Enzyme replacement therapy with pancreatin preparation in children with exocrine pancreatic insufficiency

摘要

The article discusses the problem of pancreatic diseases accompanied by the development of absolute and relative pancreatic insufficiency. An algorithm for the appointment of pancreatin preparations depending on the cause of the formation of exocrine pancreatic insufficiency is presented. The indications, contraindications, and features of appointment of pancreatic enzymes in children are discussed. It is noted that modern enzyme preparations consisting of enteric-coated pancreatin-containing minimicrospheres and minitablets significantly reduce steatorrhea in a dose-dependent way. The authors provide recommendations for dosages of pancreatic enzyme preparations in the treatment of absolute and relative pancreatic insufficiency.

摘要

Tandem mass spectrometry (TMS) opens up a new era in the diagnosis of hereditary diseases of metabolism. This technology expanded the list of detected violations that are treatable, but had not previously been diagnosed. TMS is one of the most modern methods of analysis of compounds in microquantities of biomaterial, to quantify large numbers of metabolites that can serve as a markers of hereditary metabolic diseases (HMD). TMS is used for the diagnosis of the three main groups of HMDs: amino acid metabolism disorders, organic acids metabolism disorders, and defects of mitochondrial fatty acid β-oxidation. The use of this method for the detection of NBO in patients with defined clinical symptoms (selective screening) allows to increase the number of diagnosed curable diseases.

The article discusses the problem of infections caused by Clostridium difficile in children. Screening for C. difficile is indicated for children born by cesarean section, children born to mothers with a burdened anamnesis, who suffered from intrauterine infection, with dysbacteriosis, who underwent surgery, with long time staying in a hospital or transferred from other hospitals, receiving various antibacterials. Complex treatment (antibiotics+probiotics) is necessary for the treatment of children with this infection. Appointment of other drugs depends on the severity of the condition. Dynamic follow-up should be also carried out.

Effectiveness of enterosorption in children with bronchial asthma and concurrent endotoxemia

摘要

Clinical significance of endotoxemia in the development of bronchial asthma (BA) was investigated in 70 children aged from 10 months to 15 years. Of them, 50 children had BA and 20 were healthy controls. Children with asthma had elevated plasma levels of endotoxin up to 0.90±0.34 EU/ml in atopic and 1.93±0.22 EU/ml in non-atopic form (p<0.001) depending on the period and severity of the disease. The presence of systemic endotoxemia in such cases is an indication for administering Enterosgel (polymethylsiloxane polyhydrate) as a part of the complex antiallergic therapy. This resulted in a 12.8-fold decrease in plasma endotoxin levels from 0.90±0.58 to 0.07±0.03 EU/ml, a 3-fold reduction of the exacerbation length and 25-30% improvement in respiratory function. This treatment approach is highly effective and can be used in managing bronchial asthma.

Over the past decades, due to rapid development of high-tech society and the deterioration of the ecological situation in the world, diseases based on allergic inflammation, such as allergic rhinitis (AR) and bronchial asthma (BA), occupied the leading position. Currently, there is a tendency to rejuvenation and increased severity of AR and BA, and the debut of disease occurs in social promising age of adolescence. Many clinicians are faced with a problem of diagnosis of BA because of the long asymptomatic periods with no change in objective indicators. Thus, early diagnosis of BA for teenagers is extremely urgent, because appointment of adequate treatment determines the further prognosis.

The article provides an overview of new data on the mechanisms of formation and approaches to the treatment of atopic dermatitis (AD) in children. Recent research indicates that the prevention of the formation of AD can be achieved through early intervention to ensure the safety and effectiveness of the skin barrier. In children with debut of AD, it is necessary to provide therapy that will effectively control both local and systemic effects of immune activation. Early optimal therapeutic intervention can improve long-term results of AD treatment, and reduce systemic sensitization to allergens which leads to the formation of comorbid allergic respiratory diseases and gastrointestinal diseases.

The use of Lactobacillus rhamnosus GG-containing probiotics for the prevention of atopic dermatitis in children

There are qualitative and quantitative differences in the composition of the intestinal microflora in healthy infants and in children with atopic dermatitis. These data gave an impulse to development of concept that the beneficial bacteria in the intestinal microflora, called probiotics, can restore intestinal homeostasis and prevent allergies or decrease its manifestation through interaction with the immune cells in the intestine. Not all probiotic strains have the same properties. The article summarizes the current understanding of the possibilities of prevention of atopic dermatitis in children using Lactobacillus rhamnosus GG. A review of research on the prevention of atopic dermatitis in children in the prenatal and postnatal periods is presented.

Acute otitis media in children

The literature data suggest overdiagnosis of acute otitis media (AOM). The use of evidence-based clinical practice guidelines allows to accurately diagnose a disease and to avoid unjustified appointment of antibacterial drugs. Instrumental methods of diagnosis of AOM include pneumatic otoscopy, tympanometry, and spectral acoustic gradient reflectometry. Antibacterial and symptomatic therapy, including analgesics and antipyretics, eliminates general and local symptoms within 48-72 hours. Among the possible complications of AOM in children, special attention should be given to persistent loss of hearing because of great social significance in relation to the impact on language development and learning.
