Аннотация
Рассматриваются современние представленая о развитии инфекции, начиная от колонизации слизистих оболочек (носительство), включая бактериемию, и кончая острим гнойним менингитом (ОГМ), визванним комменсалами тела человека — Neisseria meningitidis, Streptococcus pneumoniae, Streptococcus agalactiae и т. п. Чтоби достичь субарахноидального пространства, возбудители острих гнойних менгингитов используют различние фактори патогенности. Подчеркивается роль бактериальних капсул. Колонизация слизистих оболочек проявляется в образовании микроколоний и биопленки. Однако лишь ограниченное число генотипов, так называемих гиперинвазивних клонов, способно пересечь эпителиальный барьер и проникнуть в кровоток. эпителиальный барьер преодолевается трансцеллюлярным путем (менингококк) или трансцеллюлярним и парацеллюлярним путями (пневмококк, стрептококк серогруппи В). В крови возбудители выживают и размножаются благодаря капсулам и системе инактивации комплемента. Гематознцефалический барьер преодолевается преимущественно парацеллюлярним путем, хотя для менингококка не исключается и роль трансцитоза. Успешное преодоление как эпителиального барьера, так и эндотелиального слоя кровеносних сосудов требует участия пилей IV типа и других адгезинов бактерий. Тесное взаимодействие микроорганизма с эндотелием мозговых капилляров приводит к активации сигнальних путей в клетке хозяина, что ведет к разрыву весьма прочних связей между клетками стенок сосуда. Подчеркнута необходимость мониторинга клональной структуры циркулирующих возбудителей острых гнойних менгингитов как важной части эпидемиологического надзора.
