABETON MB IN THE TREATMENT OF TYPE 2 DIABETES MELLITUS AND PREVENTION OF ITS LATE COMPLICATIONS

Abstract

The article is dedicated to the role of sulfonylureas (SU), primarily gliclazide (Diabeton MR) in the treatment of type 2 diabetes mellitus (DM2), discusses their effects on the cardiovascular system. Data supporting the benefits of gliclazide in front of so-called old SU (glibenclamide, etc.) are presented. It is shown that in addition to pronounced hypoglycemic action gliclazide has an antioxidant property, ability to prevent development of micro- and macrovascular complications of DM2, including myocardial infarction and stroke. The antioxidant properties, the lack of effect on apoptosis provide gentle action of this drug on β-cells, that is also a undeniable advantage of gliclazide.

ROLE OF GLYCATION END-PRODUCTS BLOCKERS IN THE DEVELOPMENT OF NEUROLOGIC COMPLICATIONS OF DIABETES MELLITUS

Abstract

The article considers the pathogenesis of neurological complications of diabetes mellitus in terms of the damaging effect of hyperglycemia and activation of pathological ways of glucose utilization. One of the most important mechanisms of tissue damage by glucose is the formation of advanced glycation end-products (AGEP) and their predecessors. B vitamins, including fat-soluble thiamine analogue benfotiamine, play an important role in the treatment of diabetic neuropathy. It was shown that benfotiamine is a neurotropic substance with the ability to block excessive formation of the AGEP. Benfotiamine with pyridoxine are components of drug Milgamma compositum. Clinical evidences for the therapeutic effect of benfotiamine in the treatment of diabetic neuropathy are presented.

Abstract

The article focuses on the problem of treatment of patients with active acromegaly partially resistant to therapy with somatostatin analogues (SA). The review of clinical studies on medical treatment of acromegaly with octreotide and lanreotide is presented; aspects of the effectiveness of these analogues are discussed. The effectiveness of increasing doses of the SA and the combined treatment with dopamine agonists and SA in patients of this group are analyzed. The options for differentiated management in the absence of disease control against the background of monotherapy with SA are proposed.

COMBINED THERAPIES OF OSTEOPOROSIS: BISPHOSPHONATES AND THE D-HORMONE ANALOGS

Abstract

The article considers new potentials for prevention of vertebral and nonvertebral fractures, which are mainly attributable to postmenopausal and glucocorticoid-induced osteoporosis. In this context, the combination of bisphosphonates (BP, especially alendronate) and D-hormone analogs is of considerable interest. The theoretical basis of the complementary actions of alendronate and D-hormone analogs is discussed; data on the effects of BP and D-hormone analog on the falls, bone mineral density and fracture risk in various forms of osteoporosis are presented. The data on the safety of this combination are considered. It is emphasized that the combined use of active vitamin D metabolites with BP (for example, alfacalcidol with alendronate) demonstrates greater efficacy compared with monotherapy because of the different mechanisms of action, complementing each other (complementary or synergistic effect), that makes this combination valuable alternative for the treatment of osteoporosis.

Abstract

The article shows the advantages of dipeptidyl peptidase-4 inhibitors (DPP-4) associated with their multifaceted and physiological effects on the basic pathogenetic mechanisms of this disease, especially when used in combination with metformin, in patients with type 2 diabetes mellitus (DM). In Russia, DPP-4 inhibitors vildagliptin (Galvus) and a fixed combination of vildagliptin + metformin (Galvus Met) are widely used. Preliminary results of the program Efficacy of Galvus Met in real-life clinical practice of endocrinologists started in Moscow are presented; it was shown that the use of this drug in DM2 patients significantly improves glycemic control, is well tolerated and has beneficial effect on body weight and blood pressure.

