Азитромицин в терапии хронической обструктивной болезни легких: лечение и профилактика обострений

Аннотация
Хроническая обструктивная болезнь легких (ХОБЛ) в силу широкой распространенности, высокой смертности и значительных экономических потерь для общества является одной из наиболее актуальных проблем здравоохранения. К важнейшим факторам, определяющим прогрессирование ХОБЛ и снижающим качество жизни пациентов, относятся обострения болезни. Рассматриваются этиопатогенез обострений ХОБЛ, возможные пути их лечения и профилактики с использованием антибактериальных препаратов. Обсуждается место макролидных антибиотиков, прежде всего азитромицина, в терапии обострений ХОБЛ. Подчеркивается, что азитромицин играет важную роль в терапии обострений ХОБЛ в связи с широким спектром действия и благоприятным профилем безопасности. В отличие от β-лактамных антибиотиков он эффективен в отношении основных актуальных патогенов при обострении ХОБЛ, в т.ч. внутриклеточных, а по сравнению с другими макролидами обладает выраженной активностью в отношении гемофильной и синегнойной палочек. Азитромицин существенно отличается от других антибиотиков уникальной фармакокинетикой, прежде всего накоплением в высоких концентрациях в тканях, особенно в клетках, и длительным периодом полувыведения из организма, что позволяет применять его один раз в сутки коротким курсом.
