STRATEGIES OF LUNG FUNCTION TESTING IN WORK OF THE PRIMARY CARE PHYSICIANS

Abstract
The lung function tests have become standard practice to confirm the diagnosis, evaluate the severity of respiratory impairment, assess the therapy response and follow-up patients with various respiratory disorders. Frequently, no single pulmonary function tests yields all information in an individual patient and multiple tests have to be combined to allow proper evaluation of the patients.

Abstract
Disseminated lung diseases (DLD) are heterogeneous group of the pathological conditions with a radiological syndrome of lung dissemination. The review highlights the main nozology forms of DLD, the basic clinical features, which primary care physician can face with, allowing to suspect DLD. The diagnostic procedures are defined. Approaches to therapy of DLD and prognosis are discussed.

Abstract
Spirometry one of the most popular methods of functional respiratory tests especially in the outpatient practice of primary health care. The question of the boundaries of normal spirometric parameters is essential in determining the presence or absence of pathological changes in the individual patient. The article analyzes the consistency of the various reference systems in the interpretation of spirometry results in terms of predicted values and the lower limit of normal LLN (low level of normal — LLN).

SMOKING AND MORTALITY IN A LARGE PROSPECTIVE STUDY

Abstract
Smoking is a major health risk factor. In a large male cohort the relation between smoking, smoking cessation and smoking intensity with mortality is analyzed. Follow-up period was more than 30 years. The strong relation between smoking and overall mortality was confirmed and also shown that most important smoking-related causes of death in this population were cancers of esophagus, stomach and lung and acute ischemic disease. It is shown that risk is dose-dependent and as time passes when person is not smoking risk is decreasing.

Abstract
Respiratory tract infections and acute cough are the most common reason for antibiotic prescribing in primary care. Point of care test (POCT) for C-reactive protein (CRP) may significantly reduce antibiotic prescribing for low respiratory tract infections without decreasing of quality of care and result of treatment.
