Surgical rehabilitation of hearing in children with chronic suppurative otitis media.

Abstract

The main objectives of tympanoplasty are surgical debridement of the middle ear, creation of a closed tympanic cavity, creation of a mobile and functional sound transmission system. The article analyzes the state of hearing according to pure tone threshold audiometry data before and after tympanoplasty in 65 children from 5 to 17 years old. Hearing status was assessed before surgery, on days 18-21 and 6-12 months after surgery. The patients underwent tympanoplasty types I, II and III according to H. Wullstein. To close the defect of the tympanic membrane, restore the lateral wall of the attic and replenish the elements of the chain of auditory ossicles, the following materials were used: perichondrium of the tragus and auricle, cartilage of the tragus and auricle, formed perichondrial-cartilaginous flaps, periosteum. The satisfactory anatomical result of tympanoplasty in our study was 86%, the satisfactory functional result was 78%. Cartilaginous ossiculoplasty performed on both the stapes head and the oval window, as well as the use of a perichondrial-cartilaginous flap to form the tympanic membrane, is an effective method for creating a sound-conducting system of the middle ear in children.
