Роль морфологических методов исследования в прижизненной диагностике хронического гепатита

Аннотация

В последние годы, после принятия новой Лос-Анджелесской классификации хронического гепатита (1994), возросла роль морфологических методов исследования в диагностике этиологии, степени активности и стадии этого заболевания. Известными гепатологами-морфологами (Knodell R. G. et al., 1981; Desmet J. V. et al., 1994; Ishak K. et al., 1995; Серов В. В., Севергина Л. О., 1996; и др.) предложены несколько полуколичественных методов оценки степени активности гепатита и стадии его хронизации. Они нашли широкое применение и признание в клинике, но не лишены недостатков и требуют дальнейшей разработки и усовершенствования. Имеются характерные гистологические черты вирусного и алкогольного поражения печени, а также их сочетаний. Различаются морфологические признаки HBV-, HCV- и HDV-инфекций в печени. С помощью полимеразной цепной реакции и метода гибридизации in situ стало возможным определение геномов вирусов непосредственно в ткани печени, возросла роль повторных биопсий для оценки эффективности противовирусной терапии. С помощью иммунофенотипирования клеток воспалительного инфильтрата в печени изучаются особенности иммуноопосредованного повреждения печени. Морфологическое исследование позволяет наблюдать апоптоз и уточнять его значение в патогенезе хронического гепатита и канцерогенезе. Компьютерный анализ видеоизображения гистологической картины обеспечивает точную количественную оценку морфологических изменений в печени. Все вышеперечисленное подтверждает неоценимую роль прижизненного морфологического исследования биоптата печени - «золотого стандарта» в диагностике хронического гепатита.
