Диагностические, клинические и морфологические особенности хронического гепатита В - «дикой» и мутантной форм

Аннотация

В исследование включены 112 больных хроническим гепатитом В (ХГВ). Они разделены на две группы: I - 14 (24,1%) больных с классическим серологическим профилем «дикого» HbsAg-плюс, Hbe Ag-плюсАБНВе-минус-типа HBV и II – 44 больных (75,9%) мутантного HbsAg-плюсHBeAg-МинусАbНВе-плюс-типа HBV. Наличие мутаций в precore/core-зоне подтверждено частичным секвенированием вирусного генома. Выявлено, что больные мутантной формой ХГВ встречаются чаще, они старше по возрасту, у них в 2 раза дольше сроки инфицирования, в 2,9 раза чаще встречается цирроз печени, достоверно выше уровень лабораторных и гистологических показателей активности, но реже выявляется ДНК HBV в сыворотке крови, ниже вирусная нагрузка в крови и ткани печени, несмотря на это наблюдается более тесная положительная коррелятивная связь между показателями некротически-воспалительного процесса и вирусной нагрузкой, чем при «дикой» форме хронического гепатита В.