Galvus and Galvus Met in treatment of elderly patients with type 2 diabetes mellitu

Abstract

In old age, the prevalence of type 2 diabetes mellitus (DM2) increases progressively, reaching 20 % among people older than 70 years. Elderly DM2 patients are a heterogeneous group with multiple comorbidities, high risk of hypoglycemia and susceptibility to side effects of drug therapy, which greatly complicates the treatment of this complex patient population. The risk of development of severe hypoglycemia, probably, is the most difficult problem in the treatment of elderly patients with diabetes mellitus. Incretins, including glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1), are the hormones released in the intestinal mucosa in response to a meal; they provide glucose-dependent stimulation of insulin secretion and suppression of postprandial glucagon secretion. Therapy based on incretins, including GLP-1 analogs and dipeptidyl peptidase-4 (DPP-4) inhibitors, can be considered as reasonable choice for older patients. These drugs are characterized by good tolerance, high efficacy and low risk of hypoglycemia. Recently, several meta-analyses on evaluation of the effectiveness and risks of treatment of elderly patients by DPP-4 inhibitor vildagliptin in monotherapy and in combination were published. The article analyses the published data on the use of Galvus (vildagliptin) and Galvus Met (vildagliptin + metformin) in a population of elderly patients.

The article considers main functions of insulin pump therapy, imitating physiological insulin secretion, as well as the principles for calculating the temporary basal rate (TBR) and selection the type of bolus, which allows to reduce the variability of blood glucose levels and provides long-term compensation of diabetes mellitus. Clinical examples of these features of advanced menu of insulin pump user are presented. The importance of training the patient to analyze the need for insulin depending on the situation and maximize the advantages of pump for achieving of compensation of diabetes is emphasized.

Abstract

The article considers the definition, epidemiology, etiopathogenesis and differential diagnosis of incidentalomas and hormone-inactive tumors (HIT) of adrenal glands. The existence of differences between incedentalomas and HIT is emphasized. It is shown that term "incidentaloma" is collective concept and indicates that the identified (usually by accident) tumor has not led to typical clinical picture of endogenous hypercorticism, primary hyperaldosteronism, phaeochromocytoma, and feminizing or virilizing tumor. This diagnosis should be considered only as provisional diagnosis; therefore, physician should focus on a particular set of tests for clinical diagnosis and treatment-management of the patient (surgery, observation).

HORMONAL-METABOLIC PARAMETERS IN YOUNG MEN WITH OBESITY MANIFESTING AT PUBERTY

Abstract

65 young men with obesity manifesting in puberty, average age 19,22 ± 0,26 years, were examined with the aim of evaluation the relationship of leptin, resistin and insulin resistance. Control group included 17 healthy young men, average age 22 ± 0,72 years. It was found that young men with obesity had abdominal type of fat distribution and atherogenic dyslipidemia associated with abdominal fat distribution. In the group of obese patients, insulin resistance and compensatory hyperinsulinemia were found. There was significant increase of leptin levels. Direct correlation between leptin, body mass index, waist and hip circumference were observed. In addition, a direct correlation between leptin levels and insulin and resistin levels was revealed.

The article considers the reasons for the occurrence of episodes of hypoglycemia in pregnant women with type 1 diabetes mellitus. A comparative analysis of relief of hypoglycemia by pills d-glucose (dextrose), or by receiving the typical foods with high glycemic index are presented, clinical examples are given. The efficiency of dextrose in increasing of blood glucose level in 15 minutes after its reception during hypoglycemia without rebound hyperglycemia in 60 minutes is proved.

Neuroprotection in correction of neurological complications in primary hypothyroidism

Abstract

The article presents the results of evaluation of nature of neurologic complications in primary hypothyroidism (PH) and their possible correction for 115 patients with confirmed primary thyroid deficiency. The diagnostic findings and the results of treatment have shown that nervous system impairment in PH was almost obligate among most patients and manifested mainly by cerebellar disorders, sensitive polyneuropathy, and cognitive disabilities. Combined therapy including not only replacement therapy, metabolic and vasoactive drugs, but also neuroprotective drugs, should be used for the correction of neurological complications of PH, because it contributes to a significant improvement and leads to regression of pathological symptoms.
