THE RIGHT VENTRICULAR CARDIOHEMODYNAMICS FOLLOWING CHRONIC THROMBOEMBOLIC PULMONARY HYPERTENSION

Abstract

In the review we have discussed the mechanisms of the right ventricular cardiohemodynamic changes and methods of the evaluation of the myocardial contractility following chronic thromboembolic pulmonary hypertension. In this condition the long-term elevating of the pulmonary vascular resistance causes the development of the right ventricular hypertrophy and its dilatation with the decreasing of the myocardial contractility. However, the accurate and precise measurement of the right ventricular contractility remains the unsolved problem, because in intact cardiovascular system the preload and afterload of the heart are variable parameters. The decreasing of the right ventricular contractility leads not only to the diminution of the ejection fraction, but also to the development of the cardiac diastolic dysfunction. Obviously, the perspective directions of the future investigations could be the elaboration of the new methods of the estimation of the right ventricular contractility and diastolic function following modeling of the chronic thromboembolic pulmonary hypertension in animals and future development of the echocardiography and high frequency electrocardiography methods in clinical practice.
