CURRENT VIEW ON TONSILLOGENIC DISEASES - IGA NEPHROPATHY, HEMORRHAGIC VASCULITIS AND THE EFFECT OF TONSILLECTOMY ON THEIR COURSE

Abstract

IgA nephropathy and hemorrhagic vasculitis are IgA-mediated metatonsillar diseases. We analyzed the literature covering their pathogenesis, association with chronic tonsillitis (CT) and the influence of tonsillectomy (TE) on their course. It has been shown that in IgA nephropathy TE is associated with a higher rate of clinical remission and inhibition of the progression of renal failure as a result of a decrease in the mucosal production of abnormally galactosylated IgA1 molecules. This thesis is confirmed by highly evidenced studies among the Asian population and single comparative studies for the Caucasian race. Accordingly, Asian, in particular Japanese, recommendations include TE as a treatment option for this group of patients.

In hemorrhagic vasculitis, the effectiveness of TE has been confirmed only for clinical course with kidney damage. For other forms, symptomatic treatment is preferable, and TE is indicated when there is a proven association between exacerbations of CT and deterioration of kidney disease. Further research in this area is required, expanding the sample of patients with IgA-mediated diseases and possibly assessing the influence of genetic characteristics on treatment outcome.
