Summary.

The article discusses problematic issues of the unfavorable course of chronic sinusitis, requiring differential diagnosis of the secondary genesis of the process.

Chronic rhinosinusitis is a complex disease, and the EPOS2020 Steering Group decided to consider CRS from a primary and secondary perspective. The study of modern features of the genesis of chronic rhinosinusitis with its unfavorable course is becoming increasingly relevant due to its connection with concomitant diseases. An individual approach in this case will allow you to achieve more effective treatment and better results.

A six-year analysis of the causes and genesis of chronic rhinosinusitis in 772 patients (N) of the otorhinolaryngology purulent department with an unfavorable course of the disease, in which symptoms of the process persisted or recurred 4 or more times a year, despite drug treatment and drainage of the sinuses by puncture, showed that the phenotype of the unfavorable course of the disease is characterized by a variety of clinical manifestations and heterogeneity of genesis, which requires the exclusion of the secondary nature of the process in both polyposis and non-polyposis forms of the disease.
